Statement of Work

Fall Protection Systems Phase III

STATEMENT OF WORK (SOW)

Fall Protection Systems Phase III

1. OBJECTIVE

1.1. Contractor shall provide all material, transportation, labor, equipment, coordination, planning, and supervision necessary to purchase and install permanent type roof fall protection system on buildings identified by Anniston Army Depot (ANAD). All work shall be performed in accordance with the contract specifications, this statement of work, drawings, fall protection system specifications, and technical requirements.

2. CONTRACTOR’S POINT OF CONTACT

2.1. The Contractor shall provide the Contracting Officer (KO) with a point of contact and a telephone number for the Government to make any arrangements needed. The KO will supply the Contractor with the name and telephone number of his/her designated Contracting Officer Representative (COR) and Contract Specialist. The KO, COR and Contract Specialist shall serve as the Contractor’s point of contact with the government for this task order. The Contractor shall not seek direction or guidance with regard to this task order from anyone else but the designated KO, COR and Contract Specialist. The Contractor shall not proceed with any verbal or written change(s) to the scope of work from sources other than the Contracting Officer, who will provide changes in writing.

3. PERIOD OF PERFORMANCE

3.1. The period of performance for this project shall be 180 calendar days from Notice to Proceed

(NTP). All work to include time required to process any coordination for materials or utility outage requests must be scheduled to complete within this timeframe. Any modifications or extensions will be requested through the KO or representative for review and discussion.

4. BID SCHEDULE

4.1. Contractor shall provide pricing per each building.

5. WORK TASKS

5.1. The contractor shall purchase and install permanent rooftop fall protection system on the exterior of buildings identified by Anniston Army Depot (ANAD) in accordance with Occupational Safety and Health Act (OSHA), American National Standards Institute (ANSI), U.S. Army Corps of Engineers (USACE) Safety PAM 385-1, and Anniston Army Depot (ANAD) Policy. Fall protection system construction shall be weather resistant.

5.2. The fall arrest system shall be Miller Shock Fusion style horizontal lifeline system (or equivalent). The system shall meet American National Standards Institute (ANSI) Z359 standards and be sealed by a Professional Engineer.

5.3. Contractor shall not begin until substrates have been properly prepared.

5.4. Contractor shall prepare surfaces using the methods recommended by the manufacturer for achieving the best result for the substrate under the project conditions.

5.5. Contractor shall install in accordance with manufacturer’s instructions.

5.6. Contractor shall protect installed products until completion of project.

5.7. Contractor shall touch-up, repair, or replace damaged products before substantial completion.

5.8. Field Measurements: Where handrails and railings are indicated to fit to other construction, contractor shall check actual dimensions of other construction by accurate field measurements before fabrication. Guardrail system shall be factory finished powder-coated paint, safety yellow color.

Safety Yellow specified by Architect.

5.9. Contractor shall provide freestanding pedestrian egress barrier system on roof, including railings, bases, and hardware.

1. Standards: System shall have top and mid rail in accordance with OSHA Standards - 29 Code of Federal Regulations (CFR) 1910.29 (b).

2. Structural Load: 200 pounds (lb) (90.7 kilograms (kg), minimum, in any direction to all components in accordance with OSHA Regulation 29 CFR 1926.502.

3. Height: 42 inches (1067 millimeters (mm), nominal.

4. Railings: 1-5/8 inch (41 mm) Outside Diameter (O.D.) x .065 wall hot rolled pickled electric weld steel tubing, free of sharp edges and snag points.

5. Mounting Bases: 104 lb. Class 30 gray iron material cast with four receiver posts. Provide rubber pads on bottom of bases.

6. Receiver Posts: Shall have a positive locking system into holes that allow rails to be mounted in any direction. Receiver posts shall have drain holes.

7. Accessories:

a. Toe Board Brackets: Provide brackets as manufactured by Safety Rail

Company LLC as required.

b. Finishing Rail: D-shaped railing extension for ladder landings, length of rail section and D-loop as indicated on the Drawings as required.

c. Ladder Spanner Brackets: Provide brackets at ladder transition points as required.

8. Hardware: Securing pins shall be 1010 carbon steel, zinc plated, and yellow chromate dipped. Pins shall consist of collared pin and latch.

9. Product origin: Railings and Base Castings are specified as 100% made in USA. Manufacturer must provide steel mill and foundry certificates for verification prior to shipment.

10. Quality/Standards Certifications: Manufacturer must be American Welding Society (AWS) Welding Certified for Welding Standards AWS D1.1 & AWS D1.3 Third party qualification documentation required prior to shipment.

11. Roof pads shall be provided under each base to protect roof membrane.

5.10. When installing guardrail systems, if contractor attaches system to the roof and it’s not free standing than the contractor shall ensure that the roof is sealed watertight after installation is complete.

