About this file

This is a Performance Assessment Questionnaire template used to evaluate past performance for Task Order Proposal Request W519TC-25-R-0027 (ADA IR) issued by Army Contracting Command - Rock Island. The questionnaire is to be completed by references provided by offerors who are DRAID BOA holders competing for this opportunity under NAICS 518210 (Data Processing, Hosting, and Related Services).

The questionnaire requires detailed performance evaluation across five key areas: compliance with specifications, project management effectiveness, timeliness of performance, cost control effectiveness (for cost reimbursement contracts only), and commitment to customer satisfaction. Responses use a five-level confidence rating scale from "No Confidence" to "Substantial Confidence." The resulting task order will be a single Firm-Fixed-Price award with a 12-month base period and one 12-month option period. Completed questionnaires must be returned to Contracting Officer Brian Schou and Contract Specialist Hila Anderson, with responses due by the date specified by the requestor.
