About this file

This is a Required Affidavit for Resident Bidder Status document associated with Solicitation/Contract #RFB 758 2000000014 for an unspecified Kentucky state agency. The affidavit establishes the requirements and process for bidders, offerors, and contractors to claim resident bidder status in accordance with KRS 45A.494(2). Bidders must affirm under penalty of perjury that their entity is authorized to transact business in the Commonwealth and has maintained continuous compliance with Kentucky regulatory requirements for a minimum of one year prior to and through the contract advertisement date. The affidavit requires the bidder's signature, printed name, title, date, and company information, along with notarization.

To qualify for resident bidder status, entities must demonstrate that they have filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund, and maintained an active Kentucky workers' compensation policy during the one-year period preceding the contract advertisement. The bidding agency reserves the right to request supporting documentation to verify resident bidder claims, and failure to provide requested documentation upon demand will result in immediate disqualification of the bidder or termination of the contract if already awarded.
