About this file
This is a Proposal Form (Attachment B) for a state of Oregon contract opportunity involving the completion of a housing needs analysis. The RFP requires proposers to submit comprehensive proposals addressing multiple evaluation criteria, including detailed project plans, team qualifications, and relevant past experience. The proposal must be completed in its entirety, with failure to respond to any section potentially resulting in proposal rejection. Proposers must demonstrate minimum qualifications including at least one previous housing needs analysis for a large city, county, or state within the last ten years, and must identify Key Persons with minimum five years of relevant consulting and data analysis experience in housing needs for large jurisdictions. The proposal submission must include resumes for Key Persons (limited to two pages each), a comprehensive project plan as a separate document detailing the proposer's approach to the major activities outlined in the RFP specifications, a list of similar projects completed within the last five years with client contact information for up to three projects, and a summary of all proposed team members and subcontractors demonstrating their expertise and qualifications.
The proposal form requires extensive responsibility and capability inquiries, including certification of available financial, material, equipment, facility, and personnel resources necessary to meet contractual obligations. Proposers must disclose information regarding contract performance history within the last five years, any criminal or civil charges within the last three years, contract terminations, lawsuits, outstanding judgments, financial distress, bankruptcy filings, and current licensing and insurance status. A Pay Equity Certificate is required if the proposer employs 50 or more full-time workers and the contract price exceeds $500,000. The proposal constitutes a firm offer for 180 days following the due date. Multiple certifications are required at execution, including compliance with Oregon tax laws, non-discrimination in employment and subcontracting practices, sexual harassment prevention policies (for contracts exceeding $150,000), wage equity compliance, and verification that neither the proposer nor its agents appear on federal exclusion lists maintained by OFAC or in the System for Award Management.
