SECTION I

DESCRIPTION OF SERVICES

1. Introduction: The United States Strategic Command (USSTRATCOM) has a need for specialized custodial services including window, glass, and hard surface cleaning, and specialized dusting services supporting the newly constructed USSTRATCOM Command and Control Facility (C2F). The USSTRATCOM C2F architecture includes a vast array of glass and hard surfaces which require specialized cleaning and maintenance.

2. Period of Performance: C2F window and hard surface cleaning services are planned as a 5-year multi-year contract with a base year followed by four (4) option years.

3. Scope: The contractor shall provide all necessary labor, supervision, management, transportation, supplies, and equipment necessary to deliver comprehensive window, glass, hard surface cleaning, and specialized dusting services to the United States Strategic Command located at Building 1000, 900 SAC Boulevard, Offutt AFB, NE 68113. Cleaning services shall include designated windows, glass, and hard surfaces as represented in Attachment A, USSTRATCOM C2F Exterior Glass Schedule; Attachment B, USSTRATCOM C2F Interior Glass Schedule; and Attachment C, USSTRATCOM C2F Hard

Surfaces Schedule. Attachment D is a detailed photo binder of most C2F cleaning requirements; however not all glass and/or hard surfaces are represented in the photograph binder. Attachment D is intended to clearly define the expected level of performance for designated surfaces and to clarify specific areas which must be cleaned. Images represented in Attachment D include textual overlays which clarify required cleaning scope for designated location.

Prior to beginning cleaning services, the Contractor shall perform inspections of all surfaces to be cleaned. The purpose of inspections is to identify defects or damage to window gaskets, window seals, caulking, window blinds, or similar architectural features which could adversely affect delivery of overall cleaning services and/or should be addressed before cleaning services commence. The Contractor shall note such inspections by floor and general building location (See Attachment D) and the extent of damages or defect observed. The

Contractor shall immediately report findings to USSTRATCOM Facilities Management of the Contracting Officer’s Representative.

3.1. Standard Procedures for Cleaning Architectural Glass: The Contractor’s procedures shall be compliant with industry standards for commercial cleaning of architectural glass. Industry standards include, but are not limited to, procedures published or recognized by the National Glass Association (NGA)/Glass Association of North

America (GANA); or the International Window Cleaning Association (IWCA).

3.2. Exterior Surface Cleaning: As recognized by the IWCA – and to the greatest extent possible – the Contractor shall employ ground-based cleaning procedures that rely on water-fed pole systems – or similar industry recognized ground-based cleaning technologies. USSTRATCOM security protocols strictly prohibit Contractors from working/loitering directly outside of all second and third level perimeter office windows.

Therefore, any exterior work that is performed via ladder/scaffolding, personnel suspension harnesses, man-lift devices, or similar equipment shall be approved in https://www.glass.org/ https://www.glass.org/ https://www.iwca.org/ writing by the Contracting Officer’s Representative or the USSTRATCOM Special

Security Office (SSO). Use of these devices is also subject to close surveillance by

USSTRACTCOM security personnel and U.S. Government safety managers.

USSTRATCOM C2F Exterior cleaning requirements are documented in Attachments

A, Exterior Glass Cleaning Schedule; and Attachment D, USSTRATCOM C2F

Photograph Binder.

3.3. Interior Surface Cleaning:

• Glass Surfaces: Interior glass surfaces shall be cleaned using industry recognized commercial glass cleaning procedures (NGA/GANA, or IWCA).

Glass shall be thoroughly cleaned to remove all traces of dirt, film, or similar foreign matter.

• Window/Glass Frames and Stainless Steel Trim Work: All window frames, window sills, window sashes, window ledges, interior window shelfs, door frames and stainless steel trim work shall cleaned to remove all traces of dirt film, or similar foreign matter. Cleaning of window sills, window sashes, window ledges, interior window shelves, door frames, and stainless steel trim is considered part of the scope of window cleaning. Whenever glass is cleaned, the corresponding window trim work (sills, sashes, ledges, frames, shelves, blinds, I-beams, or stainless steel) shall also be cleaned.

• Structural I-Beams: Structural I-beams extend across the width of both the East and West Atrium glass facades – extending between the North Bar and the South Bar and are considered part of the scope of periodic window cleaning. I-beams are aligned at the second and third floor levels – two (2) at the East Atrium and two (2) at the West Atrium. Horizontal cavities of structural I-beams shall be thoroughly cleaned and dusted to remove all traces of dirt, film, or similar foreign matter. See Attachment D for photograph images of structural I-beams.