5.11. The safety swing gates/ Aerial lift access gates shall be YellowGate style gates (or equivalent). Gates shall be factory finished powder-coated paint, safety yellow color.

5.12. Horizontal lifelines shall be installed 15 feet from buildings ends.

5.13. Building 2: Provide and install 7 each fall protection tie off anchor post. Roof type is thermoplastic polyolefin (TPO).

5.14. Building 113: Provide and install 665 linear feet of handrail, 4 each Aerial lift access gates (3 feet).

Roof type is thermoplastic polyolefin (TPO).

5.15. Building 114: Provide and install 720 linear feet of handrails install on top of parapet walls, 520 linear feet of handrails, 1 each swing gate (3 feet), 3 each fall protection tie off anchor post. The 520 linear feet of handrails shall be installed on a thermoplastic polyolefin (TPO) roof type.

5.16. Building 126: Provide and install 1,386 linear feet of handrail, 1 each Aerial lift access gate (3 feet), 2 each set of crossover steps with handrails.

5.17. Building 127: Provide and install 1,392 linear feet of handrail, 1 each Aerial lift access gate (3 feet), 3 each single post tie off points, 2 each set crossover steps with handrails.

5.18. Building 130: Provide and install 1,834 linear feet of handrail, 4 each Aerial lift access gates (3 feet), 2 each set of crossover steps with handrails. Roof type on main roof is standing seam and the roof type on the additions is thermoplastic polyolefin (TPO).

5.19. Building 133: Provide and install 30 linear feet of handrail. Roof type is thermoplastic polyolefin

(TPO).

5.20. Building 133 test cells: Provide and install 148 linear feet of handrail. Roof type is thermoplastic polyolefin (TPO).

5.21. Building 134: Provide and install 1720 linear feet of handrail, 2 each Aerial lift access gates (3 feet), 2 each set of crossover steps with handrails. Roof type is thermoplastic polyolefin (TPO).

5.22. Building 135: Provide and install 1392 linear feet of handrail, 1 each Aerial lift access gate (3 feet), 2 each set of crossover steps with handrails. Roof type is thermoplastic polyolefin (TPO).

5.23. Building 161: Provide and install 250 linear feet of handrail, 1 each Aerial lift access gate (3 feet).

Roof type is standing seam.

5.24. Building 398: Provide and install 525 linear feet of handrail, 1 each Aerial lift access gate (3 feet).

Roof type is thermoplastic polyolefin (TPO).

5.25. Building 400: Provide and install 195 linear feet of handrail, 1 each Aerial lift access gate (3 feet).

Roof type is thermoplastic polyolefin (TPO).

5.26. Building 401: Provide and install 275linear feet of handrail, 1 each Aerial lift access gate (3 feet).

5.27. Building 402: Provide and install 274 linear feet of handrail, 1 each single post tie off point, 1 each swing gate (3 feet). Handrails shall be installed on thermoplastic polyolefin (TPO) roof type and single post tie off point installed on R-Panel roof type.

5.28. Building 440: Provide and install 284 linear feet of handrail. Contractor shall remove existing fall protection cables, leave post that are attached to roof and install the new handrails. Roof type is thermoplastic polyolefin (TPO).

5.29. Building 474: Provide and install 1 each single post tie off point. Roof type is standing seam.

5.30. Building 475: Provide and install 1 each single post tie off point. Roof type is standing seam.

5.31. Building 499: Provide and install 65 linear feet of handrail. Roof type is thermoplastic polyolefin

(TPO).

5.32. Building 513: Provide and install 1,830 linear feet of handrail, 4 each Aerial lift access gates (3 feet), 1 each 48-inch x 60-inch handrail for hatch with swing gate, 1 each single post tie off point.

Roof type is thermoplastic polyolefin (TPO) on two roofs, R-Panel on one roof, and standing seam roof type on tallest building.

5.33. Building 527: Provide and install 315 linear feet of handrail, 1 each Aerial lift access gate (3 feet).

Roof type is R-Panel high rib metal.

5.34. Building 688: Provide and install 227 linear feet of handrail, 1 each Aerial lift access gate (3 feet).

5.35. For buildings with skylights (translucent light panels) on rooftops, the contractor shall be responsible for insuring that employees are provided with adequate protection and conform to the safety guidelines set forth by the Occupational Safety and Health Act (OSHA), local depot safety rules and regulations.

5.36. A fall protection system shall be installed and maintained by the contractor during the performance of the contract. The contractor shall implement fall protection controls based on the type of roof being constructed and work being performed. The contractor shall evaluate the roof area to be accessed for its structural integrity including weight-bearing capabilities for the projected loading. The contractor shall be responsible for ensuring that all employees conform to the safety guidelines set forth by the Occupational Safety and Health Act (OSHA), local depot safety rules and regulations. Before work begins the contractor shall submit a written plan for approval detailing the fall protection system. The fall protection system shall comply with all OSHA regulations, guidelines, or specifications.

