Statement of Work Recreational Site Janitorial Services
A. Objective:
The Bureau of Land Management (BLM), Hassayampa Field Office (HFO), located at 2020 E. Bell Rd, Phoenix AZ, 85022 is looking to secure Janitorial Maintenance services at eight (8) of its recreational sites located within the HFO area of responsibility.
B. Scope:
The BLM Hassayampa Field Office administers eight recreation sites in need of janitorial services. All sites are located north and west of Phoenix in Maricopa and Yavapai counties.
C. Specifications:
“Maintenance” is described as: Provide all labor, transportation, equipment, materials, and supplies necessary to:
C.1. Clean, disinfect, remove trash, and replace/restock consumables, in and around eight vault toilet buildings
Big Bug Trail Head
Restroom facility is a single vault toilet waterless restroom including toilet riser, toilet paper holder, grab bar, and 1,000-gallon waste vault.
Silver Creek
Restroom facility is a single vault toilet waterless restroom including toilet riser, toilet paper holder, grab bar, and 1,000-gallon waste vault.
Table Mesa, Little Pan Staging Area
Restroom facility is a double vault toilet waterless restroom including toilet risers, toilet paper holders, grab bars, and 1,000-gallon waste vaults.
Badger Springs Trailhead
Restroom facility is a double vault toilet waterless restroom including toilet risers, toilet paper holders, grab bars, and 1,000-gallon waste vaults.
Church Camp Road Recreational Shooting Site
Restroom facility is a double vault toilet waterless restroom including toilet risers, toilet paper holders, grab bars, and 1,000-gallon waste vaults.
Boulders OHV Staging Area

Harquahala Backcountry Byway Restroom facility is a single vault toilet waterless restroom including toilet riser, toilet paper holder, grab bar, and 1,000-gallon waste vault.
C.1.1. Clean and disinfect:
Completely clean and disinfect all surfaces to include floors, walls, ledges, windows, toilet seats, risers, and grab bars using a germicidal detergent. After cleaning, surfaces will be free of deposits, dirt, streaks, and odors. Disinfection must be completed to CDC standards using appropriate chemicals.
C.1.2. Sweep and Mop Floor:
After sweeping and mopping, the entire floor surface shall be free from litter, dirt, dust and debris. Floors shall have a uniform appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, film or standing water.
C.1.3. Remove Trash:
All trash located on the floor or in the area surrounding the vault toilet building shall be collected, removed, and legally disposed of. The area shall be left clean and free of foreign matters.
C.1.4. Restock consumables:
All consumables will be refilled/restocked. Toilet paper holders are padlocked and require a key or combination to open.
Each vault holds 6 rolls of toilet paper.
It is the Contractor’s responsibility to supply all consumables, such as but not limited to toilet paper, trash bags, cleaning supplies/cleaners, water, and equipment to perform these cleaning duties.
There are no government supplied goods or services. None of the facilities have power or water available.
C.1.5. Maintenance Visit Logs:
Maintenance visit logs detailing the date and time of service shall be kept in each toilet building. Personnel shall fill out and initial the log after each visit.

D. Contractor-Furnished Items The Contractor shall provide all equipment, supplies, management, supervision, personnel, and transportation necessary to assure that all services are in accordance with the contract and all applicable laws and regulations. The Contractor shall ensure all work meets performance standards specified in this Statement of Work (SOW). The Contractor’s efforts shall provide the service to meet the requirements for high quality care, sensitivity to proper scheduling to meet client needs, and sanitation for infection control purposes. All supplies provided by the Contractor must be accompanied by a Safety Data Sheet (SDS) as appropriate for the product/supplies.

F. Schedule:
Harquahala Backcountry Byway- Once per week annually.
All other locations- Twice per week October through May.
Once Per week June through September.