• HVAC Diffusers: East Atrium; West Atrium; and Auditorium glass façades include multiple linear HVAC diffusers which are aligned horizontally along window frames on the second and third floor levels of the C2F; HVAC diffusers are considered part of the scope of periodic window cleaning. HVAC diffusers must be cleaned in a manner that minimizes penetration of cleaning fluids (e.g.

vacuumed), but ensures all dust and debris is removed from ventilation screens.

Ventilation screens shall be wiped clean following removal of dust and/or debris.

• Horizontal Cleaning (Non-glass Main Entry Vestibule Platforms and similar surfaces): Horizontal vestibule ceiling surfaces/platforms shall be thoroughly cleaned and dusted to remove all traces of dirt, film, or similar foreign matter.

Vestibule ceiling platforms are situated at the East and West main entrances.

Vestibule platforms are not rated for foot traffic; walking on vestibule platforms is prohibited!

Horizontal surfaces may also include designated painted, metallic, vinyl, wooden, gypsum board, textile, or similar surfaces. Horizontal surfaces which are not specified within this paragraph 3.3 (Interior Surface Cleaning) or in Attachments

A, B, or C shall be managed as optional cleaning services.

All surfaces shall be wiped clean and free of dust, dirt, or smudges. When required, certain surfaces may require damp mopping or wiped with a germicidal cleaner. See Attachment C, Solid Surface Cleaning Schedule; and

Attachment D, USSTRATCOM C2F Photograph Binder.

Pedestrian Bridge Glass: Four (4) pedestrian bridges are positioned at the East and West ends of the C2F Atrium and provide connecting walk-ways between

North Bar and South Bar facilities/office areas. Pedestrian bridges are positioned on both second and third levels of the building. Pedestrian bridge glass is to be cleaned on outer glass surfaces only. Inner bridge glass surfaces – surfaces which are accessible from walkways without leaning over railings – are cleaned by others. See Attachment D for photograph images of pedestrian bridge-way glass.

Interior Window Blinds/Shades: The contractor shall carefully dust blinds and operate blinds as needed to the extent necessary to effectively clean glass panes.

Special care shall be exercised to avoid damage/stains to existing blinds.

USSTRATCOM C2F Interior cleaning requirements are documented in Attachments B, Interior Glass Cleaning Schedule; Attachment C, Solid Surface Cleaning Schedule; and

Attachment D, USSTRATCOM C2F Photograph Binder.

3.4. Optional Cleaning Services: It is important to note that the scope described within the body of this statement of work does not include all glass/window surfaces. Certain surfaces are intentionally planned as options – to be executed based upon funding availability. Other glass/window surfaces are intentionally excluded and are generally the responsibility of other parties.

• Option 1 – B1000 Connecting Link Exterior Glass. Glass cleaning associated with the C2F connecting link corridor will be subject to funding availability. See

Attachment D for photograph images of the C2F.

• Option 2 – North and South Bar Penthouse Exterior Façades. Glass cleaning associated with the North and South Bar Penthouse Exterior Façades (interior and exterior) will be subject to funding availability. See Attachment D for photograph images of the C2F.

• Option 3 - Interior Cleaning Option: From time to time it may be necessary to clean interior glass or solid surfaces that are not otherwise maintained by building occupants or custodians. Therefore, the Government is incorporating an option to clean interior surfaces as measured by individual surface count (glass or hard surface). Horizontal surfaces may also include designated glass, painted, metallic, vinyl, wooden, gypsum board, textile, or similar surface. Option 3 pricing represents the marginal price for cleaning a fixed number of window panes or solid surfaces whose average measurement is 48” x 72” (4ft. x 6ft); these measurements are consistent with average interior glass pane sizes as documented in Attachment B, Interior Glass Cleaning Schedule.

3.5. Coordinating Activities: The Contractor shall coordinate all cleaning services through the USSTRATCOM Facilities Management Office or the Contracting Officer’s

Representative. Both parties can be reached by telephone at 402-912-4300. Within 10 calendar days of award, Contractor shall provide written work schedules detailing expected performance dates and the estimated date(s) of completion. The Contractor is responsible to field verify exact measurements of the areas to receive services.

The Contractor shall provide services in a manner that minimizes impact to

USSTRATCOM operations. No disruption of services serving other building functions is allowed without advance coordination.

