About this file

This is a Required Affidavit for Resident Bidder Status document associated with RFB 758 2100000067, applicable to an unspecified Kentucky state or local government agency. The affidavit establishes the legal requirements that bidders, offerors, and contractors must satisfy to claim resident bidder status when submitting bids or proposals. Bidders must affirm under penalty of perjury that their entity—whether an individual, partnership, association, corporation, or other business entity—meets specific criteria as of the contract advertisement date, including authorization to transact business in the Commonwealth of Kentucky.

To qualify for resident bidder status, entities must demonstrate that they have filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund, and maintained a current Kentucky workers' compensation policy for the one-year period preceding and through the date of contract advertisement. The bidding agency reserves the right to request supporting documentation to verify resident bidder claims, and failure to provide such documentation upon request will result in bidder disqualification or contract termination. The affidavit requires the bidder's signature, printed name, title, date, company name, and address, as well as notarization before a notary public.