G. Period of Performance:
All work shall begin within 7 days of the date of award and continue a total of 365 days with the option to extend services one year.
H. Delivery and Performance Location:
Work shall be accomplished during normal business hours. Any work required to be performed outside normal business hours shall be discussed and coordinated with the Contracting Officer
Representative (COR) prior to work being performed.
All work shall be completed between 6:00AM and 5:00PM.
I. Project Management:
The Contractor will provide overall project management and coordination.
The Contractor shall identify and provide a single point of contact (POC) to provide overall management and supervision during the period of performance of this contract. The single POC will communicate and manage the relationship with BLM representatives.
All pertinent communications should be written or confirmed in writing via email. The single
POC shall attend and participate in necessary progress meetings and write notes pertaining to the meeting(s). The notes shall be scribed into meeting minutes with action items clearly defined, with distinct due dates, and distributed to all designated BLM representatives via email.
J. Site Visit:
Site visits are optional but encouraged prior to submitting a bid.
K. Kick-Off Meeting:
The Contractor and the COR shall meet for a mandatory kick-off meeting upon execution of the award. During this meeting all questions shall be addressed along with determination of execution of the requirements. The Contractor shall provide to the COR a list of personnel who will be working at the job site within three (3) calendar days from award. The Contractor shall work with the COR to coordinate access to each location.
L. Government Holidays:
The Contractor agrees to observe the following Government holidays for onsite personnel:

The Contractor shall observe any other day designated by Federal statute, Executive Order or the Presidential proclamation.
When any such day falls on a Saturday, the preceding Friday is observed; when any such day falls on a Sunday, the following Monday is observed. All personnel assigned to perform under this contract shall limit their observation of holidays to those set forth above.
M. Recycling, Trash Removal and Disposal:
The Contractor shall clean up and dispose of waste materials or trash in accordance with this paragraph and full applicable federal, state, and local regulations, standards, codes and laws.
The selected Contractor shall at all times keep the job site clean and free from accumulations of waste material or trash.
Waste materials removed from the job site shall be disposed of in accordance with the above.
Any fees or charges to be paid for disposal of materials shall be paid by the Contractor
N. Protection Any accidental damage incurred to existing conditions will be responsibility of the Contractor to repair or correct.
O. Standards of conduct The Contractor shall maintain satisfactory standards of employee competency, conduct, cleanliness, appearance, and integrity, and shall take an necessary disciplinary action to uphold these standards.
Personnel shall conduct themselves in a professional and respectful manner at all times while on BLM-managed lands.
Interactions with visitors and Department of the Interior employees shall reflect professionalism, courtesy, and a commitment to quality service.
Personnel shall present a neat appearance, wear Contractor-provided identification, and restrict their presence only to areas necessary for the performance of contract duties.
The BLM may require the Contractor to remove any employee who fails to meet these standards. The Contractor shall promptly provide a replacement to ensure there is no interruption to services.
P. Safety, Health, and Environmental Concerns The Contractor shall take all reasonable and proper safety precautions to prevent death or injury to any person or damage to any property at the specific location, and in particular all equipment used by the Contractor shall be used in such a manner and maintained so as to minimize the danger of accident, death, injury, loss or damage arising from the use of such equipment. In addition to relevant statutory requirements, standards and other provisions of this Contract, the Contractor shall have the following requirements:

The Contractor’s personnel shall be knowledgeable with and adhere to all relevant occupational health and safety regulations.
Ensure that the Contractor’s personnel comply with all safety procedures and requirements.
Ensure that the Contractor’s personnel are adequately trained and instructed in the safe and correct usage, handling and operation of materials and equipment relevant to the task order requirements.
Ensure the Contractor’s personnel are certified as having completed occupational health and safety training and have been issued all the necessary Personal Protection Equipment
(PPE).
Q. Inspection
The COR will complete site visits twice per month to verify compliance with the contract requirements. If any work is found to be incomplete or below the required quality standards, the Contractor shall be given three (3) days to complete rework.
The Government reserves the right to conduct site inspections at any time.
R. General Information The Contractor will report any unusual or extraordinary issues encountered during the performance of the work, including any public or safety incident that requires follow up by the BLM (e.g., vandalism, hazardous conditions).
All vehicle work must be completed from the existing roads, driving off road is prohibited for any reason.
The Contractor shall assign a Project/Program Manager designated as key personnel on this contract. No substitution shall be made without notifying the COR; notification shall take place within one (1) business day of such substitution.
Project materials and execution shall comply with the following:
1. Americans with Disabilities Act (ADA)
2. Occupational Safety and Health Administration (OSHA)
3. Center for Disease Control (CDC)
S. Deliverables
1. Certificate of Insurance (COI): Contractor shall submit their COI
2. Monthly invoices will be submitted for payment using the federal electronic invoice processing and payment system, with appropriate information including the dates of service.
3. Sign and date logs at each site to show attendance and completion of duties after each visit.
Logs must be attached to the interior side of each door.
