About this file

This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017. The form serves as an informational guide for potential bidders on construction projects and outlines common bidding problems and disqualification criteria. The document is not a complete list of requirements for any specific project but rather highlights frequent issues encountered during the construction bidding process. All bidders are required to thoroughly review the complete request for quotation documents, addenda, and related instructions to understand actual bidding requirements specific to their project.

The form identifies errors resulting in immediate bid disqualification, including failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to supply valid bid bonds, non-compliance with mandatory RFQ requirements, failure to acknowledge addenda when required, late bid submissions, federal or state debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Additionally, the form lists errors that may result in bid disqualification prior to contract award, including debt owed to the state or political subdivisions, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to supply a West Virginia contractor's license number within one day of request, failure to submit a signed drug-free workplace affidavit within one day of request, and failure to provide a subcontractor listing within one business day of bid opening or DEP request.
