1.0 DESCRIPTION OF REQUIREMENTS

1.1 Introduction

This Statement of Work (SOW) documents the requirements to provide a new galvanized 174-foot self-supporting (no guy wires) structure and all mounts with structural components for use at Air Force Plant 42 (AFP 42). The tower MUST NOT exceed 180 feet (AGL) in height when external protuberance (antennas, lightning rod and/or other accessories) are attached to the top of the structure. Additionally, the Contractor is to perform the deconstruction, demolition and disposal of the previous Beacon Tower (Building 519) as defined in this SOW. The work to be accomplished under this contract includes all labor, materials, taxes, quality control, transportation, equipment and services for, and incidental to, the completion of all design and construction work in connection with the project described in the contract documents.

1.2 Background

This project is to replace the outdated and aging beacon tower which has reached the end of its serviceable life. The current tower is a 108-foot self-supporting, four-legged beacon tower that was constructed in 1959 and is beyond its useful life. The new self-supporting, (aka Spectrum Beacon), tower will allow the 412th Communications Squadron and AF Plant 42 to better manage the agencies use of radio frequency spectrum management to perform vital operations that supports our mission. Improved Spectrum management will provide the technical engineering expertise needed to perform specific spectrum resources assessments and will create an efficient and more effective conflict resolution capability. The tower will support the new Air Traffic Control Tower (ATCT) beacon requirements as well as providing improved maintenance capabilities.

1.3 Roles and Responsibilities

OL-AF Plant-42/PM will be responsible for project management.

OL-AF Plant-42/CE will be responsible for system acceptance and maintenance.

412 SFS/Det 1 will be responsible for Security

OL-AF Plant-42 CE will provide support and identify all CE requirements.

412 CS/SCOTS will be responsible for authorized frequency range.

412 TENG/ENG will be responsible for funding and design requirements

AFTC/PZIOC will be responsible for Contracting

1.4 Scope

This SOW provides specifications and requirements for a qualified contractor to design and build a 174-foot Federal Aviation Administration (FAA) compliant self-supporting tower at the 412th Test Wing/Operating Location AF Plant 42 located at 2501 East Avenue P, Palmdale, California 93550 (Los Angeles County). The coordinates for the new tower is 34° 37' 03" N and 118° 04' 34" W (Attachment 1). Directions to AFP 42 is in Attachment 2. This is a turnkey project requiring the new self-supporting tower to be capable of withstanding sustained winds of 120-mile per hour and in compliance with California Seismic 4 earthquake requirements.

1.4.1 High-Level Requirements

Design and build a self-supporting tower NOT to exceed 180 feet when apparatuses are attached (beacons, antennas, lightening protection, etc.) on the top of the structure. High-level requirements are listed below:

1.4.1.1 Provide complete sets of project drawings.

1.4.1.2 Install a concrete foundation that will support the tower to include multiple platforms with safety railings that will hold an additional 2500 pounds appurtenance dead load.

1.4.1.3 Install all fiber optic cables, electrical equipment and infrastructure.

1.4.1.4 Install commercial utilities to include underground ducts, manholes, PVCs, and conduits.

1.4.1.5 Provide all electrical infrastructure.

1.4.1.6 Install Tower hazard lighting to meet FAA requirements.

1.4.1.7 Provide a tower to withstand 120-mph sustained winds.

1.4.1.8 Ensure foundation, tower and appurtenances are built to meet or exceed California Seismic Zone 4 requirements.

1.4.1.9 Relocate backup generator and attachments from present location to new tower site.

1.4.1.10 Demolish existing tower and remove the debris.

1.4.2 General Design Requirements

1.4.2.1 Project consist of providing and installing a 174-foot self-supporting tower in accordance with this SOW. Contractor shall provide all parts, labor, materials and equipment to install the tower. This work shall include the clearing of the site, erection of the tower, installation of lightning protection, installation of backup generator, installation of new communications fiber and all incidentals necessary to place it in operating condition as a completed unit.

1.4.2.2 Project drawings shall consist of separate sheets to indicate specific aspects of the work (i.e., structural, civil, electrical, mechanical) or any sub-section of work requiring plans for proper implementation during the installation phase of the project.

1.4.2.3 The tower may be purchased commercially or as a kit per the requirements in this SOW.

1.4.2.4 The Contractor shall develop and submit a cutover plan to AFP 42 that provides a smooth transition between current tower, antennas, power and generator to the new tower. Since this an active system, there can be no interruption of services during this project.

1.4.2.5 Paint to meet AF standard

1.5 Contracting Requirements

This SOW provides the most current understanding of the work required by both parties to ensure a successful project implementation Should any of the sites change, a revision to the SOW and associated pricing will be required. It is understood that this SOW is a working document, and that it can be revised as needed to incorporate any changes associated with contract negotiations and any other changes that may occur during the execution of the project.

1.5.1 Contract Vehicle

This is a Firm Fixed Price Contract. Only the Contracting Officer (CO) can authorize a change in the SOW. Work performed by the Contractor without consent by the CO is done so at the risk of the Contractor and not to the Government or its representatives.

