About this file
This is a Past Performance Questionnaire template for the Falmouth, MA Replacement Walk-in Freezer System solicitation (1305M326Q0471). The questionnaire is designed to be completed jointly by offerors and government assessors to evaluate contractor performance history on relevant past contracts. Offerors and their major subcontractors complete Sections I and II, providing contract details such as contractor name and address, contract number, contract type, contract value, period of performance, project description, contracting role (prime or subcontractor), and termination history. Government assessors—who may be Procuring Contracting Officers, Administrative Contracting Officers, Contract Specialists, Contracting Officers' Representatives, Technical Project Leads, Quality Assurance Specialists, DCAA Auditors, or other relevant personnel—verify the submitted information and complete Sections III and IV by identifying their role and involvement period.
The questionnaire requires assessors to assign an overall performance rating based on contract requirement fulfillment, quality and timeliness of delivery, cost management, communications, proactive management, and customer satisfaction. Rating options range from Exceptional (exceeds most requirements with few minor issues) to Unsatisfactory (does not meet requirements with serious problems), with intermediate categories for Very Good, Satisfactory, and Marginal performance, plus a Not Applicable option for irrelevant or outdated information. Completed questionnaires must be submitted to the Contracting Office at Jenna Taulman (jenna.taulman@noaa.gov), with offerors also providing copies of Sections I and II for all questionnaires with their proposals by the solicitation closing date. The questionnaire is designated as Source Selection Information for official use only.
