This is a Request for Quotation (RFQ) specification document issued by the West Virginia Department of Homeland Security, Division of Administrative Services, on behalf of Anthony Correctional Center & Jail (ACCJ). The solicitation seeks a one-time purchase of aggregated limestone materials for facility grounds maintenance. The contract requires three distinct items: 50 tons of Rip Rap Graded Limestone compliant with AASHTO standards (minimum gravity of 2.5, minimum density of 1.3 tons/cubic yard, and maximum 40% loss after 500 revolutions per AASHTO T96 testing); 350 tons of #4 Graded Limestone with size specifications of 1.5 to 2.5 inches and hardness between 3-4 Mohs; and 125 tons of #57 Graded Limestone sized between 3/8 to 1 inch with specific sieve passing percentages. The total solicitation encompasses 525 tons of material to be delivered to 313 Anthony Center Drive, White Sulphur Springs, West Virginia 24986. Vendors must complete delivery within ten working days of receiving a purchase order, with the facility responsible for identifying designated placement areas.

Pricing must be submitted on a per-ton basis with all costs—including delivery, fuel, and environmental fees—incorporated into the unit price; separate delivery charges will not be compensated. Award will be made to the vendor providing the lowest overall total cost while meeting all mandatory specifications. Vendors must accept orders through multiple communication methods including wvOASIS, mail, facsimile, and email. The state will process payment according to West Virginia payment procedures. Unacceptable items will be returned at vendor expense with full credit or replacement at the facility's discretion. Vendor default provisions include failure to meet specifications, non-compliance with applicable laws, and failure to remedy deficient performance, with remedies available including immediate contract cancellation and procurement from alternative sources.
