About this file
This is a checklist template for completed Reports of Investigation (ROI) for Equal Employment Opportunity (EEO) complaints. The checklist outlines the required components and organization of an EEO investigation report, including formal complaint documentation, EEO counselor's report, investigator's authority letter, organizational charts, testimony from complainants and witnesses, and supporting documentation.
The checklist details specific requirements for handling sensitive information, including redacting privacy-protected data like SSNs, contact information, and personal details of comparable employees. It prescribes a three-section organization structure: administrative complaint documents in chronological order, investigation materials (testimonies, declarations, memoranda) in logical order, and reference documents/regulations in chronological order. The document emphasizes proper handling of privacy information, maintaining chronological organization, and ensuring all exhibited regulations were in effect at the time of the alleged discriminatory events.
