STATEMENT OF WORK

FOR

HIGH SPEED OVERHEAD DOOR

1206, 4870)

1. INTRODUCTION:

1.1. Eielson AFB (EAFB) is replacing 17 existing overhead doors with high speed fabric overhead doors and ancillary controls at three facilities across base.

1.2. LOCATION. The doors to be replaced are at buildings: 6203, Central Heat & Power Plant

(CH&PP) Thaw Shed (four doors) and Ash Shed (two doors); 1306, Jack’s house (two doors);

and 4105, Snow Barn (four doors); 1206, Fire Station #1 (two doors); 4870, Fire Station #2 (three doors)

2. PROJECT OBJECTIVES:

2.1. The objective of this project is for the Contractor to provide, deliver, remove existing, and install 14 new high speed fabric/3metal overhead doors and ancillary controls across various facilities on base per the below description of work.

2.2. Description of Work.

BLD Item Description

6203 Replace current overhead doors with high-speed fabric doors and ancillary controls at building 6203 CH&PP Thaw Shed and CH&PP Ash Shed

Work in these areas shall be conducted during summer months only due to the need to keep coal flowing.

CH&PP Thaw Shed

- Replace two (2) overhead doors (6203-3 and 6203-4) with high speed fabric overhead doors which are located over railroad tracks. These doors are located in a Class II, Division 1, Group F (coal dust) area, as defined by the NEC. All wiring, conduit, electrical fittings and fixtures provided and installed by the contractor shall be appropriate for this hazardous environment.

- Replace two (2) overhead doors (6203-5 and 6203-6) with high speed fabric overhead doors going over railroad tracks.

- Replace existing overhead door controls with controls for new doors which match the safety requirements stated.

CH&PP Ash Shed

- Replace two (2) overhead doors (6203-1 and 6203-2) with high speed fabric overhead doors.

- Replace existing overhead door controls with controls for new doors.

1306 Replace current overhead doors with high-speed fabric doors and ancillary controls at building 1306, Jack’s House

- Replace two (2) overhead doors (1306-1 and 1306-2) with high speed fabric overhead doors.

- Replace existing overhead door controls with controls for new doors.

- Work can be complete at any time of the year. Contractor to maintain 65 degrees Fahrenheit in facility at all times.

4105 Replace current overhead doors with high-speed fabric doors and ancillary controls at building 4105, Snow Barn

- Replace four (4) overhead doors (4105-2, 4105-3, 4105-5, 4105-6) with high speed fabric overhead doors.

- Replace existing overhead door controls with controls for new doors.

- Work can be complete at any time of the year. Contractor to maintain 65 degrees Fahrenheit in facility at all times.

1206 Replace current overhead doors with high-speed fabric doors and ancillary controls at building 1206, Fire Station #1

- Replace two (2) overhead doors (1206-1, 1206-2) with high speed fabric overhead doors.

- Replace existing overhead door controls with controls for new doors.

Work can be complete at any time of the year. Contractor to maintain 65 degrees Fahrenheit in facility at all times.

4870 Replace current overhead doors with high-speed insulated doors and ancillary controls at building 4870, Fire Station #2

- Replace three (3) overhead doors with high speed metal overhead doors with high R value.

- Replace existing overhead door controls with controls for new doors.

- Work can be complete at any time of the year. Contractor to maintain 65 degrees Fahrenheit in facility at all times.

2.3. Existing Conditions. Existing door descriptions and as-built’s are provided as follows:

Attachment A – Existing Overhead Doors; Attachment B – B1306 Dwgs; and Attachment C – B4105 Dwgs, Attachment D – B1206 Dwgs, Attachment E – B4870 Dwgs

2.4. Delivery. Delivery and installation shall be completed 225 calendar days after Notification to Proceed (NTP).

3. INSTRUCTIONS TO THE CONTRACTOR:

3.1. The Contractor shall remove all existing doors and controls and install all necessary equipment to complete the requirement and meet stated safety requirements. Names of Government Representatives from the 354th Civil Engineer Squadron will be provided upon contract award. The installer shall validate the system performance is in full working condition upon completion of each CLIN.