1.5.2 Period of Performance

1.5.2.1 The period of performance shall be 180-days from date of award. The Contractor shall provide the Air Force with a proposed project schedule.

1.5.2.2 Factors that may impact completion (e.g., tower worksite location, tower type, scope and complexity of work to be completed, environmental and weather-related factors, travel time, and equipment delivery schedules) must be coordinated and approved by the AF Plant 42 and the CO.

1.6 Procedural Requirements

1.6.1 Contractor

1.6.1.1 A site visit will be scheduled at the time the Request for Proposal (RFP) has been issued. The site visit shall include the potential bidders, AFP 42, 412 TENG/SRF, AFTC/PZIOC and 412th CS/SCOTS personnel involved in the project.

1.6.1.2 Upon contract award, the Contractor shall contact the PM to schedule a Preliminary Design Review (PDR) within 15 days of the Award.

1.6.1.3 The contractor shall attend the PDR to demonstrate the Beacon Tower design. The PDR shall be a formal technical review of the design. The PDR will allow the Government to evaluate the technical risk associated with the selected design approach. The PDR shall include the design, functional diagrams, infrastructure connections, concept of operations, design standards and drawings, materials/part list, logistical considerations (including training and manuals). The PDR shall be conducted at AF Plant 42.

1.6.1.4 The 100-percent design review is due 15 days after the PDR. The PM may schedule a TELECON to discuss any additional changes.

1.6.1.5 Contractor shall call 811, to schedule ground scanning to locate utilities before any digging can take place. The Contractor shall notify the PM when all Utility Locate tickets have been closed.

1.6.1.6 The Contractor shall comply with OSHA 3433-10R 2014, Cranes and Derricks in Construction.

1.6.1.7 If the Contractor plans to use a Nuke Gauge density test on soil compaction, the Contractor must allow at least 30 days to obtain the Nuke Density permit. The Contractor shall submit licensing documentation to the PM.

1.6.1.8 The Contractor shall submit a crane plan, FAA Form 7460-1 (Attachment 3) to the PM within 7 days of award for FAA approval prior to equipment being brought onto installation. FAA requires at least 45 days for approval.

1.6.1.9 Contractor shall submit all required FAA Supplemental Notice Forms 7460-2 to the PM when required along with a Crane Lift Plan.

1.6.1.10 Contractor shall obtain a Dig Permit prior to performing any excavations.

1.6.1.11 The Contractor shall obtain required Electrical Permit from AFP 42 CE (661-272-6677) at least 15 days prior to the beginning of construction.

1.6.1.12 The Contractor shall obtain a Welding Permit from AFP 42 Fire Department. The Contractor shall contact the FD (661) 272-6740/ 6741/6637) at least 14 days prior to beginning construction to schedule a Welding Cutting briefing before any welding equipment or a cutting torch can be used on site.

1.6.1.13 The Contractor shall remove and dispose of any waste materials at a proper disposal site, off AFP 42, and in strict accordance with applicable Federal, State, County and local laws.

1.6.2 Project Manager

1.6.2.1 Construction efforts and request for permits shall be coordinated with the PM to prevent any construction delays, interruptions of work within the facility and/or movement of personnel and/or equipment.

1.7 Security Information and Site Access

1.7.1 Access to the proposed worksite is restricted. The Contractor shall coordinate construction activities, including work schedule, materials delivery and personnel requiring site access with the Project Manager. The PM will coordinate visit requests with 412 SFS/Det 1 (661) 272-6663.

1.7.2 Contractor personnel are required to follow AFP 42 security policies and shall comply with pertinent Air Force security regulations while working on the project.

1.7.3 All Contractors on this project will be cleared by the 412 SFS/Det 1. Workers will be given temporary badge (dependent on construction duration) before entering the controlled area. The Contractor is required to submit the names of their Contractor personnel on the Entry Authorization List (EAL, Attachment 4) spreadsheet and AFMC ID Application (Attachment 5) at least 2 weeks prior to start of construction. The names of Subcontractors should be submitted at least 72 hours in advance

1.7.4 Contractor personnel must submit two Acceptable Forms of the following Identification

a. Drivers’ License (Must meet Real ID requirements)

b. US Passport/US Pass card

c. Permanent Resident Card (Green Card)

d. Foreign Passport (with I-551 stamp)

e. U.S. Coast Guard Merchant Mariner Credential & Mariner Legacy Card

1.7.5 Contractor personnel shall provide proof of current vehicle registration, driver’s license and vehicle insurance prior to requesting base access.

1.8 Safety

1.8.1 The Contractor is responsible for ensuring that this project and related work complies with all applicable local, state and federal safety codes and regulations related to this type of work.

1.8.2 Contractor personnel shall wear appropriate Personal Protective Equipment (“PPE”) at all times.

1.8.3 Contractor personnel must be properly trained and currently certified in tower climbing to work at heights above 6’ on the tower.

