ATTACHMENT 2 – PROPOSAL SUBMISSION INSTRUCTIONS

FORMAT

Offer Volume. The Offeror shall confine submissions to essential matters sufficient to define the proposal and provide an adequate basis for evaluation. Offerors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each proposal. The title requirements for each file are shown below. Proposals submitted in response to this solicitation shall contain the following:

Volume 1 - Non-Price Factors:

Volume 1 - Non-Price Description:

Factor 1: Technical Capabilities

The offeror shall provide details of how they will meet the requirements listed in Section 3 of the Statement of Work.

Offeror shall complete and return the following provisions and clauses located in the SF1449:

· ATF-50

· 52.209-2

· 52.209-7

· 52.209-11

· 52.209-12

· 52.212-3

Volume 2 - Price Description:

Factor 2: Price

· The Price Proposal shall be submitted separately from the Offeror’s Technical Proposal.

· The Price Proposal shall include the following:

· Breakout of all labor costs to include (If applicable):

· Labor categories

· Labor rates

· Labor Hours

· Etc.

· Breakout of all supply costs to include (If applicable):

· Quantity

· Unit of Issue

· Etc.

· Other costs (If applicable):

· Travel

· Etc.

· SAM UEI Number shall be included to verify SAM.GOV registration.

CONFLICT OF INTEREST

Any Offeror (or member of its team and/or subcontractor) having a conflict of interest as defined under FAR Part 9.5 must identify the conflict as soon as it is known and provide a recommended mitigation plan. Mitigation plans are required whenever a competing contractor has had unequal access to non-public information regarding this requirement and/or competition or has assisted the Government in defining the requirements or evaluation criteria.

ADDITIONAL GUIDANCE

Pursuant to FAR 52.204-7, an Offeror is required to be registered in SAM when submitting an offer or quotation and at time of award (see FAR clause 52.204-13, System for Award Management Maintenance, for the requirement to maintain SAM registration during performance and through final payment).

Failure to follow instructions shall result in an Offeror being considered non-responsive. A non-responsive proposal will not be evaluated. It will not be considered for the award.
