I DESCRIPTION OF SERVICES
1.1. General Description of Services: The contractor shall provide all materials, labor, equipment and transportation necessary to accomplish all of the requirements defined in this Statement of Work (SOW). These requirements include removal of existing floor coverings, abatement of asbestos containing tile and mastic, and installation of resilient flooring, wall base, broadloom carpet, carpet tiles, rubber flooring, stair treads, and stair nosing.
1.1.1. The work shall be accomplished in various facilities on Wright-Patterson AFB, Ohio. Each facility may have special requirements, such a security and hours of work. The requirements shall be detailed in each individual Delivery Order.
1.1.2. All quantities (per delivery order) of the same pattern and color are to be of the same dye lot.
1.2. Materials: If other manufacturers products meet the criteria set forth in this SOW, they may be submitted as an “or equal”. A representative of 88 ABW/CEAN shall decide if a substitute is acceptable. All materials shall be asbestos free.
1.3. Burn Permit: Before the use of any open flame, the contractor shall have (at the job site) a burn permit and a 10 lb ABC fire extinguisher.
1.3.1. Each delivery order requiring the use of an open flame shall require a separate burn permit.
1.3.2. Burn permits can be obtained by calling the fire department technical services section 24 hours in advance, at 937-904-3157
1.3.3. The Wright Patterson AFB fire department shall inspect the contractor’s equipment prior to issuing the permit.
1.4. Electrical and Communications Equipment: Removal and reinstallation of outlet cover plates, tombstones, or wire-mold, shall be done by the contractor. The contractor shall not disconnect and/or reconnect any electrical and communications cables.
1.5. Removed Materials: All removed items and materials shall become the property of the contractor unless the delivery order indicates re-use, relocation, reinstallation, or turn-in of the removed material. Materials that become the property of the contractor shall be removed from the job site on a daily basis unless otherwise noted by the representative of 88 ABW/CEAN.
1.5.1. All carpet removed from the base shall be entered into a carpet reclamation program. The reclamation program must guarantee that regardless of manufacturer, fiber type or construction, the removed carpet shall does not end up in a landfill. Carpet removed as asbestos containing material shall be disposed of in accordance with Appendix A, “Removal and Disposal of Asbestos Containing Materials”.
1.5.2. All cardboard carpet tile boxes and other recyclable packaging, resulting from the installation of carpet tile and other flooring shall be entered into a reclamation program.
Use of the base recycling bins near Facility 10278 is acceptable, if available.
1.5.3. Contractor shall comply with the appendices.
1.6. Installation: All materials shall be installed per the manufacturer’s recommendations unless otherwise stated in this SOW to include Spot Prepping/Repair Floors.
1.7. Protection: Work shall be accomplished in a manner that minimizes the spread of dust to other parts of the building and in occupied portions of the work area.
1.7.1. All demolition work shall be accomplished in a manor such that existing sound materials that are to remain, or that are to be reused, shall not be damaged. Any damage caused by the Contractor shall be repaired at no cost to the Government.
1.7.2. All demolition performed in excess of this required, shall be repaired at no cost to the
Government.
1.7.3. When noted on the task order, Class II infection control measures shall be implemented (See Appendix B “Description of Required Infection Control Precautions for Class II”).
Damage to Government property during the process of implementing these measures shall be repaired by the contractor at no cost to the Government.
1.8. Resilient Flooring: All vinyl flooring shall be rated for use in exits and exit access corridors, in accordance with NFPA 101 10.2.7.2 (a)
1.8.1. Sheet Vinyl: Sheet vinyl shall be equal to “Armstrong’s Possibilities Petit Point, textile-inspired visuals in complex colors for creating sophisticated, soft-looking floors.” The product shall be available in light to mid-range colors in tone on tone combinations, styled and constructed for healthcare and leisure/hospitality spaces.
1.8.1.1. The sheet vinyl gage shall be: 0.085” (85 mils) (2.16 mm) overall Nominal, 0.050”
(50 mils) (1.27 mm) wear layer (nominal). Width: 6 feet.
1.8.1.2. The sheet vinyl shall meet or exceed the following reference standards;
ASTM F 1303, Type II, Grade 1, Class A backing Fire Test data, ASTM E 648 Critical Radiant Flux – 0.45 watts/sq.cm. or more -
Class I and ASTM E 662 Smoke – 450 or less Static Load Limit, ASTM F 970 and 500 psi (35.16 kg/sq cm)
1.8.1.3. The product shall have the capability of being heat welded for seams that resist the penetration of dirt and moisture.
1.8.1.4. Sheet vinyl flooring shall have at minimum a Limited 5-Year Commercial Products Warranty.
1.8.2. Vinyl Composition Tile: Vinyl Composition Tile shall be or equal to Armstrong Raffia
Stream with Diamond 10 Technology Coating, Standard Excelon Imperial Texture, Standard Excelon MultiColor, and Static Dissipative”. The tile shall have the following characteristics:
1.8.2.1. Colors and pattern details, dispersed uniformly throughout the thickness of the material. Color pigments are insoluble in water, and resistant to cleaning agents and light. The pattern shall last the life of the flooring material.
