Performance Work Statement (PWS)

1) Requirement Title: Uninterruptible Power Supply (UPS) Replacement

2) Background: The Veterans Healthcare System of the Ozarks (VHSO) has one Uninterruptible Power Supply System in use on 24/7 basis. The GE Uninterruptible Power Supply System MODEL LP33-100KVA is in Building 01-Room 034 which serves OI&T Network. (VHSO), located at 1100 N. College Ave., Fayetteville, AR 72703.

3) Purpose: The purpose of this Performance Work Statement (PWS) is to procure services for Uninterruptible Power Supply Replacement in support of VHSO operations.

4) Scope of Work / Performance Objectives A) The Contractor must furnish all labor, material, equipment, supplies, transportation, management and supervision necessary to perform de-installation and disposal of the current UPS system, and installation of the new UPS system as outlined in this Performance Work Statement (PWS). Materials supplied by the Contractor must be of a quality to conform to applicable Federal and State specifications.

B) All equipment shall be maintained to operate in a correct and safe manner as intended and designed by the Original Equipment Manufacturer (OEM).

C) Contractor shall ensure new UPS System and any related equipment will fit in area of previous UPS System.

6) Place of Performance A) Veterans Health Care System of the Ozarks (VHSO) Main Campus 1100 N. College Avenue, Fayetteville, AR 72703

7) Period of Performance See contract Section B.2 PRICE/COST line-item schedule.

8) General Requirements A) Existing UPS system must be disposed of to a location licensed by the State where performance occurs following all Federal, State and local licenses and regulations for such purposes. The Contractor must obtain all necessary permits for disposing of the existing UPS system. All costs associated with permits, licenses, and insurance shall be at the Contractor's own expense.

B) Contractor must coordinate all services with the COR, and communicate all questions, concerns, or issues to the COR. The CO must approve any request, or proposal for changes in project scope or additional requirements, in writing prior to the Contractor proceeding with any new work. Any Contractor changes without prior written approval by the CO are unauthorized. For unscheduled events, the Contractor must respond within 48 hours of the notification from the COR or his/her designee.

C) Contractor must plan contract work activity that interferes with Medical Center (MC) operations and must not commence without prior COR written approval at no additional expense to the Government.

D) All contractor employees must be easily identifiable at all times via a distinctive uniform, badge, or other identification method.

E) Contractor must make every reasonable effort to protect VHSO property from damage. Any property, including but not limited to, electrical, natural gas, medical gas, communication and utility lines, landscaping, walkways, roads, and curbs damaged by Contractor must be repaired or replaced at no additional cost to the Government. Government property must be protected from damage at no additional cost to the Government. Anything remaining in place that is damaged or defaced by the Contractor must be restored by the Contractor to the pre-existing damaged or defaced condition at no additional cost to the Government.

9) Constraints A) The Contractor must comply with all Federal, State, local, VA environmental and hazardous laws, regulations, directives and applicable instructions pertaining to the services required herein and to the conduct and qualifications of personnel in performance of this contract. The VA directives can be obtain at (wwwl .va.gov/vapubs/). The Contractor must utilize safe products and equipment and qualified personnel to ensure the health and safety of all in compliance with the Environmental Protection Agency (EPA) (www.epa.gov/) and the Occupational and Safety Hazard Administration (OSHA) (www.osha.gov/).

10) Hours of Operation A) Regular Business Hours: Site work must be scheduled during regular business hours, 0600 to 1600 hours, Monday through Friday.

B) National Federal Holidays are excluded. If a holiday falls on Sunday, the following Monday will be observed as the National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government agencies. Scheduled services should be completed on the first business day following any federal holiday.

11) Security Requirements A) While on campus, Contractor may be subject to wear a visible VHSO issued identity Badge. Contractor must report lost Badge(s) immediately to the COR and document on Badge Log. Contractor must return Badge(s) immediately on day of departure to COR and document on Badge Log. Contractor must return remaining Badge(s) at contract closeout to COR. Contractor must reimburse the Government a $100.00 fee for each lost or unreturned Badge.

B) Contractor must be escorted by COR or VHSO personnel to take job site related photos. Contractor must not photograph any patients, personnel, visitors, and others on VHSO property.

C) Contractor personnel will have no access to the VA network, systems, or patient information. The Certification & Accreditation (C&A) requirements of VHA Handbook 6500.6 do not apply to this requirement; therefore, a Security Accreditation Package is not required.