4.0 CONTRACTOR FURNISHED ITEMS AND SERVICES

4.1 Technical Services

4.1.1 The Contractor shall submit project construction drawings electronically for review and approval prior to the Notice to Proceed being issued. The drawings must be submitted to AFP 42 CE and the PM.

4.1.2 The contractor shall provide all equipment, installation, on-site performance and system testing, with complete documentation (to include product assembly drawings, operation manuals and wiring diagrams), in electronic format (latest Adobe ®, PDF Format) viewable format. Documents should provide sufficient details for training, operating and maintaining support activations, configuration management, troubleshooting, and preventive and corrective maintenance.

4.1.3 The Contractor shall ensure all materials and equipment furnished are new and are of good quality and in conformance with the contract and SOW. Any and all substitutions must be properly approved and authorized in writing by AFP 42 and the Contracting Officer prior to installation. The Contractor shall furnish satisfactory evidence as to the kind and quality of the materials and equipment being substituted.

4.1.4 The Contractor is required to maintain all pipes, ditches, and other drainage structures free from obstruction until work is accepted by AFP 42. The Contractor is responsible for any damages caused by failure to maintain drainage structure in an operable condition.

4.1.5 The Contractor shall follow the manufacturer’s hardware assembly instructions exactly which shall supersede any conflicting notes herein.

4.1.6 The Contractor shall restore existing access drive to previous found condition or better after completion of project.

4.1.7 The Contractor shall provide complete system demonstration to verify the system is installed exactly as proposed to satisfy the requirements.

4.1.8 The Contractor shall provide an acceptance test plan to outline the testing sequence and steps to follow during system acceptance.

4.1.9 The Contractor shall provide, by 2:00 PST each Friday, a Monthly Progress Report (MPR) that documents the week’s activities (See Attachment 7). If the Friday falls on a holiday, then it shall be provided the next working day by

2:00 PM PST.

4.1.10 The contractor shall provide Operator and Maintenance Manuals that provide instructions suitable for use by the intended audience of the system. The Operator Manual shall also include operator maintenance tasks such as preventive maintenance checks and services, inspection, lubrication, adjustment, and operator level repair and replacement tasks as needed.

4.1.11 The Contractor is prohibited from taking photos of any facility on site, with the exception of the old tower if the information is to be used for “Reference Only.”

The Contractor MUST get approval from the PM before any photographs can be taken. All photographs must be clearly labeled in the lower right-hand corner to include the date the photo was taken. No photos shall be released without stamped approval from the Primary or Alternate Security Manager.

4.1.12 Contractor shall maintain, on site at all times, construction plans (updated as changes occur) as well as a complete set of the Contract Documents and all Contract Change Orders. The Contractor shall provide the Government with a structural design analysis for the foundation, tower, and appurtenance installation.

4.2 Equipment and Materials

4.2.1 Tower General Requirements

4.2.1.1 The Contractor is solely responsible to determine the appropriate tower, equipment, and construction procedures to ensure the safety of structure and its component parts during erection.

4.2.1.2 The Contractor shall adhere to the equipment list as specified by the tower manufacturer. Materials certified in the Manufacturer's drawings shall be used. If changes and/or substitutions are required, the Contractor shall bring them to the attention of AFP 42. Approval must be acquired before any change is implemented.

4.2.1.3 All equipment and materials shall be subject to acceptance through manufacturer's certification and compliance with this SOW.

4.2.1.4 AFP 42 CE reserves the right to reject any and all equipment, materials or procedures, which does not meet the system design, the standards and codes, specified herein.

4.2.1.5 All equipment/materials furnished and installed under this SOW shall be guaranteed against defects in materials and workmanship for a period of twelve (12) months from final acceptance by the AFP 42. The defective materials and/or equipment shall be repaired or replaced, at the Contractors expense, with no additional cost to AFP 42.

4.2.2 Tower Design

4.2.2.1 The Contractor shall build/purchase and install a 174-foot self-supporting tower that meets or surpasses the requirements and specifications documented in this SOW. Deviations or changes to the requirements must be approved by AFP 42 PM/CE personnel.

4.2.2.2 The tower manufacturer’s information and engineering stamped drawings shall be submitted to the Government for approval.

4.2.2.3 If the tower is custom built to meet the required specifications, the design must include drawings and construction notes. The design shall include a materials list, type of tower (i.e., 3 or 4 legs), type (solid or tubular) and size of legs, face widths (bottom to top), type of bracing, type and size of bolts, type and size of diagonals, section connections, type and size of anchor bolts, weld points, and any other materials or hardware needed to build the tower, but not specifically stated in this general list or in the specific requirements. The design shall include all materials, types, dimensions, and quantities to build a 174-foot self-supporting tower per the requirements specified in this SOW. The top rail will be no greater than 174 feet.