1.8.2.2. Three-dimensional highlights and shadows that create a rich, three-dimensional effect (equal to Imperial Texture Standard Excelon). Tile to compliment/accent the tile with three-dimensional highlights and to create a more contemporary look (equal to Standard Exelon MultiColor).
1.8.2.3. Durable and resistant to grease, alkali and dilute chemicals.
1.8.2.4. The tile shall have the following physical characteristics: 12” x 12” (305 mm x 305
mm) tile. Product Gage (Thickness): 1/8” (3.2mm), Wear Layer: 1/8: (3.2mm) Raffia only- 12” x 24” (305mm x 610mm). Gage: 1/8” (3.2mm).
1.8.2.5. The vinyl tile shall meet or exceed the following reference standards:
ASTM F 1066, Class 2 through pattern Fire Test data, ASTM E 648 Critical Radiant Flux – 0.45 watts/sq. cm. or more
- Class I and ASTM E 662 Smoke – 450 or less Static Load Limit, ASTM F 970 @ 125 psi
1.8.2.6. Vinyl Composition Tile shall have at minimum a 5 Year Commercial Resilient Limited Warranty.
1.8.3. Luxury Vinyl Tile: Luxury Vinyl Tile shall be or equal to “Tandus - Centiva Event Series
Planks, Contour Series Planks, Event Series Stones and Contour Series Stones”. The Luxury Vinyl Tile shall have the following characteristics:
1.8.3.1. Colors and pattern details; vivid colors with design variation within the full pattern, dimensional highlights and shadows that create a rich, three-dimensional effect.
Color pigments are insoluble in water, and resistant to cleaning agents and light.
The pattern shall last the life of the flooring material.
1.8.3.2. Durable and resistant to grease, alkali and dilute chemicals.
1.8.3.4. The Luxury Vinyl tile shall have the following physical characteristics:

1.8.3.5. The Luxury Vinyl Tile shall meet or exceed the following reference standards:

1.8.3.6. Luxury Vinyl Tile shall have at minimum a Limited 20-Year Commercial Products
Warranty
1.8.4. Rubber Flooring and accessories: Rubber Flooring shall be or equal to Nora Grano vulcanized rubber compound with environmentally compatible color pigments that are free of toxic heavy metals such as lead, cadmium or mercury. The Rubber Flooring must have the following characteristics:
1.8.4.1. Hammered Textured surface flooring shall have a hammered texture; Thickness
0.36 inches (9.0mm). Tile Size: 39.45 inches x 39.45 inches (1002 mm by 1002mm).
1.8.4.2. The rubber flooring shall meet or exceed the following reference standards:
Static Load Limit: ASTM F970, Residual compression of 0.005” with 800 lbs. ≤
0.005” with 250 lbs. is required.
Rolling Load Limit: ≤ 850 lbs. /sq. inch; nora polyurethane adhesive is required Slip Resistance: ASTM D2047 Static coefficient of friction, Neolite dry 0.93
Neolite wet 0.93 ≥ 0.5 is required (not recommended for ramps).
Flammability: ASTM E648; NFPA 253; NBSIR 75 950, 0.56 Class 1, ≥ 0.45 watts/sq. cm is required.
Smoke Density: ASTM E662; NFPA 256; NBS, 289 (flaming) and 373 (non-flaming) < 450 is required.
Bacteria Resistance: ASTM E2180 and ASTM G21, resistant to bacteria, fungi, and micro-organism activity.
Warranty: Limited 5-year warranty.
1.8.4.3. The Stair treads shall be One-Piece Nosing, Tread, and Riser or equal to Nora Grano Hammered Texture. Thickness: 0.20 inches (5mm). Material Length: 50.59 inches (1285mm), 63.54 inches (1614mm) and 78.89 inches (2004mm) Depth:
19.88 inches (505mm) Nosing Height: 1.77 inches (45mm).
1.8.4.4. The Stair Treads shall meet or exceed the following reference standards:
Static Load Limit: ASTM F970, Residual compression of 0.005” with 800 lbs. ≤
0.005” with 250 lbs. is required.
Abrasion Resistance: ASTM D3389 1.1 lbs. (500g) load on H-18 wheel with
1000 cycles, 0.002 oz. (0.05g) weight loss; ≤ 0.035 oz. (1.0g) is required.
Slip Resistance: ASTM D2047 Static coefficient of friction, Neolite dry 0.99
Neolite wet 0.95 ≥ 0.5 is required (not recommended for ramps).
Flammability: ASTM E648; NFPA 253; NBSIR 75 950, 0.92 Class 1, ≥ 0.45 watts/sq. cm is required.
Smoke Density: ASTM E662; NFPA 256; NBS, 267 (flaming) and 130 (non-flaming) < 450 is required.
Bacteria Resistance: ASTM E2180 and ASTM G21, resistant to bacteria, fungi, and micro-organism activity.
Warranty: Limited 10-year warranty.
1.8.4.5. The stair treads shall meet or exceed the following reference standard:
ASTM F 2169, standard specification for resilient stair treads, type TS.
1.8.4.6. All rubber flooring shall meet the following:
Flammability: ASTM E 84, Class A.
