About this file

This document is an appendix of technical instructions issued by the State of Tennessee for conducting a network accessibility analysis as part of RFP #31786-00188 for Employee Assistance Program (EAP) and Behavioral Health Organization (BHO) services. Respondents must submit a Quest Analytics Network Access Analysis report using Quest Analytics Suite Version 2025.2 to demonstrate their provider network's capacity to serve Tennessee's public sector health plan members across the state. The analysis requires respondents to submit provider data files containing provider names, addresses (including separate records for multiple office locations), cities, states, ZIP codes, specialty codes, and patient acceptance status in formats compatible with Quest Analytics (Access, Excel, or Text). The state has provided Tennessee member data organized by ZIP code in Microsoft Excel format to accompany the RFP. All network accessibility analyses must be conducted for the entire State of Tennessee and include a cover page, contents page, access summaries by county per ZIP code classification, provider location maps by provider type, and detailed access summaries identifying members with and without desired access.

The analysis must adhere to strict data standards and specifications, including geocoding provider addresses at the address level using estimated driving distance calculations rather than straight-line distances, classifying providers by primary specialty only, and including only current contracted providers delivering services during the measurement period within Tennessee who will remain contracted on January 1, 2027. Respondents must analyze network accessibility across three geographic classifications—urban, suburban, and rural—against defined access standards for eight provider groups and facility types, with required access varying by geography (for example, psychiatrists must be within 10 miles in urban areas, 15 miles in suburban areas, and 30 miles in rural areas). The state will accept detailed written attestations as exceptions to access standard deficiencies solely attributable to the lack of available licensed providers or facilities in specific areas. Respondents are responsible for ensuring accuracy of their analyses, including verification that company names and report dates appear on title pages, provider classifications match state requirements, and total enrolled member counts match exactly the numbers contained in the state's provided ZIP code counts appendix.

APPENDIX 7.2

Quest Analytics Analysis Instructions

Quest Analytics Behavioral Provider & Facility Network Accessibility Analysis

RFP #31786-00188

Data Requirements & File Layouts

The State would like to know the potential of your organization’s provider network to service the accessibility needs of its members who choose to enroll in the Public Sector’s Health Plan. To measure that potential, this RFP requires each Respondent to submit a Quest Analytics Network Access Analysis report. The network’s relative potential will be measured by 1) the current number of providers in the network and practicing; and 2) the average distance to providers for all Plan Members within Tennessee.

Data Items To analyze the accessibility of Respondent’s provider network for the State’s population, the Quest Analytics Suite requires both an employee data file and a provider data file. Contractor shall use Quest Version 2025.2 to run this report. Tennessee Member data, listed with tallies by ZIP code and ZIP code classification are included in a file accompanying the RFP. The file is in Microsoft Excel format and has also been provided to Quest Analytics. Your provider files must contain the following

• Provider Name (Last Name, First Name)

• Street Address – NOTE: For Network Providers who have more than one office location, the file should contain a distinct record for each office address.

• City

• State

• Five-digit ZIP Code

• Specialty Code – please include the coding scheme used

• Accepting/Not Accepting Patients Practice Indicator (A=Accepting, N=Not Accepting)

Data Format

If you intend to have Quest Analytics conduct this analysis, you must submit your data to Quest Analytics in one of the following formats: Access, Excel, Text (Fixed Width or Delimited) and send electronically.

If you have the Quest Analytics Suite™ and need technical support on how to utilize the software, the team can be contacted at support@questanalytics.com or 920.739.4552. If you need Quest Analytics to perform the analysis on your behalf, please contact consulting@questanalytics.com.

Standards and Specifications

The standards and specifications listed are to be followed by all respondents. This will enable the State to evaluate, on a consistent basis, the accessibility of the Respondent’s provider network. It is critical that the analysis meet the following data standards and report specifications.

A. Analysis Requirements

1. Use the Quest Analytics Suite™ Access Analysis and Geocoder Module most current update. If you do not have the Quest Analytics Suite™, call 920.739.4552 or email consulting@questanalytics.com at Quest Analytics to receive a quote for them to perform the analyses for you.

B. Data Standards

1. Use all employee ZIP codes data contained in APPENDIX 7.4 - TN ZIP Code List, to analyze your network relative to the State of Tennessee member population within Tennessee.

2. Your Network Provider addresses should be geocoded at the address level. For any Network Provider’s address that cannot be exactly geocoded, the zip-distributive address points can be used, which takes into account population patterns to distribute locations. Placing providers at ZIP code centroids or randomly within ZIP codes is not permissible.

3. Distance is to be calculated based on Estimated Driving Distance, not on an “as-the-crow-flies” distance.

4. If more than one provider is located at the same address, all providers at that address should have the same geographic coordinates.

5. Providers should be classified based on their primary specialty only.

6. Analyses should include all current providers in your network which are delivering services during the measurement period within Tennessee only.

7. Providers who are currently contracted, but will not be on January 1, 2027, shall not be included in your analysis.

8. Providers who are not currently contracted shall not be included in your analysis.

C. Report Specifications

1. Prepare a network accessibility analysis for the entire State of Tennessee. The report should closely resemble the Sample

Analysis contained in APPENDIX 7.3 - Network Access Analysis Sample Report and should include:

• Cover page

• Contents page

• Access Summary by County - per Zip Classification

• Provider Locations Map - by Provider type

• Access Summary by County - by Provider type

• A complete set of reports for the network for the provider types in each location category listed below (#2), and as described in the table at the bottom of this page; and

2. Conduct an analysis for the network for urban, suburban, and rural employee access to providers, with an access standard as shown in the boxes below. The State will accept a detailed written attestation as an exception to any access standard deficiency that is due solely to a lack of available licensed providers or facilities for which to contract in a specific area(s).

Page Content/Display Cover Report title, Proposing organization’s name, Date

Contents Table of contents listing all analysis pages included.

1 Access Summary by County – Per Zip Code Classification (report page includes Members with and without desired access): Summarize by key geographic areas with and without desired access, the required Access analysis specifications and Average Distance to the 1st and 2nd closet Network Provider Group. There should be 1 access summary page by county, per zip code classification, resulting in 3 report pages (Urban, Suburban, and Rural.)

Plot providers on a map of the State of Tennessee, with counties labeled, resulting in 1 map page per provider type.

Access Summary by County (separate report pages for Members with and without desired access): Detail the Average

Distance to the 1st and 2nd closest for each provider group above for Members with Desired Access and without Desired Access summarized by county, including Zip Code Classification on the report.

Check the Statistics Page Option when generating this page. (This is a summary page detailing, among other things, the report creation date, the software version, analysis inputs and calculation method.)

Checks for accuracy: Respondents are responsible for ensuring their Quest Analytics Analyses are accurate. The following items should be reviewed for accuracy:

b. Accessibility analysis specifications:

• Provider Groups – Providers should be classified by their primary specialty and shall not be counted in multiple categories.

• Enrolled Member Group – The total number of State enrolled members MUST MATCH EXACTLY the number contained in APPENDIX 7.3 - TN ZIP Code Counts.

• Access Standard – The Analysis report MUST MATCH EXACTLY standard required by the State (see page 2 of these instructions)
