About this file

This is a Past Performance Questionnaire template used by the U.S. Army Contracting Command - Redstone Arsenal to evaluate contractor performance on federal contracts relevant to the Inert Warhead for Reduced Range Practice Rocket (RRPR) solicitation (W31P4Q-26-R-0013). The questionnaire is distributed to points of contact familiar with offerors' past performance on recent and relevant contracts within NAICS classification 336419 (Other Guided Missile and Space Vehicle Parts and Auxiliary Equipment Manufacturing). Completed questionnaires must be returned within 15 days of receipt to monica.e.clemons.civ@army.mil and allison.c.beason.civ@army.mil.

The questionnaire uses a four-tiered rating system—Substantial Confidence, Satisfactory Confidence, Limited Confidence, and No Confidence—to assess contractors across five performance criteria: compliance of products and deliverables to specification requirements and workmanship standards; effectiveness of project management and subcontractor control; timeliness of performance; commitment to customer satisfaction and business-like concern for customer interests; and general relevant performance information. Respondents must provide rationale for each rating and identify other relevant past efforts and points of contact. Part I collects contract identification information completed by the offeror, while Part II requires the point of contact respondent to complete the performance evaluation ratings with detailed rationale, and Part III directs return submission to the contracting officer.

ACC-RSA Solicitation No. xxxx Attachment xxxx

The US Army Contracting Command – Redstone Arsenal is currently conducting a competitive source selection to evaluate offerors on the subject RFP. As part of this evaluation, we have requested that the offerors provide information about their past performance on recent and relevant federal, state, or local government or commercial contracts as compared to the North American Industry Classification System [NAICS] 336419, Other Guided Missile and Space Vehicle Parts and Auxiliary Equipment Manufacturing.

You have been identified as the point of contact for the below requirement. Your assessment of the contractor’s performance is extremely valuable to our evaluation. Therefore, it is important that your information be as factual, accurate and complete as possible to preclude the need for follow-up by the evaluators. Please complete the questionnaire below and return to the Contracting Officer, no later than 15 days after receipt of request or (insert date), whichever occurs first. Submit your completed questionnaire to monica.e.clemons.civ@army.mil and allison.c.beason.civ@army.mil .

Please consider the rating definitions below in completing the questionnaire:

Substantial Confidence: Performance meets contractual requirements and exceeds many requirements that benefit the end user. Work was accomplished with few, if any, minor problems for which corrective actions taken by the contractor were highly effective. The offeror has been highly successfully in performing the required effort.

Satisfactory Confidence: Performance meets contractual requirements and exceeds some requirements that benefit the end user. Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective. The offeror has successfully performed the required effort.

Limited Confidence: Performance does not meet some contractual requirements. Serious problems with contractor performance were experienced for which the contractor has either not yet identified corrective actions or the corrective actions taken appear only marginally effective. The offeror has had little success performing the required effort.

No Confidence: Performance does not meet most contractual requirements. Serious problems with contractor performance were experienced for which the corrective actions were ineffective. The offeror has not successfully performed the required effort.