3.6. Final Inspection and Acceptance: Upon completion of periodic cleaning services, the

Contractor shall coordinate with the USSTRATCOM Facilities Manager or Contracting

Officer’s Representative a request for final acceptance inspection. Prior to requesting/scheduling periodic acceptance inspections, the Contractor shall provide a written report which describes specific work performed (referencing corresponding contract CLINs) and any facilities defects (or similar issues) which were noted during performance. The purpose of this report is to demonstrate compliance with specific contract cleaning requirements and for the Government to initiate appropriate maintenance action to correct noted facilities defects.

3.7. Re-performance of Services: The Contractor shall ensure that all surfaces are thoroughly cleaned per the requirements of the Contract – including hard-to-reach interior and exterior surfaces. If, during the 14 calendar-day period after services are performed, the Government identifies areas which were clearly missed or poorly performed, the Contractor shall respond within 72 hours for re-performance. Examples of faulty performance include unreasonable streaks on interior or exterior glass (possibly caused by improper water treatment or ineffective cleaning procedures). Faulty workmanship also includes ancillary surfaces (window sills, ledges, shelves, HVAC diffusers) which are missed or overlooked during cleaning of windows. Performance time(s) for correction of defects will be coordinated with the CO or USSTRATCOM

Facilities Manager.

SECTION II

GOVERNMENT/CONTRACTOR FURNISHED RESOURCES AND EQUIPMENT

1. Government Furnished Resources: Government furnished resources include electricity, sewage, and water which will be made available where existing.

Restrooms and eating facilities (USSTRATCOM cafeteria) are available.

2. Contractor Furnished Equipment//Services: The Contractor shall provide all equipment, materials, parts, supplies, components, and facilities to perform the requirements of this contract. With respect to Government furnished water, the Government makes no claim as to the suitability of water for use in cleaning services. The Contractor shall be solely responsible for treatment of water to accommodate industry standard cleaning specifications

– including, but not limited to, water softening and or reducing the level of total dissolved solids to an acceptable level for commercial cleaning services (20 parts per million).

The Contractor shall ensure all employees practice utilities conservation. Alterations or temporary connections to government utilities shall be the responsibility of the Contractor;

and shall be restored to the original condition upon completion of each task/call. No temporary connections will be allowed until the Contractor has submitted a detailed plan for the connection and has received approval from the Government for the connection as planned. The plan shall include a detailed description on how the connection will be restored to its original condition or better.

The Contractor shall be responsible for safeguarding government resources provided. At the close of each work period, facilities, equipment and materials shall be properly secured.

SECTION III

GENERAL INFORMATION

1. General Information

1.1. Worksite: The worksite shall be maintained in a clean manner at the end of each workday. Contractor shall clean and restore jobsite to its original or better condition within two (2) business days after project completion.

1.2. Protection of Government Property: The Contractor shall be solely responsible while at the work site for complete protection of Government personnel, property, equipment, and buildings, as well as Contractor-owned property/equipment from injury and/or damage.

The Contractor shall provide appropriate types of shielding, barricades, warning signs and any other protection necessary to protect personnel and equipment in areas where work is performed – this includes protection from injury, damage, and/or contamination during the term of the Contract.

Unless otherwise approved by USSTRATCOM Facilities Management or the

Contracting Officer’s Representative, protection shall be installed at the beginning of the work period and removed as soon as the work is complete.

The Contractor shall be held financially responsible for any damage, as determined by the Contract Officer or Contracting Officer’s Representative, caused by the Contractor’s personnel, equipment, procedures or materials used in the performance of the work under this Contract. This includes damage to grounds and/or landscaping; damage to concrete surfaces; damage to interior flooring/carpeting; and damage to glass or similar hard surfaces. Special care must observed when maneuvering equipment inside of the C2F facility.

Equipment – including telescopic man-lifts, scissor lifts, moveable scaffolding, and similar heavy gear – require placement of temporary protective barriers

(plywood or similar hardboard covers) over existing finished floors or carpeted surfaces. Protective covers (e.g. tarps or plastic sheeting) may also be required to protect flooring or ancillary surfaces from cleaning solutions, or unintended overspray, which could occur during performance of services.

The Contractor shall immediately notify USSTRATCOM Facilities Management or the Contracting Officer’s Representative in the event of any damage to

Government property caused by the Contractor’s personnel, equipment, materials or processes, or damage discovered prior to the beginning of the work.