5.37. The contractor shall ensure that a qualified person over sees work on this project. The qualified person shall be certified in accordance with ANSI/ ASSE Z490.1-2009, ANSI/ ASSE Z359.6-2009, and ANSI/ ASSE Z359.2-2007 and must be current. The qualified person shall have at a minimum 15 years of experience working with Fall Protection systems that comply with Federal OSHA standards and applicable ANSI Standards. The qualified person shall be familiar with the hazard survey as required by ANSI Z359.2 and provide examples where it was used. The contractor shall ensure that all other workers are competent person trained in fall protection.

5.38. When weather conditions worsen (precipitation, thunder-storms, tornados, snow/ice storm) work on the roof will cease after the roof is secured and made watertight until operations can resume.

The contractor shall provide adequate protection of both the interior and exterior of the building and contents thereof from inclement weather and from all other damages that might occur as a result of the contractor work.

5.39. Work near or around any electrically energized item shall only be performed after electricity to that item has been shut off by Anniston Army Depot (ANAD). Contractor shall notify COR to have electricity shut off. No work will be done over or around any power service entrance with power on.

Disconnection of utility services, with related equipment, shall be accomplished in accordance with the applicable sections of the National Electrical Code.

5.40. Contractor shall be responsible for damage to government property caused by the operation and associated work.

5.41. All actions performed must adhere to all federal, state, local, Depot, and equipment manufacturer’s rules and guidelines. Contractor shall verify all conditions in the field and report discrepancies to the COR for clarification.

6. DELIVERY, STORAGE, AND HANDLING

6.1 Contractor shall deliver materials to the job site in good condition and adequately protected against damage as handrails are a finished product.

6.2 Contractor shall inspect rail sections for damage before signing the receipt from the trucking company. Truck driver shall note damaged goods on the bill of lading if damaged product is found.

6.3 Contractor shall store products in manufacturer's unopened packaging until ready for installation.

6.4. Contractor shall palletized and labeled products by roof level or designated drop zone.

7. REFERENCES/STANDARDS

8. REQUIREMENTS

8.1. Contractor shall provide as-built drawings (one hard copy and one DVD formats) upon completion of work describing any and all details beyond those specified in the statement of work.

8.2. Contractor shall provide a draft DD 1354 30 days prior to project completion to COR for Real Property Coordination. Final DD 1354 shall be submitted on or before project completion date to same.

8.3. All equipment and associated parts installed by the Contractor shall remain the property of the

Government and shall at no time be removed from ANAD without approval.

8.4. Contractor shall prepare and submit a critical path method schedule (preferably in Gantt chart or MS Project format) with target dates and milestones which shall, at a minimum, meet the above identified days for completion 14 days prior to Notice to Proceed (NTP).

8.5. The contractor shall provide a minimum of 2 hours of operator training after system has been installed and proof tested. Training shall be conducted at the installation site with the users of the systems.

8.6. Contractor shall provide manufacture’s two (2) year warranty.

8.7. Contractor shall remove all debris and salvage items from jobsite daily and transport it to the designated disposal areas on depot. All items removed from a building remain the property of the Government.

8.8. The contractor shall attend a Pre-Construction Conference held by the government prior to starting work. Work shall NOT begin until this meeting is held. This meeting serves to answer any questions from the contractor and to discuss building issues such as safety, storage, security, access, and working hours.

9. PROJECT SUBMITTALS

9.1. Contractor shall provide one hard copy and one electronic copy of all submittals to the COR, courtesy copying Contract Specialist for review and approval prior to construction. All submittals shall be in PDF format as well as hard copies as required.

9.2. Contractor shall provide certification and evidence of experience that contractor meets requirements in section 5.37 (submitted with solicitation response).

9.3. List of Submittals:

9.3.1. Product Data, Shop Drawings, and Manufacturer’s Instructions. (14 days prior to Notice to

Proceed)

9.3.2. Systems Manual to include Maintenance Procedures, Operation Procedures, Installation Certificate stamped by Professional Engineer, Product Certificate, and As-Built Drawings. (14 days prior to completion date)

9.3.3. Site Specific Safety Plan. (14 days prior to Notice to Proceed)

9.3.4. Site Quality Control Plan. (14 days prior to Notice to Proceed)

9.3.5. Lightning Protection Plans/Drawing. (14 days prior to completion date)

9.3.6. Project Schedule. (14 days prior to Notice to Proceed)

9.3.7. Manufacturer’s warranty. (14 days prior to completion date)

10. FINAL ACCEPTANCE

10.1. Upon completion of the project, the Contractor must schedule a Final Inspection walkthrough with the COR to the satisfaction of the Government. This final project walkthrough must demonstrate that all contract work has been completed, including any deficiencies known to exist in accordance with the contract and scope. Once all contract work has been fulfilled, project acceptance will be granted.