4.2.2.4 Detail drawings and specifications furnished by the tower manufacturer shall be strictly followed during construction. All tower shall be erected in sections from the ground up unless otherwise specified. In final assembly, all bolts and fastenings shall be installed, and the structure shall be plumb, true, square, and level. Nuts shall be taken up to a firm bearing after which the bolts shall, if necessary, be cut to proper length to protrude at least three full threads.

4.2.2.5 The tower shall include a platform at 140 feet (SRF Platform) and shall include safety railing for fall protection. The platform shall be at least 7 feet X 7 feet or greater to provide adequate space to house the equipment and perform maintenance.

4.2.2.6 The SRF platform shall be capable of supporting up to 1,500 pounds appurtenance dead load and 1,000 pounds live load.

4.2.2.7 The platforms shall be designed out of a material that promotes water drainage (i.e., grated steel).

4.2.2.8 The design shall include 110V outdoor rated convenience outlet on each platform. The outlet shall include a NEMA 3R cover kit.

4.2.2.9 The design shall include a second platform at 172 feet (Beacon Platform) and shall include safety railing for fall protection. The platform shall provide adequate space to install the beacon and perform maintenance tower without exceeding edges.

4.2.2.10 The Tower shall be designed with safety railing around the platform capable of preventing a fall.

4.2.2.11 Beacon Platform shall be designed to hold 1,000 pounds appurtenance dead load.

4.2.2.12 The tower design shall include mounting apparatus for Hali-Brite HBM320L-802M military rotating beacon at the top rail of the tower (174 feet). The product specifications to ensure proper mounting (Attachment 8).

4.2.2.13 The tower design shall include a pulley system capable of bringing equipment (300 psf) from the bottom to the Beacon Platform.

4.2.2.14 The tower design shall include a ladder with a vertical safety climb mechanism for fall protection and safe access to the platform and catwalk.

4.2.2.15 The tower design shall include a lockable ladder guard that prevents access to unauthorized climbing.

4.2.2.16 The design shall include tower hazard lighting that meets ATC standards.

4.2.2.17 The design shall include proper grounding in accordance with R-56 Grounding standards for lightning protection.

4.2.2.18 The tower design shall include lighting protection.

4.2.3 Tower Foundation

4.2.3.1 The contractor shall be responsible for all site analysis, preparation, design, site development, and ensuring that the structural integrity of the tower foundation and tower meet wind, seismic, and appurtenance/mounting load requirements.

4.2.3.2 The tower foundation and footings shall be sized appropriately to meet the tower manufacturer specifications.

4.2.3.3 The tower foundation shall be designed to support the tower (including the two platforms) and hold 2500 pounds appurtenance dead load.

4.2.3.4 The tower and foundation must be designed to meet or exceed California Seismic 4 requirements.

4.2.3.5 The tower and foundation shall be designed to withstand 120-mph sustained winds (Attachment 9).

4.2.3.6 The tower and foundation shall be designed to withstand other element loads such as rain and snow. Ice loading for the applicable County shall be considered when calculated in the dead load to be supported by the foundation system.

4.2.3.7 The Contractor shall perform an engineering site survey and provide an installation plan.

4.2.3.8 The Contractor shall ensure that the foundation, tower, and all installed equipment on the towers meet applicable installation codes and safety requirements.

4.2.3.9 The Contractor shall design and build the foundation following criteria from the tower manufacturer, geotechnical survey results, and in accordance this SOW.

4.2.3.10 The tower foundation and self-supporting tower design shall be approved and stamped by a California Registered Professional Engineer prior to construction.

4.2.3.11 The contractor shall coordinate all necessary permits with PM prior to foundation and tower installation.

4.2.3.12 The contractor shall conduct a final inspection of the foundation and tower when installation is complete and repair any noted deficiencies.

4.2.4 Lightning Protection and Grounding

4.2.4.1 Lightning rods shall be furnished by the tower manufacturer and installed by the Contractor.

4.2.4.2 Lightning protection shall comply with NFPA-780, Standard for the Installation of Lightning Protection Systems, AFI32-1065 Grounding Systems, and UFC 3-575-01 Lightning & Static Electricity Protection Systems. All materials shall comply with Class II requirements regardless of tower height. Ground rods and underground cables shall be installed in accordance with the specifications and this SOW.

4.2.4.3 The complete lightning protection should be approved prior to installation to the satisfaction of AFP 42 UFC 3-575-01 Lightning & Static Electricity Protection Systems

4.2.4.4 Lightning protection installation must be certified by an independent third party upon completion of project. The contractor shall properly ground the tower and applicable equipment in accordance with R-56 Grounding standards

4.2.5 Paint Specifications

4.2.5.1 The tower shall be constructed of heavy duty steel that has undergone the “hot dip galvanization” process which results in a steel surface protected by a zinc coating to prevent environmental corrosion.

4.2.6 Obstruction Lighting

4.2.6.1 The Contractor shall provide and install obstruction lighting in accordance with Federal Communication Commission (FCC), Federal Aviation Authority (FAA) and/or local aviation authority requirements. The Contractor shall provide an FAA Advisory Circular AC 150/5345-43 compliant obstruction light system for night-time operation installed in accordance with Chapter 5 of FAA Advisory Circular AC 70/7460-1.

