About this file
This document is an attachment related to a state and local contract opportunity for the design of a new bridge to replace one washed away in a flood on Holbrook Creek Road in Union County, Oregon. The attachment outlines the required submittal items if selected for the contract, including a breakdown of costs, certificates of insurance, a DBE breakdown and certification form, and authorization for direct deposit payments. The proposed project involves modifying the stream bed that was expanded during the flood event. The contract is being managed by the Agency, and they prefer that consultants who receive ongoing monthly payments authorize direct deposit to expedite payments to both the consultant and any subcontractors.
The document does not provide information about pricing terms, set asides, incumbents, funding requirements and sources, or other salient contract details. It focuses solely on the required submittal items for the selected consultant.

Attachment 1 – Award Submittal Requirements
Please email to me, the Contract Coordinator, the following items if marked as “REQUIRED” in the Mini‐Solicitation (see detailed requirements and due date for each item in Section 2.3 of the
Mini‐Solicitation).
Breakdown of Costs
Certificates of Insurance
COI Disclosure
Committed DBE Breakdown & Cert. Form(s)
Direct Deposit via Automated Clearing House (ACH). For Consultants that receive ongoing monthly payments from Agency on one or more contracts or work orders, Agency prefers making Consultant payments via ACH direct deposit. While not required, if Consultant has not previously authorized ACH direct deposit and would prefer ACH to expedite transfer of payments after invoices are approved, Consultant must complete a Vendor ACH Authorization form and submit to Agency per the instructions on the form. To expedite Consultant’s payments to its subcontractors, if applicable, Consultant should make reasonable efforts to offer ACH direct deposit payments.
