About this file
This file is an Applicant Information, Certification and Cover Sheet for RFA 6127 (OB# S-44300-00013845) issued by the State of Oregon for Lead-Based Paint Remediation services. The document serves as the cover sheet and certification form that applicants must complete and submit as part of their application packet. Applicants are required to provide their legal name, address, state of incorporation, entity type, and primary contact information. By submitting the completed form, applicants certify their understanding and acceptance of all RFA requirements, contract terms and conditions outlined in Attachment A, and any amendments or addenda issued. Applicants further agree to perform the scope of work and meet performance standards as set forth in the final negotiated contract scope of work.
The certification requirements outlined in this document establish multiple compliance obligations for applicants, including certification of tax law compliance under Oregon Revised Statutes (ORS 305.620 and chapters 316, 317, and 318), non-discrimination in employment and subcontracting practices, and compliance with wage and hour laws (ORS 652.220). For contracts with an anticipated price of $150,000 or more, applicants must certify implementation of a written policy and practice preventing sexual harassment, sexual assault, and discrimination as required under ORS 279A.112. Applicants must also certify that they and their employees, agents, and subcontractors are not listed on the Office of Foreign Assets Control's Specially Designated Nationals and Blocked Persons list or government-wide exclusion lists in the System for Award Management. Additionally, applicants must certify the absence of actual or potential conflicts of interest with the State and that all application contents have been prepared independently without collusion or fraud, acknowledging that false statements or misrepresentations may constitute claims under the Oregon False Claims Act.