Smoke Density: ASTM E 84, Class A.
1.8.4.7. Stringers shall be, or equal to, Roppe: Thickness .080 inches, 10” x 72” or 12” x 72” or Johnsonite: Thickness .080 inches, 10” x 72”.
1.8.4.8. Primers and adhesives shall be type recommended by the resilient flooring manufacturer for the specific material and location.
1.8.5. Cove base shall conform to ASTM F1861 Type TP, top set coved four (4) inches and six (6) inches high, 1/8-inch thick
1.8.5.1. Provide all of the manufacturer’s standard color selections (18 or more).
1.8.5.2. A minimum of 18 inches shall be maintained between joints.
1.9. Reducer Strip: Reducer shall be made of heavy-duty vinyl or rubber and shall provide a smooth transition between surfaces of varying heights, such as carpet to tile, Roppe #50 rubber or #177 vinyl, or equal. Color to be selected from manufacturer’s standard samples.
1.10. Carpet: Carpet shall meet the technical and aesthetic criteria listed below. Carpet, carpet cushion
(if used), and flooring adhesives shall be certified as CRI “Green Label” or “Green Label Plus.”
1.10.1. Manufacturer shall provide patterned carpet tile that is tufted or woven.
1.10.1.1. Manufacturer shall provide patterned carpet tile with level loop, multi-level textured loop, cut and loop, and/or tip sheared.
1.10.1.2. Manufacturer shall provide patterned carpet fiber that is 100% bulk continuous filament.
1.10.1.3 Manufacturer shall provide patterned carpet with 100% nylon type 6, 6 or nylon type 6 fiber
1.10.1.4 Manufacturer shall provide patterned carpet with face fiber that is solution dyed or bleach resistant.
1.10.1.5 Manufacturer shall provide patterned carpet that has a minimum density of 5000 calculated using the following formula: (Average pile yarn density = 36 x pile weight (oz. /yd.
sq.) / pile thickness in inches).
1.10.2 When purchasing carpet, the carpet shall be purchase in accordance with the Carpet Ordering Guide attachment. The Carpet Ordering Guide provides for required sources and ordering procedures via pre-existing government contracts with pre-established prices.
1.10.2.1. Changes to the current Carpet Ordering Guide and the existing government carpet contracts may occur during the duration of this contract. If such changes occur, they shall be incorporated via a bilateral modification and the contractor shall comply with the future requirements.
1.10.2.2. The contractor shall only be reimbursed for the cost of the carpet ordered in accordance with the Carpet Ordering Guide. No overhead, indirect costs, profit, general and administrative costs, shipping, storage or other costs shall be recognized with the orders off the Carpet Ordering Guide. Any such costs should be built into the other costs bid for this effort in the Bid Schedule attachment.
1.10.3 Waiver to required sources: If a waiver is received from AFICC, which allows for the purchase from other sources than those required by the Carpet Ordering Guide, the carpet shall meet the technical and aesthetic criteria listed below. Carpet, carpet cushion (if used), and flooring adhesives shall be certified as CRI “Green Label” or “Green Label Plus.”
1.10.3.1. Construction of broadloom carpet and carpet tile shall be tufted, woven, needle bonded or fusion bonded.
1.10.3.1.1. Fibers shall be continuous filament nylon 6.6 or nylon 6 with a cross-section modification ratio no greater than 2.5.
1.10.3.1.2. Pile height shall comply with Americans with Disabilities Act Accessibility Guidelines (ADAAG), Section 4.5.3 Carpet.
1.10.3.1.3 Carpet shall meet the following Pile weight density stated in appendix D.
1.10.3.1.4. The minimum acceptable gage is 3.175 millimeters (0.125).
1.10.3.1.5. The minimum acceptable tuft bind is listed below, as tested by ASTM D1335, Standard Test Method for Tuft Bind of Pile Yarn Floor Coverings:
10 pounds for loop pile broadloom 3 pounds for cut pile broadloom
8 pounds for modular carpet tile
1.10.3.1.6. Both primary and secondary backings shall be a synthetic material.
Jute back is not acceptable.
1.10.3.1.7. Attached open-cell urethane cushion or closed cell vinyl backing is acceptable.
1.10.3.1.8. Color fastness to light (fading) shall comply with American Association of Textile Chemists and Colorists (AATCC) Test Method (TM) 16E, Color fastness to Light, and have a minimum of 4 after 40AATCC fading units using the AATCC gray scale for color change.
1.10.3.1.9. The maximum Electrostatic Propensity shall be 3.5 Kilovolts (kV) or less.
1.10.3.1.10. Carpet shall meet ASTM E648-00, Standard Test Method for Critical Radiant Flux of Floor-Covering Systems Using a Radiant Heat Energy Source, and ASTM D662 and meet the following criteria: Minimum average critical radiant flux of 0.45 watt per square centimeter (Class1) or greater within fire egress routes in unaccompanied personnel housing, child development centers, detention and correctional facilities, lodging facilities and hospitals. If protected by a sprinkler system, Class II may be installed. Minimum average critical radiant flux of 0.22 watt per square centimeter (Class II) in all other facilities.
