About this file

This is a Required Affidavit for Resident Bidder Status document related to Solicitation/Contract #RFB 758 2000000514. The affidavit establishes the eligibility criteria that bidders, offerors, and contractors must meet to claim resident bidder status in Kentucky. To qualify as a resident bidder, the entity must be authorized to transact business in the Commonwealth of Kentucky and must meet three specific requirements as of the contract advertisement date: filed Kentucky income taxes for the one-year period prior to and through the advertisement date, made payments to the Kentucky unemployment insurance fund during that same period, and maintained an active Kentucky workers' compensation policy throughout the year leading up to the contract advertisement.

The bidding agency retains the authority to request documentation from any bidder claiming resident bidder status to verify compliance with these requirements. Failure to provide requested supporting documentation upon demand will result in the bidder's disqualification or contract termination. The affidavit requires notarization and must be executed under penalty of perjury, with the affiant providing their signature, printed name, title, date, company name, and address, along with notary attestation and seal.
