NOTE: If the company or division performing this effort is different than the offeror, or the relevance of this effort to the instant acquisition is impacted by any company/corporate organizational change, note those changes.
General Instructions
A. Provide the information requested in this form for each contract/program being described. Provide concise comments regarding your performance on the contracts you identify.
B. Limit the number of present/past efforts submitted to three (3).
C. Submit information on contracts that you consider relevant IAW the solicitation in demonstrating your ability to perform the proposed effort. Include rationale supporting your assertion of relevance and clearly demonstrate what aspects of this contract/program relate to the scope, size, and complexity of the proposed effort. If only a portion of the contract/program is relevant, make it very clear what portion you consider relevant in relation to the proposed effort.
Recency and relevancy are defined in the solicitation. In addition to the assertion of relevance, include any other information demonstrating customer satisfaction, quality of service, or performance metrics etc. related to the contract/program submitted.