The Contractor shall promptly repair, or cause to have repaired, to the satisfaction of the USSTRATCOM Facilities Manager or the Contracting

Officer’s Representative, any and all damage caused by the Contractor’s personnel, equipment, including the procedures and/or the materials used by them in the performance of the work under this Contract.

1.3. Debris Removal: Unless otherwise agreed by USSTRATCOM Facilities Management or the Contracting Officer’s Representative, the Contractor is responsible for removing all waste materials and debris from C2F work location(s) and discarding at a location off the premises of Offutt AFB in accordance with all federal and State regulations.

1.4. Contractor Personnel: The Contractor shall perform adequate background checks on all employees to validate U.S. citizenship and to validate that employees are legally allowed to work in the U.S. Naturalized citizens may be required to present their naturalization certificate to be eligible to apply for a Base access pass IAW DFARS

252.203-7001 (Prohibition on Persons Convicted of Fraud or Other Defense Contract-

Related Felonies). The Contractor shall ensure that all employees and subcontractor employees have current, valid licenses and certifications necessary to perform their functions. Any cost or time associated with obtaining licenses or certifications shall be at the Contractor’s expense.

All work shall be performed in a professional manner by employees/craftsmen who are qualified in the trade which they are employed.

The Contractor shall ensure that all employees are trained and qualified to operate specialized equipment such as a scissor lift, “hi-lo”, boom lift, or any other specialized equipment as required by industry standards.

Certain activities may require the Contractor (including subcontractors) to submit documentation which demonstrates proper registration/licensing for the respective trade or equipment.

The Contractor shall immediately replace any individual not acceptable to the Government as directed by the CO.

The Contractor shall not employ any person who is currently or formerly an employee of the U.S. Government, if the employment of that person would create a conflict of interest.

1.4..1. Contract Manager. The Contractor’s Contract Manager (CM) shall be responsible for the overall performance of this contract and shall possess the necessary experience to provide direct oversight of all project activities. The CM shall have full authority to act for the Contractor. The CM shall provide overall contract management, including full authority to develop proposal responses;

negotiate and sign contracts; and provide subcontract purchasing and administration. The CM and alternate(s) – who act for the Contractor when the

CM is absent - shall be designated in writing prior to commencement of work.

Subcontractors may not perform duties of the CM. The CM shall be able to speak, understand, and read the English language.

During the course of performance, the assigned contract manager (or alternate) shall be available within one (1) business day to meet with government personnel to discuss on-going work or problem areas.

1.4..2. Superintendent. The Contractor’s Superintendent shall provide on-site supervision and visit the project site daily; submit periodic progress reports for each phase of the cleaning activities; schedule and coordinate subcontractor and material suppliers, site visits, and inspections; and attend all status meetings, site visits, and pre-final/final inspections as required. Subcontractors may not perform duties of the Superintendent. The Superintendent shall be on-site during normal work hours and for contingencies requiring work beyond normal work hours. The

Superintendent shall be able to speak, understand, and read the English language.

The Superintendent shall ensure that a complete, current set of the contract documents and approved submittals are on the jobsite at all times.

Throughout the life of this contract, the Superintendent shall be available to meet within 120 minutes of notification by the CO or USSTRATCOM Facilities

Manager regarding project activities; the CO or Contracting Officer’s

Representative must be able to access this individual at all times by phone or e-mail.

1.4..3. Subcontractors: All subcontractors shall be properly registered as in the state of

Nebraska in their respective trades/specialties where required.

The Contractor shall coordinate the work of subcontractors and shall ensure coordination of work between various trades. The Contractor is responsible for any subcontractor work performed.

1.4..4. Contractor Dress and Conduct: Employees are expected to comply with reasonable dress and grooming standards based on comfort, productivity, health, and types of positions occupied. Attire shall be in good repair and shall not be considered offensive, disruptive, or unsafe. The Government is authorized to restrict base or facility access to any contractor employee or prospective contractor employee who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. In addition, a Contractor employee may be asked to provide proof of a government-issued badge at any time; the employee must comply with such request. In the case of a Contractor employee who is working on the base in a temporary capacity, the employee will be required to show their temporary pass showing they are on base legally.

1.4..5. Smoking Restrictions: Smoking is prohibited inside of all Offutt AFB buildings/facilities and/or within fifty (50) feet of flammable or combustible liquids. Smoking is permitted only in designated smoking areas.