4.2.6.2 The Contractor shall provide and install FAA-approved obstruction light at the location specified according to the tower height. Model number of lights are provided in (Attachments 10).

4.2.7 Rotating Beacon

4.2.7.1 The Contractor shall provide and install a Hali Brite HBM 320 L-802M Military Airport Rotating Beacon that complies with FAA Advisory Circular AF150/5345-12F, Specification for Airport and Heliport Beacons.

4.2.7.2 The Rotating Beacon shall be installed per the Manufacturer’s instructions.

4.2.7.3 The Contractor shall install mounting apparatus/pedestal for the Hali-Brite beacon at the top rail of the Beacon platform (174 feet). Mounting shall comply with FAA Advisory Circular AF150/5345-12F.

4.2.7.4 The Contractor must field verify that the factory preset elevation angle of 5 degrees (FAA requirement) did not change the degree of elevation after installation.

4.2.7.5 The Contractor shall provide a properly rated surge suppression device to protect equipment. Per IEEE C62.41, Medium System Exposure, Location Category C2, Table 4.

4.2.8 Fiber Optic

4.2.8.1 The Contractor shall install 24-strand, single-mode fiber optic cable which will include a minimum of 3-Cell Maxcell fabric innerduct from the Grey Eagle site to the new tower location (Attachment 11).

4.2.8.2 The Contractor shall install a spare with string/pull interdirect.

4.2.8.3 The Contractor shall install a new precast hand-hole sized appropriately with traffic rated lid for the fiber installation at the new tower location.

4.2.8.4 The Contractor shall use “Direct Drill/Bore” and jack roadway for running fiber underneath roadways support during construction efforts. The opening of roadways will not be approved.

4.2.8.5 The Contractor shall terminate and test both ends of the fiber optic cable.

Termination will match the initiation fiber optic cable in the communication vault. The contractor shall provide the government with the test results.

4.2.8.6 The contractor shall protect communications conduits in the ground by a four (4)-sack slurry mix.

4.2.8.7 The contractor shall backfill the trenching and compact it.

4.2.9 Electrical Installation

4.2.9.1 AFP 42 will locate and mark existing utilities prior to the Contractor beginning any trenching. Any damage caused to existing utilities shall be repaired at the contractor’s expense.

4.2.9.2 The Contractor shall install 208V electrical power from the Grey Eagle site to the new tower location and should also include SPD for surge protection on any electrical installed equipment (See Attachment 11).

4.2.9.3 The Contractor shall provide labor, materials, inspections and testing to provide code compliance for electric, and ground/lighting systems.

4.2.9.4 Electrical materials, devices, conductors, appliances and equipment shall be labeled/listed by UL or accepted by jurisdiction (Local County, State or Third Party Testing Agency).

4.2.9.5 The contractor shall install all electrical components; to include wire, switches, transformers, electrical panels, and Uninterruptable Power Supply (UPS). The Contractor shall provide an electrical diagram prior to installation to be approved by AP 42 CE.

4.2.9.6 The contractor shall install a new precast hand-hole sized appropriately with traffic rated lid for the electric cable installation at the new tower location.

4.2.9.7 Electrical installation shall comply with applicable laws and codes. These include but are not limited to NFPA-70, serving utility regulations and local and state laws.

4.2.10 Backup Generator Relocation

4.2.10.1 The contractor shall comply with electrical requirements in Section 4.2.9.

4.2.10.2 The contractor shall submit drawings of the concrete pad and electrical connections to AFP 42 CE for approval prior to construction generator.

4.2.10.3 The Contractor shall build a concrete slab that is sufficient to support the generator. The foundation must meet or exceed California Seismic 4 requirements. The slab should allow for a 3-foot walk area around the generator in order to perform maintenance.

4.2.10.4 The contractor shall remove the current generator from its present location at the base of the old tower (Bldg. 519) and install it in its new location at the newly constructed beacon tower. The generator is a DF-2 type diesel fuel with sub-base tank. Contractor should be prepared to move both units together.

4.2.10.5 The contractor shall install all required electrical connections necessary to make the system operational.

4.2.10.6 The Contractor shall install three buried conduit and pull rope from the existing generator site to the new concrete slab.

4.2.10.7 The Contractor shall remove the Automatic Transfer Switch (ATS) from the old tower and reinstall it at the new tower site. The Transfer switch shall be fed with the new 100A 3-pole breaker for existing panel EP1.

4.2.10.8 Upon completion of the re-installation, the Contractor shall operate and adjust the equipment to ensure it’s in full operations.

4.2.11 Clearing, Grading, Excavation and Fill

4.2.11.1 The Contractor shall provide AFP 42 with a site plan that must be approved by the PM and CO prior to starting work.

4.2.11.2 Site preparation shall be conducted IAW the design specifications and drawings, applicable federal and state regulatory standards.

