About this file

This is a Resident Bidder Affidavit document required for solicitation RFB 758 2000000263. The affidavit establishes the requirements for bidders, offerors, and contractors claiming resident bidder status under Kentucky Revised Statutes (KRS) 45A.494(2). To qualify for resident bidder status, an entity must be authorized to transact business in the Commonwealth of Kentucky and must demonstrate continuous business operation within the state for one year prior to and through the contract advertisement date. The affidavit must be executed before a notary public and include the affiant's printed name, title, company name, address, and the notary's seal and commission expiration date.
