Tomah EHRM Training and Administrative Support Space - Site Visit

Due to COVID 19 restrictions at the Tomah VA Medical Center, they will be facilitating three

(3) separate Site Visits. COVID 19 has also created a limited capacity concerning the availability of their meeting room (a total of ten (10) outside visitors per Site Visit). Each prospective vendor will be limited to ONLY two (2) Attendees per Company and will be required to pre-register.

Vendors that would like to attend must email the Contract Specialist (CS) Thomas Council at thomas.council@va.gov. Their email request MUST be received no later than Wednesday – January 5, 2022 at 1:00 PM (ET). Request received after this date and time will not receive consideration. All requests should contain the following:

1) Company Name

2) Name and Tile of the person attending

3) Email address and phone number of the two (2) Attendees

Once your request has been received, the CS Thomas Council will respond to your email request.

He will provide you with confirmation concerning which date and time that your Company’s two

(2) Attendees have been assigned to attend. The dates and times are contained below.

Monday – January 10, 2022 at 2:00 PM (CT)

Tuesday – January 11, 2022 at 9:00 AM (CT)

Wednesday – January 12, 2022 at 9:00 AM (CT)

Participants will meet at the following location:

Tomah VA Medical Center – 500 E. Veterans Street – Tomah, WI 54660 Project section: B23, Room – 12322

COVID19 screening: All attendees must be screened for COVID19 at building 402 overhang before entering a VA building. All attendees are required to wear a mask at all times while inside the facility. Masks will not be provided by the VA. Attendees are advised to arrive early to ensure that they can be screened in a timely manner.

All campus roads are currently open with no restrictions.

After the COVID19 screening all attendees shall report to:

Use man door adjacent to entrance P6 and proceed up the stairs to the conference room.

Contractors should use parking lot P5 for initial meeting at site.

Please Note: that Attendees MUST bring their pre-registration email confirmation to the Site Visit to eliminate any confusion as to who should be attending. Due to the limited capacity of the meeting room and COVID 19 restrictions, pre-registered Attendees that fail to produce this documentation will not be permitted to participate. No Exceptions.
