About this file
This is a Past Performance Review Template for the FY25 Food for Peace NOFO applicants. The template is designed to document an applicant organization's prior performance on relevant awards or agreements. Part I requires the applicant to provide award information including the organization name, project name, award number, award type, award value, work description, performance location, and contact information for the contracting and technical officers. Parts II and III must be completed by the reference organization or personnel who directly supervised or managed the prior work, and the completed form must be submitted to ppded@usda.gov with the subject line "Past Performance Reference for [Proposal Name]." USDA/FAS may contact the reference organization for verification purposes.
The template evaluates performance across four assessment areas: instances of good performance measured by timeliness, consistency in meeting goals and targets, personnel effectiveness, cooperation, problem-solving, and cost control; instances of poor performance using the same metrics; significant achievements including beneficiary relationships, management of complex activities, partner coordination, and problem correction; and significant problems in these same areas. Applicants must complete Part I, noting N/A for items that do not apply, while reference organizations provide detailed performance commentary in Parts II and III to substantiate the applicant's capability to execute Food for Peace Title II projects.
