About this file
This is a Request for Quotation (ARFQ DCR2600000011) issued by the West Virginia Department of Homeland Security, Division of Administrative Services (DAS) on behalf of the Northern Correctional Facility for outdoor exercise equipment. The solicitation seeks to establish a contract for a one-time purchase of five specific exercise equipment items: a Spartan multi-gym, Versa Hi-LO Pulley System, Everest Cardio Climber, Apollo Multi-gym, and Cali-Rig 04. Each equipment item has detailed mandatory specifications including multiple exercise stations, specific dimensions, weight requirements, and features like instructional placards with QR codes for fitness programming.
The contract will be awarded to the vendor providing the lowest overall total cost for the contract items, with the pricing to be completed through wvOASIS or an attached Exhibit A. The estimated purchase volume represents approximate anticipated purchases, and no future use is guaranteed. Vendors must provide quarterly reports and annual summaries showing purchased items, quantities, and total dollar values. The contract includes provisions for vendor default, which could result in immediate contract cancellation, and requires vendors to maintain a primary contract manager available during normal business hours to address customer service issues.
