About this file

This document is a Past Performance Questionnaire for Solicitation 2031JW26Q00008, issued by the Office of the Comptroller of the Currency (OCC) for Uniform Commission Examination (UCE) Hotel Venue Support. The solicitation is a combined synopsis/request for quote (RFQ) for commercial services, seeking hotel and lodging support for a uniform commission examination. The solicitation is not set-aside and uses NAICS code 72111, with a closing response date of November 12, 2025, at 10:00 AM EDT.

The questionnaire provides a detailed template for references to evaluate a potential contractor's past performance across four key factors: quality of product/service, timeliness, business relations, and problem resolution. References are asked to rate performance using a scale from Excellent to Unsatisfactory and provide comments. The document is designed to help the OCC assess a contractor's capabilities for hotel venue support by collecting comprehensive performance feedback from previous government and commercial contracting offices.

To Quoter:

Please provide this Past Performance Questionnaire package to each of your “Reference” government / commercial contracting offices according to the instructions in the Instructions to Quoters of the Solicitation.

To Quoter’s Reference:

The Office of the Comptroller of the Currency (OCC) Office of Management – Acquisition Management requests your assistance in providing past performance information on the Contractor (a prospective “Quoter” under our Solicitation) and corresponding contract listed herein. Please complete this Questionnaire in accordance with the instructions set forth immediately below.

Acquisition Management agrees to protect your information as For Official Use Only/Procurement Sensitive Information consistent with the Federal Acquisition Regulation (FAR) 3.104, as noted in the header on each page of the Questionnaire package.

Instructions for Quoter’s Reference:

1. If you are providing a reference based upon a Federal Government contract, then please complete the attached Past Performance Questionnaire regarding the Quoter based upon your most recent Contractor Performance Assessment Report (CPAR) prepared in accordance with FAR 42.15; provided, you do not actually enter your CPARs into the Contractor Performance Assessment Reporting System (CPARS). Alternatively, if you are providing a reference for a non-federal Government contract, then please proceed with completing the past performance questionnaire.

NOTE: If your office enters its CPARs into CPARS, please so advise the Acquisition Management contact to that effect rather than completing this Questionnaire. This will enable Acquisition Management to retrieve your latest CPAR for the Quote on-line from CPARS directly.
