Work shall be performed during normal duty hours (6:00 AM to 3:30 PM, Monday through Friday), with prior approval required for weekend or holiday work. The contractor must submit material approvals via AF Form 3000 and cannot begin work until all submittals are approved. Final payment may be withheld at five percent of contract value until all deliverables including product data sheets, operations and maintenance manuals, and training materials are submitted and accepted.

1.0 Introduction:

Contractor shall provide all supervision, labor, materials and equipment necessary to fully meet all requirements of the Statement of Work, Construction Specifications, Construction Drawings and all other Contract Documents related to and/or titled: “Sustain Doors and EIFS, B755” at the Minneapolis Air Reserve Station, Minneapolis Minnesota.

2.0 Scope of Work:

The tasks to be performed shall be based on the project description, other data furnished in this Statement of Work (SOW), and information covered during the Pre-Construction (Pre-CON) meeting.

The scope of this project is to repair the Base Exchange (Building 755) EIFS, doors, exterior lighting, and relocate sign and tank alarm conduit. The specific work areas are identified on the drawing(s). This shall include, but is not limited to:

a) Drawing (attachment 1)

b) Specifications (attachment 2)

2.1 Applicable Documents: The Contractor shall adhere to the following guidance during this project:

all requirements as shown in the contract documents. If there is a conflict between codes, regulations and specifications, the most stringent requirements shall apply. Documents listed by and/or provided by the Government are for reference only and do not relieve the Contractor from adherence with applicable codes not specifically addressed. Where conflicts between the drawings and specifications occur, specifications shall govern in accordance with FAR 52.236-21.

2.2 Background: The Base Exchange Facility serves all tenants of the Minneapolis Air Reserve Station, including active duty, reserve/guard, and veterans of the Air Force, Army, Navy, and Marine Corps.

2.3 General Expectations of Work: As part of the proposed effort, the Contractor shall implement means and methods that will limit disturbance to the base activities to the greatest extent practicable throughout the project performance. The Contractor shall accommodate continuous operation of the facility, in the scheduling of work, minimization of odors, and minimization of disruption to facility. Work shall be coordinated with the project manager identified at the pre-construction meeting. The Contractor shall ensure the work area is properly ventilated during construction to reduce the buildup of dust, noxious odors, fumes, and/or chemicals in the space.

2.3.1 The Government-provided documents and existing field conditions shall be reviewed by the contractor to identify any potential discrepancies and develop necessary requests for information (RFI) prior to construction. The repair efforts shall in accordance with the contract documents. Substitutions and/or deviations will not be accepted unless approved by the Contracting Officer (CO) in writing.

2.3.2 The contractor shall clean up all construction debris, restore disturbed areas, and dispose of all waste off site at the conclusion of work. Ancillary efforts required to access and complete the work not specifically listed in the task description shall be anticipated and accounted for in the contractor’s proposal documents.

2.3.3 The Period of Performance shall be no longer than 120 calendar days from issuing the notice to proceed (NTP). Contractors shall provide a construction schedule, with their proposal, breaking out all key work features along with a sequence of procedures, means of access, space for storage of materials and equipment, and use of approaches.

2.3.4 The task descriptions below are summaries of the primary work to be completed.

2.3.4.1 Exterior Insulation and Finish System (EIFS): Repair EIFS cracks, holes, voids, joints, and similar damages and apply new finish coat.

2.3.4.2 Sustain doors: Includes replacement of main entrance door operators, sensors, and controls; removal and brick infill of one double door; and replacement of a single door, frame, hardware, and accessories.

2.3.4.3 Relocate sign: remove sign and exterior conduit and mount new sign in designated location.

2.3.4.4 Tank Level Alarm Conduit: remove exterior conduit and reroute new conduit on the interior of the facility.

2.3.5 Deviations will not be accepted or considered by the Government without formal approval in writing from the CO. Criteria Drawings detailing the contracted work are included in SOW as Attachment 1. Contract Specifications are included in SOW as Attachment 2.

2.3.6 Site/Utility Verification: The contractor shall verify actual existing site conditions and identify potential conflicts prior to proposal in accordance with (IAW) contract clause 52.236-3.

2.3.7 Protect Non-Work Areas & Equipment: Install protective coverings and barriers to protect non-work areas and equipment remaining in the work area from damage, construction debris and dust during performance of the work IAW clause 52.236-9.

2.3.8 Upon approval by the CO for all preconstruction requirements, submittals, and applicable permits; the Contractor may commence on-site repair activities. The Contractor shall conform to all specific references and other applicable requirements during performance of the work as described in the Contract Documents.

2.3.9 Mobilization: Mobilize equipment, labor, tools, materials, and ancillary items necessary to complete the work defined in the Contract Documents.

2.3.10 Demolition/Construction: Perform awarded tasks as defined in this section.

2.3.10.2.1 Protect work areas with barricades, warning tape, signs and/or other appropriate methods for the work being completed to ensure the safety of pedestrians and Contractor personnel.

