About this file
This is a Cost Proposal Attachment document for a contract opportunity involving the Indiana State Department of Toxicology and the Indiana Department of Health. The contract requires respondents to provide pricing for laboratory testing services across four primary test categories: Blood Testing, Urine Testing, Vitreous Testing, and Non-Screening (Confirmation) Testing. All pricing must be inclusive of shipping and associated costs. The contract term spans five years, with the initial contract period covering Years 1 through 4, with Year 5 included in the cost proposal template. Respondents must submit all-inclusive prices for each testing component with no additional costs accepted by the State.
Cost evaluation is structured around tiered pricing based on annual test volumes, with respondents required to provide separate per-test costs for three volume tiers: 1-500 tests, 501-1,000 tests, and 1,001 or more tests. These tiers reflect workload patterns observed in fiscal years 2025 and 2026 across both agencies, though the State does not guarantee specific test volumes. The Total Bid Amount, calculated as the sum of Years 1 through 4 costs, serves as the basis for cost proposal evaluation. Respondents must populate all yellow-shaded cells in the cost proposal template without altering formatting or adding caveats, with failure to comply potentially resulting in cost score disqualification. Subcontractor commitments must reference the Total Bid Amount in cell E25 when calculating commitment percentages.

Instructions
(Initial Contract Period)
Please populate ALL cells shaded in YELLOW.
Please do NOT alter the formatting of the cells or add caveats to your pricing or to the cells -- as this may potentially put your cost score at risk of disqualification.
The TOTAL BID AMOUNT will be the sum on which your cost proposal will be evaluated. This amount will be automatically calculated based on the amounts entered for years 1, 2, 3 and 4. Prices must be submitted for all components. Listed prices must be all-inclusive. No additional costs will be accepted by the State.
Cost Proposals will be evaluated based on the initial contract term, or Total Bid Amount.
For Subcontractor commitments, Respondents should use the Total Bid Amount in cell E25 when figuring the commitment percentage.
Cost evaluation will be completed by evaluating specific prices for increased amounts of tests completed.
Respondents must provide the cost per test for each of the following tiers: 1–500 tests, 501–1000 tests, and 1001 or more tests. These tiers are informed by workload patterns observed in fiscal years 2025 and 2026 across both Indiana State Department of Toxicology and the Indiana Department of Health and are used to evaluate how pricing scales under varying annual test volumes. While the State does not guarantee that any specific number of tests will be requested, the tiered structure supports consistent cost modeling and comparability among Respondents. All prices submitted must be all‑inclusive, including shipping and any associated costs.