3.2. Quality Assurance. The Government shall periodically inspect the Contractor during installation. Any discrepancies discovered will be reported to the Contractor for remedial action. The Government Representative will have the final decision on the acceptance of work carried out and ensure that the final installation of the high speed fabric overhead doors and associated controls are fully operational. The doors and controls must be 100% compatible and in working order with the Eielson AFB provided utility hookups.

3.3. Standards: The contractor shall adhere to all relevant NEC, UFC, and base structures guidelines.

GOVERNMENT SPECIAL INSTRUCTIONS, TERMS AND CONDITIONS

4. GOVERNMENT FURNISHED EQUIPMENT/PROPERTY:

4.1. There will be no government furnished equipment/property.

4.2. The Contractor shall provide the following:

4.2.1. Tools, labor, and equipment needed for installation.

4.3. Toilet Facilities. Contractor will need to provide toilet facilities.

4.4. Emergency Service. The government will provide emergency service, emergency medical treatment, and emergency patient transportation service for Contractor personnel who are injured or become critically ill while at Eielson AFB. Treated personnel will be responsible for reimbursing the government for the cost of medical treatment and patient transportation. For all emergencies using an on-base phone, please dial 911. If using a cell phone, please dial (907) 377-5130 (Security Forces Law Enforcement Desk/BDOC). Fire emergencies/routine calls can also be reported at the before mentioned phone numbers.

4.5. Telephone. The government shall not provide telephone services. If available, the Contractor may use government phones in a case of emergencies during field and site work.

No long distance telephone calls will be authorized.

4.6. Damage to Property or Equipment. The Contractor, at his own expense, shall repair any damage to Government property or equipment that can be attributed to the Contractor.

Said repairs shall be made by a skilled workman, and to the satisfaction or the Contracting Officer or a designated representative.

5. GENERAL INFORMATION:

5.1. Hours of Operation. The worksites at buildings 6203, 4105, 1206, 4870 shall be available for access weekdays and weekends. Work is permitted during and after normal business hours for these sites. Building 1306 shall only have access weekdays and during

SOW – HS OH Door Replace (1306, 6203, 4105) 2 February 2022 Page 5 of 7 normal business hours of 0730 – 1630. Quality inspections will be conducted on weekdays between 0730-1630. If the Contractor requires a government representative to be present, the Contractor shall make his/her request to the Contracting Officer five (5) calendar days in advance.

5.2. Disposition of Waste, Excess, Scrap, and Salvage Materials. The title to all waste, excess, scrap, and salvage materials generated as a direct result of this contract is vested in the Contractor - unless specifically excepted below. Further, these materials shall be disposed of off base by the Contractor in accordance with all applicable Federal, State, and Local Regulations.

5.3. Clean Up. The Contractor shall, keep the work site and storage area(s) free from accumulation of waste, rubbish, or construction debris. All loose or lightweight materials shall be secured to prevent blowing or scattering. The burning of trash or construction debris is strictly prohibited. Prior to final inspection, the Contractor shall remove all construction debris, tools, equipment, and materials not the property of the Government.

Upon completion of the work, the Contractor shall leave the work site area(s) in a clean, neat and workmanlike condition satisfactory to the Contracting Officer.

5.4. Recognized Holidays. Any of the below holidays falling on a Saturday will be observed on the preceding Friday; holidays falling on a Sunday will be observed on the following Monday.

6. SERCURITY REQUIREMENTS:

6.1. Levels of Security. Eielson AFB security may be viewed as being one of three broad levels, the level in effect being dependent on the location or AREA of Eielson AFB in question.

6.1.1. The lowest level of security exists in “GENERAL ACCESS” areas. These areas are all of Eielson AFB outside of the "USAF Controlled" and "USAF Restricted" areas.

6.1.2. “USAF Controlled” access areas are the mid-level security areas. Presence within the CONTROLLED ACCESS AREA is by authorization; however, the movement of authorized personnel in and out of these AREAS is generally not impeded.

