About this file
This is a Signature of Bidder's Authorized Representative Form (Attachment 1) for contract opportunity S-62900-00015271 issued by the Oregon Department of Forestry (ODF). The form must be completed and returned with the bid submission and serves as the official attestation document by which an authorized representative certifies the bidder's qualifications, compliance with solicitation requirements, and adherence to applicable laws and regulations. The form requires bidders to provide contractor data including legal business name, mailing address, federal tax identification number, business designation, and authorized representative contact information. No specific project deliverables, timelines, or contract terms are detailed in this attestation document.
The form establishes residency classification requirements under Oregon Revised Statutes (ORS 279A.120), requiring bidders to declare whether they are resident or non-resident bidders and, if non-resident, to identify their state of residency and any applicable government contract preferences in their home state. Bidders must certify under penalty of law that they possess adequate resources and staffing to perform required services, have not engaged in discriminatory practices against minority or women-owned businesses or disabled veteran-owned enterprises, maintain written policies compliant with HB 3060 (2017) regarding sexual harassment and discrimination prevention, and are in compliance with Oregon tax laws as enumerated in ORS 305.380(4). The signature of the authorized representative, executed in ink with any alterations or erasures initialed, constitutes the legal certification that all representations and attestations contained in the bid are true and accurate.