Smoke Density, when necessary to meet specific application, shall be tested in accordance with ASTM E662 and shall have a maximum specific optical density not exceeding 450 (flaming mode exposure).
1.10.3.1.11. Carpet submitted for use in medical and child development facilities shall feature antimicrobial effectiveness and electrostatic propensity guaranteed for the life of the carpet. These carpets shall also be solution dyed or bleach resistant. The carpet must have a soft non-abrasive texture.
1.10.3.1.12. Stain Resistance. Carpet shall meet a minimum rating of 8 using the AATCC red 40 stain scale after 2 HWE (hot water extraction) using the AATCC 171, Carpets: Cleaning of; Hot Water Extraction Method, test method followed by the AATCC 175, Stain Resistance: Pile Floor Coverings, stain test.
1.10.3.2. Carpet shall be patterned, or a bold tweed. (Solid colors are acceptable for border carpet only.) Tone-on-tone coloration does not meet the criteria as a multi-colored carpet. Random multi-color patterns are preferred.
1.10.3.2.1. Light colors and high contrast bold geometric patterns are not
1.10.3.2.2. Linear patterns and/or patterns which appear to be linear are not
1.10.3.2.3. A bold tweed shall contain a minimum of three (3) distinctively different colors.
1.10.3.3. All carpet shall have a 15-year wear warranty against 10 percent loss of face fiber, edge raveling, snags, picks, runs, and delamination.
1.10.3.4 All carpet shall have an Appearance Retention Rating (ARR) of Heavy or Severe. (See Appendix D)
1.11. Porcelain tile: Porcelain tile shall be or equal to Daltile Colorbody porcelain. The Porcelain flooring shall have the following characteristics:
1.11.1. Colorbody Porcelain shall have continuous colored stains from the glaze surface throughout the body of the tile
1.11.2. The Porcelain tile shall meet or exceed the following reference standard:
1.11.2.1. ASTM C373 Water Absorption < 0.5%
1.11.2.2. ASTM C648 Breaking Strength > 275 lbs
1.11.2.3. ASTM MOHS Scratch Hardness 8.0
1.11.2.4. ASTM C650 Chemical Resistance- Resistant
1.11.2.5. A DCOF value of ≥ 0.42 is the standard for tiles specified for level interior spaces expected to be walked upon when wet, as stated in ANSI A137.1-2012, Section 9.6.
1.11.3. All porcelain shall have a minimum of a 1 year warranty.
1.12. Porcelain Accessories:
1.12.1. Porcelain Cove Base to coordinate with porcelain flooring.
1.12.2. Schluter RENO transition or equivalent. Prevents tile edges from chipping and provides a smooth transition between flooring types.
1.12.3. Schluter RONDEC joint or equivalent. Prevents tile edges from chipping.
1.12.4. Schluter DILEX joint or equivalent. Provides a transition for floor and wall transitions.
Prevents tile edges from chipping.
1.13. Carpet Installation: All carpet shall be installed per the manufacture’s recommendations using full spread glue method.
1.13.1. Whenever possible, the contractor shall use low emissive, water clean-up products, especially in occupied work areas. All adhesive shall be approved by the flooring manufacturer as acceptable for the installation of their product.
1.13.2. At the end of each work day, all newly installed carpet shall be cleaned with a commercial beater-type vacuum cleaner.
1.13.3. All carpet installation shall have a one-year warranty.
1.14. Porcelain Installation: All Porcelain shall be installed per the manufactures recommendations.
1.14.1 Grout Joint shall be 1/8” or according to the manufacturers recommendations.
1.14.2 Grout shall either be epoxy or power grout according to the severity of moisture in the space (i.e. bathrooms require epoxy)
1.14.3 Thinset Mapei or equivalent
1.14.4 Tile size options: 12” x 12”, 12” x 24”, 6” x 36”, 24” x 24” or greater. Tiles 24” x 24” or greater require additional floor prep.
1.15 Porcelain Base Installation:
1.15.1 Grout to be either epoxy or power grout according to the severity of moisture in the space (i.e. bathrooms require epoxy)
1.15.2 Thinset Mapei or equivalent
1.16. Ceramic/ Porcelain Flooring Demo: All existing ceramic/porcelain tile flooring shall follow General Environmental Protection Requirements (WPAFB), as spelled out in Appendix F.
1.17. Ceramic/ Porcelain Base Demo: All existing ceramic/porcelain tile base to follow General
Environmental Protection Requirements (WPAFB), as spelled out in Appendix F.
1.18. Furniture Lift System: The contractor shall be able to install modular carpet using a hand-controlled vertical lifting system. The lifting system shall be capable of:
Lifting systems’ furniture without disassembly of the furniture or damage to the furniture and its components Lifting systems’ furniture without disconnecting power, communications or computer lines
1.18.1. The contractor shall be trained by the manufacturer of the lift system, giving the contractor access to new tools and techniques.
