About this file

This is a Cost Rate Sheet attachment for a Colorado Department of Public Health and Environment (CDPHE) Request for Proposals for operation and maintenance services at the North Clear Creek Water Treatment Plant (NCCWTP). The contract encompasses three primary service categories: transition and startup activities (Schedule A1), ongoing monthly operational and maintenance labor (Schedule A2), and transition and training services upon contract termination (Schedule A3). Schedule B details estimated material and supply requirements including 500 tons of hydrated lime, 4,000 pounds of polymer, 5,000 pounds of carbon dioxide, 6,000 cubic yards of sludge disposal, and various monthly recurring expenses such as laboratory analysis, internet/phone, janitorial supplies, and maintenance supplies. The contract structure combines fixed-price labor agreements with cost-reimbursable supplies and materials based on actual quantities purchased, with estimated quantities subject to change based on water quality and treatment requirements.

Schedule C establishes firm labor rates for twelve defined worker classifications (Project Manager, Principal Health and Safety Officer, Quality Assurance/Quality Control Officer, A and B Certified Industrial Wastewater Treatment Operators, Assistant Operator, Laborer, Maintenance Worker, Electrician, Instrumentation and Controls Specialist, Laboratory Technician, and Janitor), with provisions for additional classifications as needed. Offerors must include hourly and overtime rates with fringe benefits, overhead, and profit built into their pricing. The contractor is responsible for providing all necessary training, personal protective equipment, and common mechanics tools at no reimbursement cost to CDPHE. Non-routine and emergency expense allowances of $100,000 and $50,000 respectively are included as line items with the contractor's proposed markup rate applied to all Schedule B supplies and materials. CDPHE reserves the right to negotiate final scope and costs with the successful offeror and will not be liable for anticipated fees or profits on items not provided if actual quantities fall below estimates.

Cost / Rate Sheets

These cost schedules shall be completed and returned as a part of the response to the Request for Proposals (RFP).

The quantities contained in the schedules are estimated quantities only. Contractor shall be reimbursed for actual quantities purchased. Documentation of purchases must be provided.

Labor and material rates are to be considered firm offers.

The contract will be a combination of a fixed price agreement for labor and cost reimbursable for supplies and materials.

Schedule A

Schedule A is to act as Offeror's full cost of labor required to perform the Statement of Work (SOW) for one year.

Costs will be considered in the evaluation of proposals. CDPHE reserves the right to negotiate the final scope and cost of services with the successful Offeror.

The Contractor is responsible for providing all necessary training and Personal Protective Equipment for their employees. The Contractor will not be reimbursed for these costs.

Item A1 is to include the cost for preparation of the required plans, and the cost for performance of all transitional/startup functions as described in the SOW. This should be an all inclusive lump sum for labor, materials, supplies, and other costs associated with services leading up to the Offeror’s taking over operation and maintenance of the North Clear Creek Water Treatment Plant (NCCWTP).

Item A2 is to represent the proposed total monthly labor cost for operational services associated with operation and maintenance of the facility. The Offeror shall include a proposed work schedule and shall indicate the number of employees and the estimated number of work hours in the response to this RFP. Administrative and support costs are to be included.

Item A3 is to include any costs that would be in addition to the normal operating costs during the time the Offeror is preparing to transition the operation and maintenance functions to another party, including preparing the transitional plan described in the Scope of Work, and training the incoming operation and maintenance contractor. Offeror should assume these services would be performed concurrently with operating the SMSS.

Schedule B

Schedule B is intended to include a firm offer of unit prices to supply chemicals, and estimated routine expenses to complete the SOW.

Prices used shall include the Contractor’s proposed markup rate.

The quantities indicated on this sheet are estimates. Payments will be made based on actual quantities. Actual quantities may differ from that indicated and will change depending on the quantity and quality of the water to be treated. CDPHE will not be liable for any anticipated fees or profits for items not provided or performed if actual quantities are less than the estimates indicated above.

The Contractor shall provide common mechanics tools for use by their employees (e.g. hammers, screwdrivers, wrenches, etc.). The Contractor will not be reimbursed for these costs.

Schedule C

Offeror is to indicate, as a firm offer, its labor rates including fringe benefits, overhead and profit for the worker classifications that may be used on the project. Classifications on Lines H1 through H12 shall not be modified.

The Offeror should indicate the rates for other classifications that may be required. The Offeror should include rates for all key personnel. The Offeror shall use lines H13 to H25 for additional anticipated classifications to be utilized, if necessary. The Offeror should indicate the classification of all facility personnel.

The rates indicated in Schedule C will be used as the basis for developing a cost estimate for non-routine work.
