Asbestos Limited Survey Report
Clackamas River Water Attn: Vance Voyles vvoyles@crwater.com Project 23-56757
Subject: Asbestos Limited Survey at 9100 SE Mangan Drive, Clackamas, Oregon 97015.
Dear Mr. Voyles:
Alpha Environmental Services (Alpha) is pleased to provide you with the results of our asbestos limited survey at 9100 SE Mangan Dr., Clackamas, OR 97015. This survey conforms to the requirements of OAR 340-248-0270(3). According to the information provided, the existing structure was built in 1980, has been in commercial use since it was constructed as part of the wastewater treatment plant facilities. The reason for the survey is to prepare for upcoming remodel/renovations of the building and is limited to the portion of the building used as break room, office space and restroom.
Alpha conducted the investigation on October 12, 2023, when Alpha staff member Rodolfo Gomez, an AHERA certified inspector (Cert. no. 186804) performed the survey. A total of 12 sampling sections were collected from the existing structures. The sampling sections were placed in airtight containers, properly labeled, and sent with a Chain of Custody to SanAir of Virginia, an accredited laboratory.
Material identification was performed by SanAir using Polarized Light Microscopy with Dispersion Staining (PLM/DS) in accordance with the Environmental Protection Agency's (EPA) "Interim Method of Determination of Asbestos in Bulk Insulation Samples" (EPA-600-M4-82-020).

According to the EPA, any material analyzed under polarized light microscopy (PLM) that yields greater than 1% of asbestos qualifies as asbestos containing material (ACM).
A visual inspection was performed of the survey area; spot demolition was performed and the suspect asbestos containing materials observed were sampled according to standards. The analytical results indicate that asbestos was found above the guideline limit in the office space flooring (A11) sampling section. These materials are considered homogeneous; therefore, it is presumed that asbestos is present in other areas surveyed with the same floor tiles & mastic.
According to the EPA, ACMs that are intact and in good condition can be safely maintained in place until abatement or removal is dictated by deteriorating conditions and renovation or demolition activities. Alpha recommends following the EPA and Oregon DEQ guidelines for ACMs prior to any disturbance. Please note that in conformance with current State asbestos regulations, a copy of this report must be kept on site during all remodel/renovations, additionally if new suspect materials are encountered activities must be stopped, and the new suspect materials tested or treated as ACMs.
SanAir Technologies is accredited by the National Institute of Science and Technology (NIST) under the National Voluntary Laboratory Accreditation Program (NVLAP).