1.19. Asbestos Containing Materials: All floor-tile that is under carpet or being removed shall be considered asbestos containing material unless otherwise noted on the task order. During the removal of flooring material, asbestos containing floor tile and/or mastic may come up with the flooring material. If there is more than 10% of the floor tile and/or mastic coming up with the flooring material being removed, or the floor tile under the flooring being removed is in poor condition, the contractor shall stop work immediately and notify the Project Manager 88 CEG/CENMP. The Government shall test the floor-tile and mastic for asbestos. Depending upon the results, work shall resume or a change order shall be written to have the floor-tile removed by the contractor’s licensed abatement contractor. If change order is needed, then the contracting office shall be notified by 88 CEG/CENMP.
1.19.1. The contractor shall have the capability to remove asbestos containing flooring material and/or mastic. They shall follow Appendix A, Section 02081 Removal and Disposal of Asbestos Containing Material.
II. GOVERNMENT FURNISHED EQUIPMENT AND SERVICES
No Government-furnished equipment or services shall be provided.
III. GENERAL WORK REQUIRMENTS
3.1. The contractor shall provide a Contractor Representative (CR) for the effective period of the contract. The CR shall be able to respond in person to Wright Patterson AFB within twenty-four
(24) hours, and respond by telephone within four (4) hours of a telephone request. This request can be for any purpose relating to the contract. (Some examples are future task order estimating, and previously completed unsatisfactory workmanship.) If the CR should change, the Contracting Officer shall be notified in writing as soon as possible, but no later than two (2) working days prior to the current CR leaving and the start date of the new CR taking over. No lapse shall be allowed between CRs.
3.2. A flooring installation contractor job-site foreman is required for each job and shall be certified with Certified Floor Covering Installers (CFI) at a C-II level (minimum).
3.3. The contractor shall have the capability and experience required to purchase various brands and types of flooring.
3.4. Prior to the CR’s submission of proposed quantities, the CR shall visit the premises with the project inspector to become thoroughly familiarized with all details of the work and working conditions. The CR shall verify all dimensions. The contractor shall be specifically responsible for the coordination and proper relation of all work specified within each delivery order, including neat appearance, correct fitting, required quantities, etc.
3.5. The contractor shall have the capability to receive, warehouse, and cut carpet in order to facilitate
Just-in-Time delivery. Storage requirements shall vary depending upon the quantity of flooring required to complete individual or multiple delivery orders and the installation schedule. The contractor shall be able to concurrently complete up to 15 delivery orders.
3.5.1 The contractor shall store all materials in a climate controlled, safe, dry, clean and well ventilated area. Materials shall be protected from damage, soiling, and moisture. All products shall be stored in accordance with the manufacturer’s recommendation. All damage during or after delivery and before final inspection shall be the responsibility of the contractor and shall be repaired at no cost to the Government.
3.5.2. The contractor shall be able to provide package labels and original wrappings if requested by the COR.
3.5.3. The contractor shall deliver the materials to the delivery location just-in-time for installation. Materials shall not be stored at installation areas. Carpet shall be cut off site and transported to the job site daily. There shall be no Government-provided forklift or transportation of flooring products.
3.6. The contractor shall be responsible for all own quality control in accordance with its company
Quality Control Plan. After the contractor has made a quality control inspection and any necessary corrections (prior to invoicing), the CR shall make an appointment with the representative of 88 ABW/CEAN to perform the final inspection. The project inspector, with the CR in attendance shall perform the final inspection. The CO shall notify the CR in writing of incomplete and defective work. If deficiencies exist, the contractor shall remedy the work within two (2) working days and the re- performed work shall be re-inspected.
3.6.1 Asbestos abatement work shall be accomplished and accepted in accordance with section
1.2.11 of this SOW and Appendix A.
3.7. The contractor shall guarantee the installation for a period of one (1) year from the date of final Government acceptance of the completed installation. The contractor shall promptly and properly repair and/or replace, to the satisfaction of the project inspector, any and all improper work that may become apparent within the guarantee period. Defective workmanship causing tripping hazards or blocking a path of exit shall be repaired within 24 hours.
3.8. Hours of work shall vary per individual delivery order, depending on the facility, and the organization within the facility. It is mandatory the contractor shall be able to work any hours within a seven (7)-day work week. The delivery order shall state when the work is to be completed to include night or holiday hours. The COR and the CR shall negotiate the required labor hours prior to award of the task order).
3.8.1. Day work shall be from 6:00 am to 5:59 pm.
3.8.2. Night work shall be from 6:00 pm to 5:59 am.
3.8.3. Holidays are limited to include New Years Day, Memorial Day, Independence Day, Labor
Day, Thanksgiving Day, and Christmas Day. If holidays fall on a Saturday; it shall be observed on the immediate previous Friday. If holidays fall on Sunday, it shall be observed on the following Monday.
3.8.4. When carpet tile is being installed in conjunction with lifting system’s furniture, the entire
24-hour day shall be considered day work and the hours of work shall be determined by the
COR.
3.9. VOLUNTARY PROTECTION PROGRAM
3.9.1. APPLICABILITY: Wright-Patterson AFB is in the process of pursuing VPP recognition or has already been recognized under the OSHA Voluntary Protection Program (VPP). VPP impacts all “applicable contractors” operating on Air Force Installations. The contractor shall be responsible for ensuring their employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. The contractor shall follow the safety and health rules of the installation or VPP site. Detailed information on VPP is available on the OSHA website at http://www.osha.gov/dcsp/vpp/index.html.