1.5. Site Safety:

1.5..1. Fire, Environmental, and Public Health Inspections. The Contractor is subject to routine fire, safety, environmental, or public health inspections by the CO or

Government Safety Officer without notice. Any discrepancies noted shall be corrected immediately. If discrepancies cannot be immediately corrected, the

Contractor shall remove the items from the installation. Any costs or time delays associated with correcting discrepancies are the sole responsibility of the

Contractor.

The Contractor shall assume full responsibility and liability for compliance with the applicable regulations pertaining to the health and safety of personnel during the execution of work, and shall hold the Government harmless for any action on his part or that of his employees or subcontractors, which results in illness, injury, or death.

1.5..2. Regulatory Compliance. Contractors shall comply with the Occupational Safety and Health (OSHA) IWCA Alliance program – specifically the Window Cleaning

Field Safety Guide and relevant portions of OSHA 1910 Standards that are applicable to window cleaning services.

1.5..3. Contractor’s Safety Plan: The Contractor shall establish and maintain a detailed plan or standard operating procedure for Accident Prevention, Safety, and Health

(Contractor’s Safety Plan). The Safety Plan shall be signed and approved by the

Contractor’s Safety Director. The Contractor’s Safety Plan shall clearly designate the “Competent Person” who is overall responsible for job-site safety. The

Competent Person means a person – because of knowledge, skill, training, and experience – who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees. Required competency areas include:

Electrical

Fall Protection

Ladders

Rigging

Scaffolding

The Safety Plan shall be provided as specified and submitted to the CO within 30 calendar days after contract award. The safety plan shall be subject to

Government approval by the CO. Contractor compliance with the safety plan will be monitored by the Government.

1.6. Contractor Vehicles: The Contractor shall provide and maintain vehicles for transportation to meet the requirements of this contract. All vehicles used in the performance of this contract shall have the company name prominently displayed on the vehicle and be maintained to present a clean, neat, and professional appearance at all times.

1.7. Wireless Electronics/Cell Phone Use/Photography: Cell phones (including all electronic tethered/wireless/Bluetooth accessories) are strictly prohibited within Building

1000 and within any USSTRATCOM facility. Photography of any sort is also strictly prohibited. Contractor employees shall not possess or use wireless electronic devices – including all “smart” devices (e.g. cell phones, tethered wrist watches, FitBits, cameras, electronic notepads, iPads, etc.) while working within the C2F (Bldg. 1000) or any

USSTRATCOM facility. Wireless electronic devices shall be secured outside of Bldg.

1000 and USSTRATCOM. LEAVE THESE DEVICES AT HOME OR IN A

PERSONAL VEHICLE. The use of cell phones while driving is also strictly prohibited unless using a hands free device. Texting while driving is prohibited.

1.8. Environmental:

1.8..1. Hazardous Materials: Materials classified as hazardous by U.S. and Nebraska environmental rules, regulations, or laws shall be handled and disposal IAW those rules, regulations, or laws. Any cost provisions to meet these requirements shall be the sole responsibility of the Contractor. Any project delay to meet these requirements shall also be the sole responsibility of the Contractor.

1.8..2. Environmental FAR Provisions: In accordance with FAR Clause 52.223-3, Hazardous Material Identification and Material Safety Data, the Contractor shall list any hazardous material to be delivered under this contract. For each item, the

Contractor shall submit to the CO an SDS listing any hazardous material to be delivered under this contract prior to the hazardous material entering the installation.

1.9. Performance of Services during Crisis: Services at Offutt AFB are generally not considered mission essential and only limited services will continue in the event of crisis

– whether declared by the National Command Authority, under provisions of DoDI

3020.37, or otherwise. In the event of a crisis, Contractor shall request project disposition instructions from the CO or USSTRATCOM Facilities Management.

1.10. Holidays: The Contractor is not required to provide services on the following holidays: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Memorial

Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.

1.11. Conservation of Utilities: Contractor employees shall adhere to utility conservation practices. The Contractor shall establish and implement operating procedures which facilitate energy/utility conservation.

A1. Scope: These standards apply to all new construction, demolition, rehabilitation, alteration, modification, repair, and maintenance contracts for existing facilities.

A2. Compliance with Laws and Regulations: The Contractor shall comply, and ensure that all subcontractor(s) comply, with all applicable federal, State, and local laws, regulations, ordinances and standards related to safety and security matters at Offutt AFB. The

Contractor shall further comply with security regulations imposed by the Installation

Commander and the agency occupying the space where work is to be performed, including any necessary security clearances.