2.3.10.2.2 Remove and dispose of all demolition and general construction debris from the site (off base).

2.3.10.2.3 Perform ancillary efforts required to complete the work to provide a clean, complete and functional facility meeting the standard of quality for each trade utilized.

2.3.10.2.4 Limit disturbances to natural vegetation, wildlife and base activities to the greatest extent possible.

3.0 General Requirements

Applicable provisions of the Contract Clauses in the contract will govern work under each section of the specifications.

3.1 Pre-Construction (Pre-CON) Meeting: The contract specialist will schedule the Pre-CON. The PreCON will be accomplished with the Contractor, Contracting Officer (CO), Civil Engineering Squadron (CES), and user. The meeting objective is to review project details, construction start date, security procedures for site, dumpster locations, and any applicable requirements during construction.

3.2 Identification of Employees: The Contractor shall be responsible for obtaining a base pass for each employee, and for requiring each employee engaged on the work to display identification as approved and directed by the CO and Security Forces. The contractor shall submit requests for a base pass through the Base Contracting Office no later than 10 calendar days prior to the contract start date. Exceptions will be made for contracts where the award date and performance start date are less than 10 calendar days. Detailed information regarding the Contractor’s responsibility of security requirements is in the contract document.

3.3 Job Site Tools and Equipment: All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use. Construction attire shall be in accordance with applicable OSHA regulations.

3.4 Notification Requirements: The Contractor shall notify the Government a minimum of 21 days business days in advance of mobilization and start of work. Notification shall include submittal of schedule and traffic plan for work that requires closure of roads and/or parking stalls. The Contractor is required to notify the CO and Government Project Manager of critical issues that may affect the contract performance and/or human health and the environment. For critical issues pertaining to human health and the environment, the Contractor shall immediately notify the CO and stop work. Work shall only resume at the direction of the CO following resolution of the issue.

3.5 Environmental Protection: Submit timely notification of demolition and renovation projects to

Federal, State, regional, and local authorities in accordance with 40 CFR 61, Subpart M. Notify the Regional Office of the United States Environmental Protection Agency (USEPA), State's environmental protection agency and the CO in writing 10 working days prior to the commencement of work in accordance with 40 CFR 61, Subpart M. Comply with federal, state, and local hauling and disposal regulations. In addition to the requirements of the "Contract Clauses," conform to the safety requirements contained in ASSP A10.6. Comply with the Environmental Protection Agency requirements specified. Use of explosives will not be permitted.

In addition to requirements specified elsewhere for environmental protection, provide coating materials that conform to the restrictions of the local Air Pollution Control District and regional jurisdiction. Notify Contracting Officer of any paint specified herein which fails to conform.

3.6 Hazardous Waste Management: Contractor shall notify 934 CE if any hazardous waste is encountered or generated. The Contractor shall stop all work in the event 934 CE identifies noncompliance with federal and state regulations and shall correct any discrepancies immediately within two (2) hours of notification by 934 CE.

3.7 On-Site Field Oversight: The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure responsible oversight throughout accomplishment of the construction period. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this contract.

3.8 Federal Holidays: The Minneapolis Air Reserve Station observes the following Federal Legal Holidays:

3.9 Working Hours: The normal working hours for the base are from 6:00 AM to 3:30 PM, Monday through Friday, excluding Federal Holidays. All work shall be performed during normal duty hours.

All other hours to include weekend and holidays shall be requested through the CO for approval.

This request shall be submitted no less than five (5) business days prior to the time requested.

3.10 Availability and Use of Utility Services: The Government will make all reasonably required utilities available to the Contractor from existing outlets and supplies, as specified in the contract.

3.11 Sanitation: The Contractor shall provide and maintain within the construction area minimum field-type sanitary facilities approved by the CO. Government toilet facilities will not be available to Contractor's personnel.

3.12 Preparation of Progress Schedules and Reports: The reports contemplated by Clause 52.236-15, Schedules for Construction Contracts, shall be accomplished on and in accordance with instructions pertaining to AF Form 3064, Contract Progress Schedule, and AF Form 3065, Contract Progress Report. Contract Schedules and Reports shall run from Monday through Sunday and be submitted to the CO. Contractor’s Contract Progress Schedules and Phasing Plans shall be provided with the proposal.

4.0 General Safety Requirements

4.1 Workplace: Provide a safe and healthy workplace for employees and the public while performing the contract work.

4.2 Personal Protective Equipment (PPE): Supply each employee with personal protective equipment.

This equipment includes but is not limited to protection for the eyes, face, head, ears and extremities. Use the proper protective equipment for the work done.

4.3 Roads: Obey the posted base speed limits. Wear Seat belts on base in accordance with AFI 91207.

4.4 Accidents: Report Contractor accidents involving injury to Air Force personnel or damage to

Government property to the Base Law Enforcement Desk and the CO.

4.5 Safety Regulations: Familiarize yourself and comply with the general safety requirements in accordance with DOL regulations, OSHA 29 CFR Part 1926, as well as applicable Air Force Office of Safety and Health (AFOSH) standards and Air Force Instructions (AFI).

