About this file

This is a Past Performance Questionnaire (PPQ) template for the Selective Service System's Readiness Simulation (RS) solicitation (90MC0026Q0005). The questionnaire is designed to be completed by references listed by offerors to evaluate a contractor's past performance on similar work. Responses must be submitted via corporate email to Contracting Officer Georgette Hendricksen (georgette.hendricksen@sss.gov) and Contract Specialist Cane Toussaint (cane.toussaint@sss.gov) no later than 2:00 PM Eastern Standard Time on August 14, 2026. If the reference contact is unavailable on that date, an alternate point of contact must be provided.

The questionnaire requires references to rate contractor performance across five key areas: (1) ability to meet requirements, (2) timeliness and issue resolution capability, (3) problem-solving effectiveness, (4) overall performance rating, and (5) likelihood of awarding a future contract. Ratings use a five-point scale (Exceptional, Very Good/Good, Satisfactory, Marginal, Unsatisfactory) plus a Not Applicable option. References must also provide details about the referenced contract including contract number, value, period of performance, and a brief description of services delivered. Detailed narrative comments are requested to support each rating. The questionnaire template includes spaces for the offeror's name and contact information, the reference contact's details, and supporting documentation fields.