A3. Contractor/Employee Base Pass and Registration:

A3.1. General: The Contractor shall comply with all requirements and procedures IAW

FAR 5352.242-9000, Contractor Access to Air Force Installations. Contractor employees shall be required to obtain and display identification badges. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination. Drive designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.

A3.2. Identification Credentials: The Contractor shall provide name, driver’s license number and date of birth of on-site workers to the designated Security point of contact

(POC). The Contractor shall complete Air Force Material Command (AFMC) Form

496, Application for AFMC Identification Card. Contractor requests shall be submitted to Pass and Registration – commonly known as “Pass and ID” which is located at

Building 478, Offutt AFB USSTRATCOM Gate. Upon approval, the Government will provide a Defense Biometric Identification System (DBIDS) Credential. The

Contractor shall ensure that all employees have the proper identification credentials prior to entering Offutt AFB, NE. All Government issued cards MUST be returned to the Pass and ID office (Building 478) upon the completion of the contract.

A3.3. Contractor Security Requirements: The Contractor shall be required to comply with all security regulations and directives as identified herein and other security requirements as are shown elsewhere in this contract. When applicable, the Contractor shall comply with DD Form 254, DoD Contract Security Classification Specification.

Identification badges shall be surrendered to Security Forces upon demand for the purpose of identification, abuse, misuse or for any other reason necessary. All identification cards are required to be returned to Security Forces upon the card expiration date.

A3.4. Display of Credentials: Contract personnel shall be required to wear and display their issued identification badge on the outside of their clothing to appropriately identify them while performing contract services on Offutt AFB.

A3.5. Controlled Area Access: Access to all controlled areas is strictly permissive and subject to direction of the CO or the authorized representative. When required and upon award of the specific contract, contract modification, or delivery order, the

Contractor shall be provided a specific named Government POC who shall provide http://farsite.hill.af.mil/reghtml/regs/far2afmcfars/af_afmc/affars/5352.htm#P249_18821

A1-2 specific instructions for accessing the controlled work areas. The assigned Government

POC will also coordinate all required escorts to project sites that are located within controlled areas.

A3.6. Lost or Stolen Identification Credentials: It is the responsibility of the individual contractor employee (card holder) to immediately notify local installation authorities (Security Forces) of a lost or stolen credential.

A3.6.1. Replacement Fees: Individual contractor employees may be assessed a fee of $50.00 for each lost (or willfully damaged) Offutt AFB Identification

Credential. This fee also applies to expired badges which are not turned in at the end of the contract period of performance. Individual employees shall be directed to the Offutt AFB Accounting and Finance Receivable Office and complete/process a DD Form 1131 – Cash Collection Voucher. The employee will produce the completed Cash Collection Voucher to Offutt AFB Pass and

Identification prior to issuing a replacement identification card.

A3.6.2. Fee Exceptions: If the identification credential is confiscated by Offutt

AFB Security Forces personnel, a receipt will be issued to the individual employee which will preclude the $50.00 charge. Individual employees will not be charged for replacement cards which are damaged due to normal fair wear and tear.

A4. Physical Security: The Contractor shall be responsible for safeguarding all Government property provided for contractor use. At the close of each work period, all Government facilities, property, and materials shall be secured.

A4.1. Key and Lock Controls: The Contractor shall establish and implement methods of ensuring effective control of keys and/or combination codes (cypher locks, combination padlocks, safes, etc.). The Contractor shall not duplicate any keys issued by the Government. The Contractor shall immediately report any occurrences of lost, duplicated keys, or improper communication of combinations.

In the event keys, other than master keys, are lost or improperly duplicated, the

Contractor may be required – upon written direction of the CO – to re-key or replace the affected lock or locks without cost to the Government. Costs associated with re-keying the affected lock or locks may also be deducted from periodic invoice payments which are due to the Contractor. In the event a master key is lost or improperly duplicated, the

Government shall replace all locks and keys for that system, and the total cost will be deducted from the payment due the Contractor.

A5. Information Security: All drawings and specifications pertaining to the B1000 complex are ‘For Official Use Only’ (FOUO). Therefore, compliance with Governmental guidance on handling unclassified but sensitive information is mandatory. Specific guidance are listed below. The Contractor shall be responsible for compliance to the below guidance of all sub-contractors and/or employees supporting the USSTRATCOM C2F. The Government may require any of the below forms to be submitted by major sub-contractors.