4.2.11.3 Contractor shall use all efforts to make the best use of the native earth materials to accomplish excavation, backfilling and grading of the site.

Contractor shall execute the excavation, backfill and grading work in a manner that manages native earth material to be optimum for compaction, keeping dust to a minimum, including protecting and preserving excavated soil for future use.

4.2.11.4 Contractor shall not use imported fill unless the Contractor clearly demonstrates that native fill material is not suitable for use. If a specific fill is required to support tower foundation, the Contractor shall send written notification to the CO to use imported fill. Materials other than native fill must be approved by the PM and CO.

4.2.11.5 Contractor shall maintain all access roads to the Site during construction.

Upon completion of construction, the Contractor shall leave roads in the pre-construction condition or better.

4.2.11.6 Contractor shall provide, at no cost to AFP 42, all necessary materials, equipment, and labor required to repair any damage to the road that occurs as a result of construction activities, site access, equipment transport, or materials delivery for the project Site.

4.2.12 Demolition and Removal of Old Tower (Building 519)

4.2.12.1 The contractor shall deconstruct, demo, dispose of tower site structures and debris, and return the land to its natural state. The contractor shall provide any materials, supplies, and parts needed for tower site deconstruction, demolition, disposal, and site restoration. Any additional material, supplies, or parts required for repairs, other than what is stated in this PWS, shall be approved by CO and PM prior to purchase and installation.

4.2.12.2 Contractor will deconstruct tower, building, fence, utility pole and demo foundations footings, and anchors.

4.2.12.3 Contractor shall demolish tower foundations, anchors, building foundations, and any other post/pole concrete at site.

4.2.12.4 : Contractor is required to remove and dispose of all items and debris, including site gravel in accordance with EPA standards and guideline.

4.2.12.5 Contractor shall return tower site to its natural state.

4.2.12.6 Contractor shall complete site work and provide backfill/ top soil as necessary at all sites.

4.2.12.7 Contractor will have demolish old tower after completion of new tower and after Air Force acceptance.

4.2.12.8 Contractor shall include Demolition/Deconstruction Plan in the Project Schedule.

4.2.12.9 Contractor shall be responsible for any damages to Government property to include roads, fences, structures, etc. Contractor shall be responsible to make repairs, provide replacement, or provide reimbursement for damages incurred during project to the Government.

4.2.12.10 After deconstruction and demo tower, buildings, fence, foundation debris and all associated pieces will become the property of the Contractor.

4.2.12.11 Contractor shall be responsible for removal of all debris from the property and proper dispose of all items in accordance with EPA standards and guideline.

4.2.12.12 Contractor shall insure Solid Waste/ Waste Material Management procedures are adhered to in accordance with para 6.6 of this SOW.

5.0 PROJECT CLOSEOUT

5.1 Acceptance

The Contractor shall submit test results, as built drawings, equipment lists to include manufacturer, model, serial number and equipment layout drawings to be delivered to the AFP 42 PM and stakeholders upon completion of the project and during the project acceptance.

5.2 Closeout

5.2.1 At Substantial Completion (Minor items), a small list of remaining tasks may be allowed as determined by the PM. Such tasks include, but are not limited to, minor site cleanup or landscape work and similar tasks. The scope of the remaining work must be accomplished without shutdown or interruption of site operations, and without placing personnel in danger while completing the remaining tasks. A list of the tasks (the punch list) must be agreed upon in writing between the Contractor and the PM

5.2.2 For Civil Completion, no outstanding punch list item(s) should be open. The remaining contract work is solely related to completion of administrative tasks.

5.2.3 For Final Completion, the Contractor shall deliver to AFP 42 one (1) hard copy and one electronic (Adobe Acrobat format on CD) copy of a draft detailed closeout package for review and approval. As-built drawing to be in AutoCAD 2014 Format and Adobe PDF Format. The Contractor shall send documents to the PM at the below address:

5.3 Property Transfer

The PM will complete a DD Form 1354 (Attachment 12) filled out and turned in through AF Plant- 42 CE to account for Air Force real property. The PM will notify the CO when this is done as the final step to close the contract.

6.0 BASE REQUIREMENTS

6.1 Base Access: All contractor employees will be subject to a criminal history background check IAW air force manual (AFMAN) 31-113, installation perimeter access control, as stipulated in the installation commander’s memorandum to the 412th SFS/Det 1 dated 23 Apr 2015 (the AFMAN and memorandum are both limited access documents but more details on the requirements/process in terms of background checks can be provided by the contracting office upon request). Vehicle registration, proof of insurance, and a valid driver's license must be presented for vehicles. Installation commanders may deny access and credentials based on information obtained during identity vetting that indicates the individual may present a threat to the good order, discipline, and morale of the installation.

6.1.1 Long-Term Base Access. Not required for this project.

6.1.2 Short-Term Base Access. Contractor employees requiring temporary access must be vetted prior to entry. The Contractor’s shall request access through the PM). 412 SFS/Det 1 will load the employee’s information into the Vehicle Control Center’s (VCC) access database.

