ATTACHMENT 1 – PROPOSAL SUBMISSION INSTRUCTIONS

FORMAT

Offer Volume.

Only the lowest priced quote will be reviewed for technical acceptability. If it’s found to be technically acceptable, award will be made to that offeror without further evaluation of the other quotes received. If the quote is technically unacceptable, then the next lowest priced quote will be evaluated for technical acceptability until a technically acceptable quote is found.

The Offeror shall confine submissions to essential matters, sufficient to define the proposal and provide an adequate basis for evaluation. Offerors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each proposal. The title requirements for each file are shown Table below. Proposals submitted in response to this solicitation shall contain the following:

Non-Price Factors Description:

Factor 1: Technical Capability

The offeror shall provide an organization structure, including the organizational chart and supporting narrative description of offeror’s proposed installation personnel, and any major subcontractor(s) and their key personnel assigned to performance of this project.

The proposal must demonstrate how the offeror will meet and perform every aspect of Section 4.0 in the SOW.

Factor 2: Past Performance The Government may seek relevant performance information on all responsive and otherwise Technically Acceptable Offerors through sources such as Contractor Performance Assessment Reporting System (CPARS), or other government data bases and other sources that are available. The government shall not be limited in its search for performance data. The offeror shall not submit performance data for this factor. When performance is evaluated, the outcome is either Acceptable or Unacceptable. Unacceptable performance will disqualify an offeror from award.

Price Factor Description:

Factor 3: Price

· The price proposal shall be submitted separately from the offeror’s technical proposal.

· The offeror pricing proposal shall include the following:

· Breakout of all material costs

· Breakout of all labor costs to include:

· Labor categories

· Labor hours

· Labor rates

· Other costs

· Travel

· Shipping

· SAM UEI Number.

CONFLICT OF INTEREST

Any offeror (or member of its team and/or subcontractor) having a conflict of interest as defined under FAR Part 9.5 must identify the conflict as soon as it is known and provide a recommended mitigation plan. Mitigation plans are required whenever a competing contractor has had unequal access to non-public information regarding this requirement and/or competition or has assisted the Government in defining the requirements or evaluation criteria.
