SECTION C-2

REQUIRED SERVICES

2.0 GENERAL INFORMATION. Contractor shall furnish all items needed to perform this contract according to all its terms.

2.1 Monthly inspections will be conducted to identify pest activity and replace, replenish or service all pest remediation equipment at the hospital facility.

2.2 The contractor shall provide the client with a paper copy of a service report that will be placed in a logbook on site for your facility's records. It will contain a summary of any rodent or general pest activity or any other material findings indicating a risk of any infestation that may impact human health and safety. Paper copies of safety data sheets for rodenticides and insecticides applied on the property will also be made available in the logbook.

2.3 Bait stations containing rodenticide will be placed by harborage sites around the interior and exterior perimeter of the facility where permissible by the product label and applicable state and federal laws. Bait stations will be used in the mechanical rooms on the lower floor. Additionally, Rodenticide bait or tracking powder is generally not recommended for mouse control inside buildings because of the potential odor from dead mice behind walls. Trapping is preferred but can be combined with rodenticide baits as long as the rodent cannot exit the trap.

2.4 Rodents will be captured using strategically placed devices consisting of but not limited to glue boards and glue boards placed inside multiple catch metal mouse traps.

2.5 Cockroach pheromone traps will be strategically placed in potential harborage sites to monitor for cockroaches. We will place a stern emphasis on sanitation and prevention. No pesticides are used in this monitoring equipment.

2.6 Other pantry pests and structural pests including but not limited to roaches, larder beetles, and carpenter ants will be treated using the most effective insecticide sprays, aerosols, gel baits and dust containing insecticides. Safety Data Sheets will be provided.

2.7 Any common bed bug infestation will be treated using either thermal remediation (heat treatments) or a low volume biological spray that utilizes a non-chemical fungal spore as the active ingredient. The fungal spores remain active for 90 days as long as the product remains undisturbed and the areas where it is applied remain below 77 degrees Fahrenheit. The biological spray may be used as a primary stand-alone treatment if necessary; however, we strongly recommend using heat treatments followed by the biological spray treatment whenever possible to achieve longer lasting results.

2.8 Heat treatments for bed bugs may be conducted using our Temp Air thermal remediation equipment if the structural design and existing safety protocols of the hospital facility allow the Eagle Pest Control technician to provide temporary electrical service consisting of a generator placed outside of the building and power distribution cables placed through an open window or door in a safe manor so the Temp Air heating units may be properly powered. The Temp Air technology can provide relief from bed bugs in as little as 7 to 10 hours; however, this method may take longer for larger infested areas. If the structural design of the facility or safety protocols make temporary electrical service for the Temp Air equipment infeasible, we have smaller more discrete thermal remediation equipment that may be powered using electricity provided by the hospital facility's electrical service. This method may take up to 24 hours or more depending upon the size of the treated area and the availability of electricity within and or surrounding the treated area.
