About this file

This is a Contractor Work Surveillance Checklist specifically designed for monitoring the placement or removal of empty rented Rolloff Box (ROB) or Vacuum Box containers. The checklist is part of solicitation SP450025R0005 for hazardous waste removal services in the San Joaquin region.

The checklist contains verification items that a Designated Government Representative must complete, including: confirmation of contractor's arrival time, verification of required equipment (appropriately sized ROB/Vacuum Box, plastic liner, watertight tarp cover), inspection of container condition and integrity, verification of weight tickets, and assessment of equipment operation and safety compliance. The form requires documentation of any defects or damage, with a note that failure of the driver to acknowledge existing defects will result in rejection of the ROB and liner. The checklist includes fields for contract information, representative details, and requires a final signature and date from the government representative upon completion of the inspection.
