About this file
This is a cost proposal template (Appendix C.3, Amendment 01) for the ARPA-H FASTPASS Innovative Solutions Opening (ARPA-H-SOL-26-160). The template does not address specific products or services required, as it serves as a standardized format for proposers to submit detailed cost estimates and supporting documentation for their proposed solutions.
The template requires proposers to provide comprehensive cost breakdowns across eleven categories: level of effort and labor costs (using burdened labor rates with estimated hours), itemized materials and equipment with quantities and unit prices (items exceeding $10,000 require backup documentation), travel details by purpose and destination, other direct costs, fully burdened consultant rates, sub-awardee costs with equivalent detail, and resource sharing arrangements. Proposers must also include task-based milestones aligned with timelines, potential cost reductions that do not impact research results, and value analysis explaining the return on investment and contribution to health outcomes. The template emphasizes that indirect costs and profit must be included in submissions, with profit broken out separately from fully burdened rates, and that all costs must be substantiated as fair and reasonable with supporting documentation such as vendor quotes, catalog price lists, or prior purchase invoices.

Cost Details Proposers should provide the following cost details and include supporting documentation that describes the method(s) and/or assumptions used to estimate proposed costs. The government expects indirect costs and profit to be included as part of cost proposal submissions. When discussing labor costs, proposers should only refer to burdened labor rates and identify and describe indirect rates only when they are being applied to other cost elements requested herein.
Level of Effort and Labor Costs
1.1 Provide the levels of effort and associated costs related to personnel who will contribute to the project. These levels of effort may be substantiated by defining key labor categories aligned to burdened labor rates with the assigned estimated number of labor hours (or percentage of effort).
1.2 A “burdened labor rate" means the base labor rate plus applicable labor overheads and fringe benefits only and is the rate that is referred to in the Cost Proposal Workbook(s). A “fully burdened labor rate” Fully burdened rates for each labor category should include both direct wages/rates and any indirect rates/costs such as, General & Administrative (G&A), Overhead, Fringe Benefits, Materials and Subcontracting handling rates, as applicable.is a labor rate that has all overheads, fringe benefits, other indirect costs (FCCM, G&A, etc.), and profit/fee represented. If profit is requested, profit should not be included in fully burdened rates and should be broken out separately. This rate is not used in the Cost Proposal Workbook(s).
1.3 Proposers should provide an explanation of any outliers and provide additional details that will allow the Agreements Officer to determine that costs are fair and reasonable. If the outlier cannot be substantiated by competitive pricing, the proposer should provide a written explanation of the value the government is receiving for the costs proposed.
Materials Provide an itemized list of all anticipated materials (including quantities, unit prices, proposed vendors (if known), and the basis of estimate for each (e.g., quotes, prior purchases, catalog price lists, etc.). Any item that exceeds $10,000 in total cost must be supported with back-up documentation (e.g., a copy of catalog price lists, past purchase order(s), invoice(s), vendor quotes prior to purchase, etc.). Include any assumptions the proposer has that it will receive government-furnished materials.
Equipment Purchases Provide an itemized list of all anticipated equipment including quantities, unit prices, proposed vendors (if known), and the basis of estimate (e.g., quotes, prior purchases, catalog price lists, etc.). Any item that exceeds $10,000 in total cost must be supported with back-up documentation (e.g., a copy of catalog price lists, past purchase order(s), invoice(s), vendor quotes prior to purchase, etc.). Include any assumptions the proposer has that it will receive government-furnished equipment.
Travel Provide the purpose of each trip, number of trips, number of days per trip, departure and arrival destinations, number of people, etc. Add rows as necessary.
Purpose
# of Trips
# of Days
# of People
Departure/
Destination Cost
Ex. PI Meeting
6
3
2
LA/DC
$10,000
Other Direct Costs (ODCs) Provide a breakdown of all anticipated ODCs (e.g., graduate student tuition, computer maintenance and/or user fees, etc.) with anticipated costs. The proposer may provide backup documentation to support anticipated costs, including an explanation of any estimating factors and their derivation and application.
Consultant Costs Provide fully burdened labor rates (e.g., hourly rates), labor hours, and any other anticipated consultant costs (e.g., travel).
Sub-awardee Costs For each proposed sub-awardee, provide the information requested in Sections 1-7 of this Cost Proposal. All documentation should provide the same level of detail and/or assumptions.
Resource Sharing Provide the source(s), nature, and amount(s) of proposed resource sharing. Include details of the Resource Share commitment, including percentages, nature of the contribution, cash or in-kind contributions, etc.
Potential Cost Reductions (Optional) Provide a written explanation of potential cost reductions that will NOT substantially impact the intended results of the effort.
NOTE: While the government seeks to eliminate unnecessary costs through value analysis, the government seeks revolutionary advances to the state-of-the-art and therefore, cost reductions should not have an impact on the proposer’s ability to deliver the desired research results.
Task, Milestones Schedule, and Pricing Under each identified task, provide a full list of milestones (aligned with TDD) and potential pricing for review.
Value Analysis Provide a brief written explanation of the value of this proposed effort and why American taxpayer dollars should be invested in it. Is there an expected return on investment for the American people and/or the Government? Does this effort contribute to accelerating better health outcomes? If this value can be determined in a quantitative manner, please provide that information here.
