PRIME CONTRACTOR INSTRUCTIONS: All Subcontractors shall be listed on the chart above – the PRIME will mark the appropriate column to either confirm if a subcontractor e-mailed their PROPOSAL directly to SAMHSA or confirm that the subcontractor did not e-mail a proposal and their information can be obtained in the eCPS system.
NOTE: Sealed CD’s of the subcontractors proposal will no longer be accepted.
Subcontractors’ that require the submission of an e-mailed proposal MUST provide the following documentation in one compressed file format (zipped):
· Narrative on the subcontractor;
· Subcontractors excel budget spreadsheet with staff names and rates
· NICRA
· Payroll documentation
· All of the subcontractors proposal documentation shall be compressed (zipped) into one file and submitted to the designated email address listed below. Only a compressed (zipped) file will be accepted.
Submission instructions:
Offerors are responsible for ensuring that their Subcontractors Proposal Packages are submitted and received at the following designated email: SAMHSAContracts@samhsa.hhs.gov , by the date and time specified for receipt in the RFP letter.
All e-mailed subcontractor proposals must provide the following information in both the Subject Line and the body of the e-mail:
· Solicitation number: 277-22-0576
· Name of the Prime Contractor Please allow ample time to ensure e-mailed proposals are received by the specified receipt date.
