About this file
This is a Contractor Work Surveillance Checklist specifically for Industrial Cleaning Services (ICS) CLINs 1002SV-1004SV & 1007SV-1009SV, which is part of a hazardous waste removal contract for the San Joaquin region.
The checklist enables government representatives to monitor contractor compliance across eight key areas: timely arrival, required equipment presence (including a 3,500-gallon vacuum truck with 300 feet of hoses, heated power washing unit, liquid transfer pump, and PPE), proper documentation of truck weight, government inspection completion, tank cleanliness and chemical compatibility, RCRA compliance for waste residue, equipment operation without delays, and overall work satisfaction. The form includes spaces for recording surveillance dates, times, government representative information, yes/no/N/A responses, comments, and an official signature.
