STATEMENT OF WORK

I. General Description:
Provide Construction Services for Project: Construct Material Shed
PROJECT NO: 676-233
PROJECT NAME: Construct Material Shed
II Project Scope:
1. General Description:
Provide Construction Services for Project #676-233, Construction of 4,800 square foot Materials storage building.
1. Contractor shall provide for and furnish all labor, materials, equipment, qualified supervision, and other items designated in this contract in accordance with the specifications and drawings to complete Project # 676-233 Construct Material Shed at Tomah VA Medical Center, 500 E. Veterans St, Tomah, WI.
2. All work shall be completed within 245 calendar days after receiving Notice to Proceed.
3. The C&A Requirements do not apply, a security Accreditation Package is not required.
This project shall be completed within Two Hundred Forty Five Days (245) days once NTP is issued.
• Phase 1: Pre-construction (kick-off) Meeting is anticipated to be scheduled on the effective date identified in the Notice to Proceed (NTP).
• Phase 2: Pre-Construction Accident Prevention Plan and Construction Schedule, to be completed no later than NTP + 30 days. Within this period the contractor shall submit the Accident Prevention Plan, submit the Construction Schedule, Submittal Registry, Product Submittals, Submittal Exchange Subscription, and schedule a review meeting with the COR and VA Safety.
• Phase 3: Pre-Construction Period duration is NTP + 60 calendar days (not to exceed). Within this period the contractor shall provide a submittal exchange subscription for the VA and submit the following (as applicable per the specification requirements):
1. Construction Phasing Plan.
2. Accident Prevention Plan.
3. Activity Hazard Analyses.
4. Construction Schedule.
5. Schedule of Values.
6. Existing Conditions Survey.
7. Infection Control Plan.
8. Quality Control Plan.
9. Environmental Protection Plan.

10. Document Submissions.
11. Demolition Debris Management Plan.
12. Certificates, Licenses, and Training Documents.
13. Obtain PIV badges.
14. Material Procurement and Delivery
• Phase 4: Pre-Construction VA Review Period. Phase 4 is anticipated to be 30 calendar days and shall be completed no later than NTP + 90 calendar days. Within this period the VA will complete all review and approvals of contractor Pre- Construction submittals.
• Phase 5: Construction Period – Phase duration is anticipated to be 120 days; to be completed no later than NTP + 210. All applicable documentation listed in Phase 2 and any necessary submittals required to mobilize must be approved by the COR prior to commencing work. No demolition or construction shall be started without COR review and approval of those items listed in the Pre-Construction Period unless authorized in writing by the COR or the CO. Should naturally occurring weather delays occur, the contractor shall restart construction immediately once conditions permit.
• Phase 6: Construction Closeout Period duration is NTP + 245 calendar days. The contractor shall provide all required materials to include the following within 45 days of completion of Phase 5.
1. Inspection and Approval by the COR and/or the CO.
2. Final Approval and Beneficial Occupancy when applicable.
3. Warranties documents and O&M Manuals.
4. As-built drawings and record submittals.
5. Staff training when applicable.
6. Final Waste Reports.
7. Existing Conditions Re-survey.
8. Release of Claims.