3.9.2. DEFINITIONS:
3.9.2.1. Applicable Contractor -- A contractor whose employees worked at least 1,000 hours at the site in any calendar quarter within the last 12 months and is NOT directly supervised by the applicant (installation).
3.9.2.2. Days Away, Restricted, and or Transfer Case Incident Rate (DART) -- The number of recordable injuries and illness cases per 100 full-time employees resulting in days away from work, restricted work activity, and/or job transfer that a site has experienced in a given time frame.
3.9.2.3. Total Case Incidence Rate (TCIR) -- Total number of recordable injuries and illness cases per 100 full-time employees that a site has experienced in a given time frame.
3.9.2.4. Voluntary Protection Program -- The Voluntary Protection Program (VPP) promotes effective worksite-based safety and health. In the VPP, management, labor, and OSHA establish cooperative relationships at workplaces that have implemented a comprehensive safety and health management system. Approval into VPP is OSHA’s official recognition of the outstanding efforts of employers and employees who have achieved exemplary occupational safety and health.
3.9.3. SAFETY & HEALTH PLAN. The contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the Contracting Officer not later than 10 calendar days after contract award. The contractor’s plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. The plan shall:
a. Demonstrate a management commitment to employee safety and health;
b. Identify the application of the safety and health plan to subcontractors;
c. Identify the roles and responsibilities of the following individuals with respect to safety and VPP: Management, Supervisors, Employees, and Safety Coordinator;
d. Identify applicable safety rules and regulations;
e. Include a worksite hazard analysis to include base-line hazard identification and required control measures;
f. Include a job site analysis to include hazards of tasks required to control measures;
g. Identify employee safety and health training requirements and the documentation process;
h. Include a workplace inspection frequency, to include identifying the individual conducting the inspections;
i. Include employee hazard reporting procedures;
j. Identify individual(s) responsible for corrective action of hazards;
k. Identify first aid/injury procedures;
l. Identify procedures for accident investigation and reporting;
m. Identify emergency response procedures; and
n. Identify the process for tracking controlled hazards in contractor work areas.
3.9.3.1. The contractor is required to submit their Total Case Incidence Rate (TCIR) and
Days Away, Restricted, and or Transfer Case Incident Rate (DART) rates and OSHA Form 300A annually to the contracting office for consolidation and submission as part of the installation’s annual VPP Safety and Health Management report. TCIR and DART rates are due by the 15th of January of each year.
3.9.3.2. The contractor shall establish these VPP requirements for all subcontractors who qualify as applicable contractors under the resulting contract.
3.10 Enterprise-Wide Contractor Manpower Reporting Application (eCMRA)
3.10.1. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields at https://www.sam.gov.
3.10.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Should contractors have issues using SAM, they should contact the Federal Service Desk at https://www.fsd.gov.
3.10.3. *Reporting Period: Contractors are required to input data by 31 October of each year.
3.10.4. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

REMOVAL AND DISPOSAL OF ASBESTOS CONTAINING MATERIALS (WPAFB)

PART 1 - GENERAL
1.01 SECTION INCLUDES This specification pertains to the removal, handling and disposal of
Asbestos Containing Materials (ACM). This includes all labor, materials and equipment required to prevent a release to the environment and protect personnel from exposure to airborne asbestos fibers.
1.02 APPLICABLE PUBLICATIONS: The contractor shall have at least one copy of all applicable publications listed below available at the job site at all times.

1.03 QUALIFICATIONS
A. The contractor shall have a current license as an Asbestos Hazard Abatement Contractor issued by the Ohio Department of Health (ODH).
B. The designated competent person shall be employed directly by the Contractor and certified as an Asbestos Hazard Abatement Specialist by the ODH. The competent person shall be listed on the ODH notification and on site during all abatement activities.
C. The Certified Industrial Hygienist (CIH) shall be currently certified by the American Board of Industrial Hygiene in comprehensive practice.
D. The Industrial Hygiene (IH) technician or Air Monitoring Technician (AMT) shall be certified as an Asbestos Hazard Evaluation Specialist by the ODH.
E. The testing laboratory shall be a current participant in the National Institute of Occupational Safety and Health (NIOSH) Proficiency Analytical Testing (PAT) Program for Asbestos Analysis. The testing laboratory shall also be accredited by the American Industrial Hygiene Association (AIHA) Program for Phase-Contrast Microscopy (PCM) analysis and the National Voluntary Laboratory Association Program (NVLAP) for Transmission Electron Microscopy (TEM) analysis.
F. The landfill used for disposal shall be EPA approved for all friable and category II non-friable
ACM. Category I non-friable ACM may be disposed of in a sanitary landfill.
1.04 DEFINITIONS
A. Terms used in the specification section applicable to asbestos abatement work shall be as defined in 29 CFR 1926.1101.b, 40 CFR 61.141, and Ohio Administrative Code 3745-20-01.
