PERFORMANCE WORK
STATEMENT TO
PROVIDE PROPANE FOR CREECH AFB
1. DESCRIPTION OF SERVICES.
The contractor shall provide all personnel, equipment, tools, materials, supervision, and any other items necessary to service a quantity of propane tanks located throughout the installation as defined in this performance work statement (PWS). (Reference Appendix for current tank building locations). The majority of service shall be performed on Creech Air Force Base (AFB), NV (located approximately 40 miles north of Las Vegas along U.S. Route 95). Also, there is one (1) tank at Bldg. 2215 directly north of Creech AFB perimeter road, approximately three and a half miles out on Mile Range Road right before a chain link fence. (It is the only facility in the area). Further, there are two (2) tanks located at “Point Bravo” (approximately five miles south of the base). And finally, there is one (1) tank located at “Silver Flag Alpha” (SFA), (approximately 15 miles south of the base, directly across from the Lee Canyon exit). The contractor shall perform to the standards in this PWS.
1.1. BASIC SERVICES:
The contractor shall provide service to deliver propane to Creech AFB to meet the requirements of this PWS. This includes transporting propane to the installation and filling associated propane tanks at various locations. This is required to meet the hot water, and/or heating demands of each applicable building. Propane tanks size very from 250-gallon capacity, to upwards of 9,200-gallons. And, depending on facility size, shift-work, occupancy, etc., the propane consumption can be quite varied. That said, the contractor is “not” required to necessarily maintain each tank as “full”. However, the contractor shall develop a servicing schedule appropriate to ensure each tank maintains “a level of propane necessary to ensure continuous operation of associated equipment requiring propane” (e.g. – hot water heater, boiler, etc.). This includes each particular location/facility/building to preclude loss of heat at any time. Further, the contractor shall be able to respond to urgent mission-related delivery within 24 hours.
1.1.2. INVOICING:
Invoicing shall be performed “monthly” in the web-based Wide Area Workflow (WAWF). This is the government invoicing system that initiates electronic funds transfer (EFT) payment by the Defense Finance and Accounting Service (DFAS).
2. GENERAL INFORMATION:
2.1. SERVICE CALLS:
The contractor shall establish a service call system to include availability of phone, mail, electronic mail (e-mail) or fax. Emergency phone numbers shall be provided to the government contracting officer's representative (COR). The contractor shall have the capability to execute urgent requests within 24 hours.
2.2 CONTRACTOR AVAILABILITY:
The contract manager or alternate shall be available during normal duty hours to discuss requirements, problems or complaints.
2.2.1. PERFORMANCE EVALUATION MEETINGS:
The contracting officer may require the contractor manager to meet with the contracting officer, contract administrator, contracting officer representative (COR), and/or other government personnel as deemed necessary. As a minimum a meeting should be held quarterly. The contractor may request an exemption with the contracting officer when he/she believes a meeting is unnecessary.
3.1. LICENCES/PERMITS:
The contractor shall, at no expense to the government, obtain all licenses and permits required for the services identified in the PWS. The contractor shall be responsible for all damage(s) to persons or property that occur as a result of contractor negligence in connection with the work services, including spills.
3.2. SPILL RESPONSE:
The contractor shall provide a spill response plan with the proposal. The plan shall include procedures for spill(s) and shall comply with all state, federal, and Environmental Protection Agency (EPA) laws, rules and regulations for coverage of non-hazardous spills.
The contractor is responsible for the cleanup costs, including but not limited to the removal and remediation of soil and all required soil and/or water testing, of any propane spills caused by the contractor, its employees or agents, and/or any subcontractor of any level (and subcontractor employees/agents). All cleanup shall be in accordance and comply with all state, federal, and EPA laws, rules, and regulations.
3.3. PERSONNEL ACCESS TO CREECH AFB: Upon arrival at the gate, all commercial vehicles will be required to be searched by proceeding through the Automated Under-Vehicle Inspection System (AUVIS) and show current driver’s license, vehicle registration, and proof-of-insurance. Also, a pictured identification (such as driver’s license, etc.) will be required. The person will be required to identify himself/ herself and the firm he/she works for. This information will be verified against the Entry Authority List (EAL). Once verified, the person may then proceed to obtain their long-term pass. Personnel cannot show up at the gate without prior notification for access and expect to gain access. Verbal access cannot be provided.
3.4. LOCKS AND COMBINATIONS:
Government provided keys or lock combinations shall be issued upon award of the contract. The contractor shall control access to all government provided lock combinations to preclude unauthorized entry.
3.5. CONTRACTOR SECURITY RESPONSIBILITY:
The contractor shall be responsible for safeguarding all government property provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
3.6. FIRE PREVENTION:
The contractor shall strictly comply with installation fire regulations and become thoroughly familiar with applicable fire safety requirements. Fire prevention Technical Services: 702-404-2231.
3.7. SAFETY:
The contractor shall observe all safety regulations presently enforced at Creech AFB. All work shall be in accordance with all Occupations Safety and Health Act (OSHA) standards, and/or other applicable standards/mandates.
3.8. BASE ACCESS:
The contractor shall enter the installation through the Gate at Creech AFB Main Gate directly across U.S. Route 95N from the unincorporated town of Indian Springs, NV, located approximately 40 miles north of Las Vegas on U.S. Route 95, five miles north of Range 63C.
3.9. QUALITY CONTROL RESPOSIBILITIES:
The contractor shall develop and maintain a quality control program to ensure services are performed in accordance with commonly accepted commercial practices for the delivery of propane.
4.0. QUALITY ASSURANCE:
The COR will evaluate the contractor’s performance through random on-site inspections of the contractor’s quality control program and receipt of feedback from base personnel.
The government may inspect each task as completed or increase the number of quality assurance inspections if deemed appropriate due to repeated deterioration of services discovered during COR inspections. Or, due to repeat, negative customer feedback.
Likewise, the government may decrease the number of quality assurance inspections if performance dictates. The government will also receive and investigate feedback from various customers located on the installation. The contractor shall be responsible for initially validating customer feedback. However, the government representative shall make final determination of the validity of customer feedback in cases of disagreement with customer(s).
5.0 HOURS OF OPERATION:
Normal work hours are 7:00 A.M. to 4:00 P.M., Monday through Friday, excluding federal holidays. If the contractor desires, or is required, to work during periods other than above, the request shall be approved by the COR based on necessity.