6.1.3 The Contractor shall comply with all security requirements imposed by AFP 42.

6.1.4 Visitors/contractors with driver’s licenses from ME, MN, MO, MT, and WA (exceptions are EDL-Enhanced driver’s licenses from MN & WA) will not be able to obtain Base access without another form of identification as an identity proofing document. The list of authorized IDs can be found in the REAL ID Act of 2005. 6.1.5 The Contractor shall be responsible for safeguarding all government property, equipment and materials provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

6.1.6 During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel.

6.1.7 The Prime Contractor shall ensure that all Base identification passes issued to employees and subcontractor employees are returned to the PM. Failure to comply with these requirements may result in withholding of payment. The Contractor shall be responsible for prompt return of credentials and vehicle passes for:

6.1.7.1 Any employee who no longer requires access to the work site;

6.1.7.2 Upon request by a CO; or

6.1.7.3 Upon completion or termination of the contract.

6.2 Security, Vehicles, and Parking:

6.2.1 The Contractor shall follow existing security traffic, parking procedures and instructions applicable to AF Plant 42.

6.2.2 The Contractor shall be responsible for the physical security of all materials, supplies, and equipment, including property that is being utilized. If required, contractor to have a dig alert (811) and third party locator to perform all locates and mark area. Copy of drawings and maps to be given to the government to be added to the AF Form 103 approval.

6.2.3 The Contractor employee, suppliers, and subcontractor’ vehicles shall not be parked on grass/landscaped surfaces at any time. In addition, vehicles may not be driven over grass/landscaped surfaces, unless indicated and approved by the PM.

6.2.3.1 Contractor vehicles shall be in top repair and be able to be shut off and restarted without outside aid. Vehicles in need of repair will be replaced not later than the following morning. Downed and unrepairable vehicles must be removed from the AFP 42. If necessary, the Contractor shall provide a flatbed trailer and rig or a tow truck at their cost to remove the vehicle from the AFP 42. Vehicles shall be in safe operating conditions at all times, have seatbelts, or will be denied access to the AFP 42.

6.2.3.2 Contractors at no time shall perform routine maintenance on vehicles which would result in the dumping of any fluid onto the ground. The Contractor shall not transport rollers, paving equipment, or other slow moving construction equipment on and throughout AFP 42 unless it is loaded on a flatbed or lowboy.

6.2.4 The Contractor shall comply with all AFP 42 security regulations. Contractor personnel may be subject to personal, equipment and vehicle inspection upon entering or leaving AFP 42. Additionally, the Contractor shall comply with actions dictated by force protection condition changes/measures for real world situation and AFP 42 exercises. Changes to force protection conditions could impact Contractor performance of work and can include denial of access to the installation to perform work.

6.2.5 The Contractor shall report suspicious activity to 412 SFS/Det 1 at (661) 272- 6600 in a timely manner. The Contractor shall contact the Antiterrorism Office, 214 SFS/Det 1/ATR at (661) 456-2366.

6.2.6 IAW DoDI O-2000.16 Vol.1_AFI10-245-O-AFGM2020-0, a one-time Level I Antiterrorism Awareness Training will be required and offered by the Government to all Contractor employees. This training provides Contractor employees with the requisite knowledge necessary to remain vigilant for possible terrorist threats. 6.2.6.1 .Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within Contractor-owned or privately owned vehicle while on AFP 42.

6.3 Vehicular Traffic:

6.3.1 Traffic Control Devices. The Contractor shall comply with the local laws and regulations, to include Air Force regulations, regarding the safe operation of equipment in and around the base roadways. Contractor owned and/or operated vehicles violating speeding, parking, or traffic regulations on Federal Government grounds property may be removed from the installation at the Contractor’s expense. Should this be the case, the Government will not be held liable for the time delay to the Contractor nor such costs that might occur based on the vehicle removal.

6.3.2 The Contractor shall coordinate to ensure proper warnings to motorists are posted and adequate traffic control is used when vehicles are present. This shall be coordinated with the CO and PM.

6.3.4 Signs, warning lights, barricades, and other traffic control devices shall be provided by the Contractor. Warning lights will be operational at all times during non- daylight hours. Barricades shall be highly luminescent and clearly visible at night. Signs shall also be highly luminescent, blaze orange in color with black lettering, and be clearly visible from at least 50 feet (15m) away.

6.3.5 Temporary traffic control signs (warning and hazard signs) shall be furnished and installed by the Contractor while projects are underway (if required). These warning signs shall be placed near the beginning of the work site and well ahead of the work site for alerting approaching traffic from both directions. Slow-moving equipment shall be marked with large warning signs indicating slow-moving equipment in operation.

6.3.6 Road Diversions/Closures. The Contractor shall notify the PM at least 15 business days prior to starting work if it will impact traffic on the roadways. The Contractor shall state in writing the exact dates of the road closure, duration of closure, and provide a traffic diversion map showing the location of the diversion (if available), closure, posting of signage, phasing plan (if applicable), and other pertinent information for the impedance.