B. CIH is defined as an individual certified in comprehensive practice by the American Board of
Industrial Hygiene, 6015 West St. Joseph, Suite 102, Lansing, MI 48917 3980.
C. The following terms are defined in the Ohio Revised Code, Section 3710.01 through 3710.99:
1. Asbestos Hazard Abatement Contractor
2. Asbestos Hazard Abatement Specialist
3. Asbestos Hazard Evaluation Specialist
4. License
5. Certificate
D. ACM is any material containing over 1% asbestos. This includes Category I and Category II, regulated and non-regulated, friable and non-friable asbestos.
E. Critical Barrier is defined as an air tight seal over an opening in a floor, wall or ceiling including but not limited to: pipe or duct penetrations, doors, windows, air diffusers, immovable objects and carpeting. Critical barriers are separate from the negative pressure enclosure and shall be sufficient to prevent airborne asbestos in a work area from migrating to an adjacent area. All critical barriers shall be constructed using polyethylene (poly) with a minimum thickness of six (6) mils and sealed with duct tape.
F. Hazardous Material (HAZMAT): Any chemical, substance or material in any quantity or volume as defined by the Occupational Safety and Health Administration in 29 CFR 1910.1200, the EPA in 40 CFR 302 and 261, and the Department of Transportation in 49 CFR Part 172.
G. On-site: Within the immediate working area.
H. RACM is Regulated ACM. This is generally limited to Category II material and/or Category I material that has become friable.
1.05 REQUIREMENTS
A. The contractor shall be responsible for designing the systems listed below in complete conformance to the requirements of 29 CFR 1926.1101 and 40 CFR 61, Subpart M. The contractor shall comply with all non-mandatory appendices unless express written approval to deviate has been received from 88 ABW/CEAN. The contractor shall comply with additional requirements for performance of the work as noted in the specifications and project drawings.
1. Exposure monitoring system shall be approved by the CIH. On-site analysis is prohibited for all air sampling except personal samples. An Asbestos hazard abatement air-monitoring technician or IH as defined by OAC 3701-34-1 shall be required to be on-site during all removal operations and final air sampling. During all Class I and Negative Pressure Enclosure (NPE) removal operations and during background and final air sampling involving Class I and NPEs, an IH meeting the requirements of OAC 3701-34- 11 must be on-site.
2. Engineering Control
a. Engineering controls and work practices shall fully conform to the procedures in
Appendix F of 29 CFR 1926.1101 and shall be treated as a mandatory requirement.
An NPE as described in 29 CFR 1926.1101 (g)(5)(i) and Appendix F of 29 CFR
1926.1101 is required unless otherwise indicated on drawings and/or approved in writing by 88 ABW/CEAN
b. Prohibit the use of glove-bags outside of an NPE (as described in 29 CFR 1926.1101
(g) (5) (i) and Appendix F of 29 CFR 1926.1101) unless otherwise indicated on drawings and/or approved in writing by 88 ABW/CEAN
3. Hygiene Facilities
a. Showers shall be portable type, shall comply with OSHA 29 CFR 1910.141.D3 and shall be contiguous both to the equipment room and the clean change room. The decontamination unit shall be contiguous to the containment unless otherwise approved by a representative of 88 ABW/CEAN
b. The clean/change room shall be constructed of six (6) mil poly, be at least 24 square feet in size with no dimension less than four (4) feet and be equipped with appropriate means for storing street clothes (up off the floor)
c. Contractor shall supply soap, towels, and a warm water supply for decontamination procedures for abatement workers, IH technicians and contract inspectors. Provisions for heated water must be made by contractor as an on-site warm water supply may not be available
d. Provide a positive back flow prevention control device at each hose connection to the facility water supply system
e. Waste water shall pass through a five (5) micron filter before being discharged into the facility sanitary system
4. Personal Protective Equipment
a. Disposable coveralls, gloves, boots and head covering shall be supplied for abatement workers, IH technicians and contract inspectors
b. Respirators shall be approved/selected per OSHA 29 CFR 1926.1101 (h)
B. All visual inspections and clearance requirements performed by 88 ABW/CEAN shall require a minimum of 24 hours prior appointment. Contractor shall be responsible for setting up an appointment with 88 ABW/CEAN for clearance inspections and review of air sample clearance results through the Civil Engineering Contract Inspector.
1.06 PERFORMANCE REQUIREMENTS
A. The contractor shall be responsible for compliance with all mandatory requirements of EPA
40 CFR Section 61, Subpart M and 29 CFR 1926.1101. All non-mandatory appendices of 40CFR 61 shall also be complied with unless express written authorization to deviate has been obtained from 88ABW/CEAN.
B. The contractor shall have one copy of all applicable publications listed in paragraph 1.02, copies of the approved work plan, Ohio Environmental Protection Agency (OEPA) and ODH notifications, Wright-Patterson Air Force Base (WPAFB) specification section 02-08-10, current ODH Certifications, medical examinations and respirator fit tests results for all personnel available at the job site at all times.
C. If suspected or presumed ACM not covered by the drawings or specifications are encountered, the contractor shall immediately notify the Contracting Officer and a representative of 88 ABW/CEAN.
