About this file
This is a Request for Quotation (RFQ) specifications document issued by the West Virginia Department of Homeland Security Division of Administrative Services on behalf of the Division of Corrections and Rehabilitations for an open-end radiation monitoring contract. The solicitation seeks vendors to supply approximately 165 PA Badges or equivalent area monitoring devices on a quarterly basis to various facility locations. The contract items must meet mandatory requirements including radiation monitoring via Luxel technology, U.S. accreditation standards, heat/moisture/pressure resistance, and include velcro holders with adhesive backing for surface mounting. Vendors must provide online access to paperless radiation dosimetry reports and submit annual NRC Form 5 Reports at no additional cost to the Agency. The contract is awarded to the vendor providing the lowest overall total cost for all specified items. The contract term is one year with four optional one-year renewal periods, with renewal requiring mutual written agreement between vendor and Agency.
Pricing must be submitted electronically through wvOASIS or as an electronic document, with vendors completing all pricing pages in their entirety or risking disqualification. No price increases are permitted within the first twelve months; thereafter, vendors may request adjustments no less than thirty days prior to contract anniversaries, limited to the lesser of prior year Consumer Price Index comparison or 3 percent, subject to Agency approval. Standard orders must be delivered within thirty working days of receipt with F.O.B. destination pricing included in bid costs; emergency orders must be delivered within fifteen working days. Vendors must maintain sufficient inventory, designate a primary contract manager, provide quarterly and annual purchase reports, and comply with West Virginia state confidentiality and information security policies.
