About this file

This is a cost proposal template for RFP 26-84336, a Pharmacy Management contract opportunity with the State of Indiana for two locations: Evansville and Richmond. The contract encompasses comprehensive pharmacy services administration across both facilities for an initial four-year term (SFY2028-SFY2031), with optional two one-year extensions (SFY2032 and SFY2033). Respondents must provide pricing for all service components at both locations, which include Pharmacy Management (monthly unit of measurement with 12 estimated units), Director of Pharmacy (full-time onsite leadership at 1,950 hourly units), Registered Pharmacist clinical and dispensing services (1,950 hourly units), Certified Pharmacy Technician support services (1,950 hourly units), and 24/7 on-call after-hours support (monthly with 12 estimated units). The cost proposal requires respondents to populate all yellow-shaded cells with pricing for each fiscal year, with annual costs automatically calculated and aggregated into location-specific bid amounts and a total bid amount across both facilities.

All pricing submitted must be all-inclusive with no additional costs accepted by the State, and respondents are prohibited from altering cell formatting or adding caveats to pricing under penalty of cost score disqualification. Cost proposals will be evaluated based on the four-year initial term total bid amount. Minority Business Enterprise (MBE) and Women Business Enterprise (WBE) subcontractor commitment percentages should be calculated using the Evansville bid amount (cell F23) or Richmond bid amount (cell F23) if bidding on only one location, or the Total Bid Amount if bidding on both locations. The estimated units of measurement provided are for evaluation purposes only, with actual work potentially varying from projections. Respondents bidding on both locations must provide separate cost proposals for the Evansville and Richmond components, which are then consolidated into a single total bid amount for evaluation.