D. The contractor shall perform demolition/renovation work without damage or contamination of adjacent work areas. Where such areas do become damaged or contaminated, the work areas shall be restored to its original condition at the Contractor's expense.
E. Non-abatement work shall not be performed in an asbestos regulated area unless otherwise approved by a representative of 88 ABW/CEAN.
F. Daily inspection checklists (provided by the Government) shall be completed and signed for each work shift by the IH technician or competent person when an IH is not required to be on-site.
G. No site work shall begin prior to approval of the pre-abatement submittal requirements, listed in see section 1.08.A of this specification.
1.07 PERMITS AND NOTIFICATIONS
A. The Contractor shall be responsible for obtaining all required permits for removal, transport and disposal of ACM. The Contractor shall also be responsible for complying with the notification requirements of the OEPA Regional Air Pollution Control Agency (RAPCA) and the ODH as the law requires. A copy of all notifications and delivery receipts shall be sent to 88 ABW/CEAN at the time notification is given.
1. OEPA
a. OEPA notifications shall be sent to the following address by certified mail or hand delivered to:

b. A copy of the notification and the certified mail receipt or a copy of the notification with the "Postmark and Date Received" blocks filled in shall be submitted to 88 ABW/CEAN concurrent with the notification being submitted to RAPCA
2. ODH
a. ODH notifications shell be sent to the following address:

b. A copy of the ODH notification shall be sent to 88 ABW/CEAN
c. If using the ODH Online Notification option, the contractor shall make a copy of the completed notification form and submit to 88 ABW/CEAN
1.08 SUBMITTALS
A. Pre-Removal
1. Precise Site Drawings
a. Scaled site drawing
b. Regulated area and/or demarcation of the enclosure
c. Hygiene facilities
d. Negative pressure system, AFD exhaust port locations, and air flow patterns
e. Location, size and construction of all critical barriers
f. All ACM locations identified as "to remain" or "to be removed". Identify the material i.e., 6-inch diameter pipe insulation, 1-inch thick ceiling tile, etc.
g. Location and size (sq. ft.) of regulated area
i. Either Bldg. and room number or
ii. Street names and/or intersections and distance and direction from street centerline for ACM insulation utility lines. See CE Spec 02-08-50 for utility lines
h. Copy of all asbestos related blueprints

5. Material Storage and Use:
a. The contractor is required to ensure that HAZMATs are used appropriately, in accordance with the manufacture's MSDS and label, so as not to expose Government employees or disrupt the work environment (office or building evacuations, etc.)
b. The contractor shall follow manufacturer's guidelines for control of humidity, temperature, cleanliness, and material handling
c. The contractor shall procure, use, handle and store all HAZMAT IAW all Federal, State, local, and Air Force laws, policies, regulations and procedures, including: AFI 32-7086 Hazardous Material Management, AFI 32 7086_AFMCSUP1 Hazardous Material Management, AFI 32-7086_WPAFBSUP1 Hazardous Material Management, the WPAFB Installation Hazmat Management Program (IHMP) Plan, AFI 32-7080 Pollution Prevention, AFJMAN 23-209 Storage and Handling of Hazardous Materials, and AFI 40-201 Managing Radioactive Materials in the USAF
d. The contractors shall furnish all equipment necessary to manage HAZMATs. This equipment includes, but is not limited to, approved chemical security cabinets, locks, secondary containment, spill response equipment, fire extinguishers, and personal protective equipment, to securely manage HAZMATs at the project site
B. Post-Removal
1. Background (taken prior to set up), personal, daily work area, and final air sampling reports prepared by the IH Technician and certified by the Laboratory Analyst and CIH.
All reports shall include name of abatement contractor, building number, and room/containment number, and sampling date, date of analysis and chain of custody for each sample.
2. Daily inspection checklists (provided by the Government) shall be completed and signed for each work shift by the IH Technician and competent person for all Class I work and whenever a NPE is utilized. For all other Classes of work the daily inspection checklist shall be completed and signed by the competent person.
3. Air pressure differential strip chart recordings
4. WPAFB Asbestos Containing Waste Manifest signed by a representative of 88
ABW/CEAN, the transporter(s) and the disposal site certifying the amount of ACM waste.
NOTE: If a contractor generated manifest(s) has already been submitted, provide the WPAFB manifest number(s) for each shipment.
5. Post-abatement submittal requirements listed above, except number 4, shall be submitted within 10 working days from the completion of the asbestos removal portion of the project.
1.09 DISPOSAL OF REGULATED ASBESTOS CONTAINING MATERIAL (RACM)
A. All Regulated ACM and debris resulting from abatement work shall be considered RACM and be handled as such during the removal and transportation process and shall be disposed of as RACM in an EPA approved landfill.
1. RACM may only be stored for consolidation with other RACM for shipment to an EPA approved landfill at a later date with prior written approval from 88 ABW/CEAN.
a. When storing and consolidating small amounts of RACM on WPAFB for shipment at later date a secured and locked transport vehicle or dumpster shall be properly marked with class nine placards while on WPAFB
b. An "Internal Manifest" provided by 88 ABW/CEAN is required when transporting
RACM from within any part of the base that requires transport upon a public thoroughfare