4.6 OSHA Inspections: The Contractor shall be subject to no-notice inspections under OSHA program by inspectors of the Department of Labor.

4.7 Fire Regulations and Preventative Practices: The Contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association (NFPA), other recognized fire prevention agencies, and base regulations.

4.8 Explosive Operated Hand Tools: The Contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one day’s supply. The Contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.

4.9 Hot Work: Drilling, soldering, brazing, tapping, grinding, heat treating, chipping, thawing, abrasive blasting, and welding shall not be performed without first obtaining a hot work permit issued by the Base Fire Department.

5.0 Generated Refuse The Contractor shall take positive action to prevent work-generated refuse from entering the sewer system and water bodies. All Contractor generated refuse, and waste shall be hauled from the construction site to a disposal area located outside the physical boundaries of the installation.

6.0 Deliverables/Submittals

6.1 Contract Progress Schedule: The Contractor shall submit a signed AF 3064 (or approved equivalent), Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance. The line items associated with project percentages and dates shall accurately depict the planned work to be performed. The Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance. The progress schedule shall be signed by the either the COR or the government contract inspector recommending approval and approved by the CO. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF 3065, an appropriate percentage applied, and the item must be received and stored on site.

6.2 Contract Progress Report (AF 3065): The Contractor shall submit AF 3065. This report shall be signed by the Contractor’s on-site representative in the Remarks section of the AF 3065. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. The Contractor shall obtain either the COR or the Government contract inspector’s acceptance by signature of the actual reported progress prior to submission to the CO. Additionally, the Contractor shall submit an updated AF 3064 as an attachment to each submission of the AF 3065.

6.3 Material Approval (AF Form 3000): The Contractor shall prepare and submit the AF Form 3000 with all submittals to the Government in accordance with the AF Form 66. The form shall document the contents of the submittal and be signed by the Contractor. The Contractor shall not start any work before all material approval submittals approved. Upon receipt of each material submittal the Government has 14 calendar days to review and respond. The Contractor owns burden of managing project time and of soliciting material submittal approval in accordance with project schedule needs.

6.4 Hazardous Material and Hazardous Waste: The Contractor shall manage and dispose of hazardous waste in accordance with Section 3.6.

6.6 Pre-Final Inspection: The contractor shall request a Pre-Final Inspection through 934 CONF at least five (5) business days prior to the intended date of the Pre-Final Inspection. The date of the Pre- Final Inspection must allow adequate time to correct all deficiencies and conduct the Final Inspection prior to the construction completion date. A contractor-provided punch list, including the status and projected completion date for all open items shall accompany this Pre-Final Inspection request.

6.6 Final Inspection: The Contractor shall conduct a final inspection with base personnel and publish the findings in a final inspection report for each project task upon substantial completion of work.

The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection (punch list) report. A final inspection shall not be performed until the pre-final inspection (punch list) report has been resolved.

6.7 Delivery/Warranty: The Contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the Contractor shall deliver to the Government the warranty documentation. The warranty shall be issued in accordance with FAR 52.246-21.

7.0 Contractor’s Quality Control

7.1 Quality Control: The Contractor is responsible for providing a quality control plan and shall establish and maintain an effective quality control system in compliance with FAR Clause 52.24612, Inspection of Construction. The quality control system shall consist of plans, procedures, and organization necessary to manage all delivery orders to produce end products, which comply with the contract requirements. The system shall cover all construction operations, both on site and off site, and shall be keyed to the proposed construction sequence. The Government will hold the project manager responsible for the quality of work on the job and is subject to removal by the CO for non-compliance with quality requirements specified in the contract. The Contractor shall verify full compliance with the contract documents prior to requesting final inspection(s).

7.2 Field Measurements: The Contractor shall be required to make his/her own field investigations to verify existing conditions, dimensions and other information shown on Government furnished reference drawings, solicitation documents and construction documents. Deviations, errors, or omissions in the Government-supplied reference materials or the contract documents shall be identified and addressed by the contractor prior to the start of construction.

7.3 Siting Location: The contractor is responsible for field reconnaissance, surveys, site investigations and testing required to obtain information necessary for the accomplishment of the project in accordance with the contract documents.

8.0 Contract Administration

8.1 Contracting Officer Authority: CO, the term used herein, does not include any representative not acting within the scope of his/her authority. Notwithstanding any of the provisions of this contract, the CO shall be the only individual authorized to in any way amend or modify the terms of this contract.

8.2 Field Changes: The Contractor and the Government may agree to perform a no cost field change.

Field changes are made when the change appears to be mutually beneficial to all parties and would not require changing the negotiated items. The CO must approve all field changes prior to execution.

8.3 Closeout Documents: Five percent (5%) of the contract value may be withheld as a final payment until all project deliverables such as Product Data Sheets, O&M Manuals, and Training Materials are submitted to and accepted by the Government.