A5.1. Certificate Pertaining to Foreign Interests: Certificate Pertaining to Foreign

Interests: Standard Form (SF) 328 – available here – must be completed and submitted prior to obtaining FOUO documents. The form shall be signed by the Company https://www.gsa.gov/forms-library/certificate-pertaining-foreign-interests

A1-3 president or general manager on behalf of the company and employees. The Contractor will require approval from Government security representatives prior to obtaining

FOUO documents. The Government reserves the right to deny any contractor FOUO documents – which could establish the contractor as unqualified to perform the scope of this contract.

A5.2. Controlled Unclassified Information Nondisclosure Agreement: A Controlled

Unclassified Information Nondisclosure Agreement shall be completed and submitted prior to obtaining FOUO documents. This nondisclosure agreement (NDA) was developed for any contractor working with information pertaining to the B1000 complex. The form shall be signed by the Company president or general manager on behalf of the company and employees. The USSTRATCOM NDA form will be provided (and prepared) only when performance requirements dictate the need for

FOUO information. This form is not included as an attachment.

A5.3. DOD Guidance: Two DOD publications outline specific measures concerning the handling, transmittal, and disposal of information/material not intended for public release. The contractor shall comply with and actively engage in all measures outlined in the below publications. Failure to do so shall be considered a breach of the nondisclosure agreement; see section A5.2.

A5.3.1. DODI 8582.01, Security of Unclassified DoD Information on Non-DoD

Information Systems. The Contractor shall reference “Enclosure 3” of DODI

8520.01 for handling of electronic “soft copy” information. DODI 8520.01 is available on the DoD Issuance website at http://www.esd.whs.mil/DD/.

A5.3.2. DODM 5200.1, Volume 4, DoD Information Security Program: Controlled

Unclassified Information (CUI). The contractor shall reference “Enclosure 3

– Identification and Protection of CUI, Paragraph 2e (Protection of FOUO

Information) for handling of hard copy FOUO materials. DODM 5200.1, Volume 4 is available on the DoD Issuance website at http://www.esd.whs.mil/DD/.

A5.4. Contractor Validation Data: The contractor shall be required to provide written information pertaining to their own company, any subcontractors, and any potential subcontractors (to each subcontractor), and major suppliers of materials and services to each. Failure to provide data in good faith (representing the total contractor’s team) shall be considered a breach of the NDA, see section A5.2. A USSTRATCOM template form

(for Contractor Validation Data) is available upon request.

A5.5. Labeling: Products may not be labeled in a way that refers to sensitive missions or equipment, special access controls, or similar designations. Generally, this requirement is interpreted as only labeling products with room number only.

A6. Fire and Emergency Services: The Government will provide general on-base emergency services. The telephone number is 911. The Contractor shall comply with fire prevention rules and regulations enforced by the Offutt AFB Fire Prevention Branch. For routine fire prevention matters call 402-294-3778.

A6.1. 911 Emergency Services: Calling 911 from a cell phone is be received by off-base emergency 911 systems operators which can cause delays in emergency response http://www.esd.whs.mil/DD/ http://www.esd.whs.mil/DD/

A1-4 times. Whenever feasible, Contractors should use base telephones to request 911 emergency services.

A6.2. Fire Extinguishers: In the event of a fire emergency, the Contractor is authorized to use any available Class A, B or C fire extinguishers at any work site.

A6.3. Emergency Procedures: Contractor employees shall follow the direction of

Government employees in regards to emergency procedures – examples include fire, tornado, active shooter, bomb threats, etc. It is the responsibility of the Contractor to provide clear guidance and establish procedures for responding to emergency of their employees. It is the Contractor’s responsibility to account for their respective employees following an emergency evacuation. Contractor personnel shall participate with

Government personnel in all emergency services practice drills (e.g. fire, tornado, active shooter drills, etc.) and at no additional cost to the Government.

A6.4. Emergency Medical Services: In the event of a medical emergency on Offutt

AFB, the Government provides twenty-four (24) hour on-base, state certified paramedic-level emergency response and patient transport services for contractor personnel. The telephone number is 911 for emergencies. The Contractor shall reimburse the

Government for emergency medical services rendered.

A6.5. Security Police Services: The Government provides general on-base security forces' service. Security forces' telephone numbers are 911 for emergencies; 402-294-

5677 for crime in progress; and 402-294-4302 for non-emergency calls.
