The questionnaire requires respondents to rate contractor performance across five evaluation categories using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Too Soon to Tell): Quality of deliverables including product performance and workmanship; Management and Business Relations including problem identification, corrective actions, and subcontract management; Schedule performance and timeliness of deliveries; Regulatory Compliance with financial, environmental, safety, and labor requirements; and General Comments for additional relevant information. For any adverse ratings, respondents must assess the extent of contractor fault versus customer/end user fault or external factors beyond the contractor's control. The questionnaire includes sections for identification of relevant contracts, offeror representative information, and respondent contact details for potential follow-up by the government evaluation team.