6.3.7 All costs including, but not limited to, design, procurement, equipment, installation, maintenance and removal, associated with the temporary traffic control signs, barriers, barricades, warning lights, diversions, etc. are the sole responsibility of the Contractor.

6.4 Restricted Area:

6.4.1 All contractor employees will be subject to a criminal history background check IAW Air Force manual (AFMAN) 31-113, Installation Perimeter Access Control, as stipulated in the installation Commander’s memorandum to the 412th Security Forces office dated 23 Apr 2015 (the AFMAN and memorandum are both limited access documents but more details on the requirements/process in terms of background checks can be provided by the contracting office upon request).

6.5 Escorts: Escorts not required. No work is taking place in a secure area such as the airfield.

6.6 Disposal of Waste/Excess Material:

Any waste or excess material resulting from any contract requirement shall be removed and disposed of at a proper disposal site off AFP 42 by the Contractor in strict accordance with applicable Federal, state, county, and local laws. Disposal, discharge, deposit, dumping, spilling, leaking, or placing of any material, wastes, effluents, trash, garbage, oil, grease, paint, chemicals, etc., in areas other than proper disposal sites off AFP 42 shall be subject to the approval of the CO. If any waste material is dumped in unauthorized areas, the Contractor shall remove the material and restore the area to

Spectrum Beacon Tower Construction Statement of Work Page 22 the condition of the adjacent undisturbed area. Contaminated ground shall be excavated, disposed of as directed by the CO and in coordination with the AFP 42/CE Environmental Management.

6.6.1 Surfaces shall be replaced with suitable fill material, compacted, and finished to match adjoining surfaces.

6.6.2 Trash and Litter Control. The Contractor shall develop a strict trash and litter control program. The litter control program shall consist of supplying an adequate number of covered trash and litter receptacles in all appropriate locations. The Contractor shall take precautions to prevent Foreign Object Debris (FOD) while working on AFP 42. The Contractor shall dispose of all trash IAW para. 6.2.

6.6.3 Landfill. The Contractor shall furnish the following information to the CO prior to work commencing on the first task order issued against this contract: 1) Name and address of the proposed landfill to be used for disposal of project related waste; 2) landfill class; and 3) proposed materials to be disposed of at this location.

6.6.3.1 This information is to be submitted for all materials to be disposed of by the Contractor, including hazardous waste as well as demolition debris, and shall be submitted and approved before work begins on the first task order.

Copies of manifests and inventories of hazardous wastes that were disposed of during the course of the contract must be provided to AFP 42/CE and the CO a Environmental Management at the conclusion of the contract. The close-out submittal must contain all information required by the initial submittal.

6.7 Hazardous Materials

6.7.1 Materials classified as hazardous by U.S. and California environmental rules, regulations, or laws shall be handled for disposal IAW those rules, regulations, or laws. Any cost provisions to meet these requirements shall be the sole responsibility of the Contractor. Any project delay to meet these requirements shall also be the sole responsibility of the Contractor.

6.7.2 Contractor shall complete Chemical/Hazardous Material Request, AF Form 3952 with Safety Data Sheets for each item brought on to Plant 42.

Form needs to be coordinated thru Environmental Management/ Safety/Fire.

6.7.3 Hazardous Material Identification and Material Safety Data, IAW FAR Clause 52.223-3: The Contractor shall list any hazardous material to be delivered under this contract. For each item, the Contractor shall submit to the CO an SDS listing any hazardous material to be delivered under this contract prior to the hazardous material entering the installation. The Contractor may submit a master list of all hazardous material at the beginning of the contract’s performance, but must update the list throughout the life of the contract when needed if a new item needs to be added. The initial and any revised lists must be submitted to and approved by the CO prior to the material entering the installation.

6.7.3.1 All hazardous material shall be stored in a safe and environmentally responsible manner within the working site.

6.7.4 Disposal of Hazardous Material. The Contractor shall comply with all Federal, state, and local laws pertaining to the protection of the environment and emission of hazardous pollutants in the performance of this contract. The Contractor shall also provide a copy to the CO and the AFP 42/CE Environmental Management Office of all correspondence with local, state, or Federal pollution control authorities. Any hazardous waste generated by the Contractor shall be the Contractor's responsibility to dispose of, IAW the above-mentioned laws, to include hauling all paint determined as hazardous waste to the nearest hazardous waste dump.

6.7.5 Spillage. Special measures shall be taken to prevent paints, chemicals, fuels, oils, grease, etc., from being spilled and particularly from entering surface or groundwater. In the event of a paint, fuel, oils, or chemical spill, the Contractor shall immediately take containment measures to prevent the spill from migrating and/or entering the AFP 42 drainage system. Hazardous spill residue shall be disposed of IAW all local, state, and federal laws pertaining to the protection of the environmental and emission of hazardous pollutants in the performance of this contract.
