About this file

This document is an appendix to a solicitation from the Wisconsin Department of Transportation (WisDOT) for a "bid opportunity list" as required by 49 CFR Part 26.11. The list must include all firms, both Disadvantaged Business Enterprises (DBEs) and non-DBEs, that bid on prime contracts, subcontracts, or provide materials and supplies for DOT-assisted projects. Prime contractors and consultants must complete all columns, while subcontractors and subconsultants may only have partial information available.

The key information requested includes the name of the bidder, their contact information, DBE status, age of the firm, type of work, and annual gross receipts. This information is intended to help WisDOT develop and maintain a comprehensive list of firms participating or attempting to participate in DOT-assisted contracts. Vendors must submit this form with all bids and proposals.