6.1.3. The highest level of security on Eielson AFB is that within the “USAF RESTRICTED AREAS”. Work schedule requests or changes to existing schedules for work to be performed in a “USAF RESTRICTED AREA” shall be submitted at least 7 calendar days in advance for approval and coordination of Eielson AFB Security Forces.

6.2. Base Access. Primary site access shall be via Hursey Gate. The Installation Entry Controller (IEC) will conduct an identification credential check on all vehicle operators; all passengers are subject to random ID checks during random anti-terrorism measures or at the discretion of the IEC. All credential checks are to be a hands-on, out-of-the-wallet, front and back inspection. IEC’s will conduct random 100 percent identification checks of all personnel entering the installation. IEC’s will deny entry to all personnel who are not in possession of a valid picture ID. The Contractor shall be responsible for and shall pay for obtaining all necessary clearances, cards, passes, decals, or other items required for access of Contractor personnel and vehicles to the base or other areas where work will be performed. The Contractor shall be required to apply for vehicle and personnel identification media at the Base Pass and Registration Section (Visitor Control Center). Upon Completion or termination of the contract, the Contractor shall ensure all vehicle and personnel identification media are surrendered to the Base Pass and Registration Section (Visitor Control Center). The Contractor shall obtain the following pass and identification items:

Eielson AFB Vehicle Pass. For all further inquiries, please contact the Visitor Control Center at (907) 377-3807.

6.2.1. The Contractor shall maintain a current listing of employees. The list will include employee names, last four digits of social security numbers, and driver license numbers.

This list shall be provided to the Contracting Officer (CO), and servicing Security Forces organization. An updated listing shall be provided upon change of employees. The Contractor must provide all names, last four digits of social security numbers and duty hours of all personnel who will be on site at the start or anytime during this project for identification purposed in case of emergencies to base security.

6.2.2. Access to the CH&PP Thaw Shed and CH&PP Ash Shed require base escorts.

These escorts will be provided upon the Contractor making his/her request to the Contracting Officer five (5) calendar days in advance.

6.3. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Eielson Air Force Base.

6.4. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware that may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

6.5. Physical Security:

6.5.1. The Contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.

6.5.2. The Government and their representative shall have access to the work area for inspection at all times. Entry into the work area by unauthorized individuals shall be reported immediately to the project supervisor and the Government representative.

6.5.3. The Contractor shall remain in compliance with all rules, codes, standards, and regulations governing the safety of all individuals at the worksite and shall be solely responsible for any injuries, accidents, exposures or liabilities occurring due to non-compliance or failure to secure the work area.

7. MISCELLANEOUS:

7.1. Cell Phone Usage. Use of cell phones while operating any motor vehicle on base is strictly prohibited. Driving privileges will be suspended for 30 days for the first violation.

7.2. Speed Limits. The on-base speed limit while passing military troops performing physical training is 10 MPH and is strictly enforced.

7.3. Coordination with Other Contractors. The Government may undertake or award other contracts for additional work. The Contractor shall cooperate with the other Contractors and Government employees and adapt scheduling and performing the work under this contract to accommodate the additional work, heeding any direction that may be provided by the Contracting Officer. The Contractor shall not commit or permit any act that will interfere with the performance of work by any other Contractor or by Government employees.

7.4. No Smoking Policy. The Contractor and employees shall comply with policies stating that smoking is prohibited within any Air Force Facility and use designated outdoor smoking areas.

7.5. Performance of Work During National Crisis. In accordance with DODI 3020.37 "Continuation of Essential DoD Contractor Services During Crises”, all services or any specifically designated portions of the functions accomplished under this contract shall be performed during all wartime operations. Wartime operations are those actions including wartime commitments and contingency planning and exercises necessary to support current or future United States Air Force wartime commitments. Contractor personnel will be advised of the functions that will be continued during wartime-related contingencies. The Government shall have the unilateral right to extend performance of this contract as long as the wartime contingencies exist. Contract payment will be in accordance with the existing contract rate.
