About this file
This document is an Appendix 5 Housing Search Needs and Preferences Assessment form used by the Massachusetts Rehabilitation Commission (MRC) as part of the Money Follows the Person (MFP-Demo) Demonstration grant, the Acquired Brain Injury (ABI) and Moving Forward Plan (MFP) Home and Community Based Services (HCBS) Waivers, and other MRC state-funded programs. The form is used to assess housing preferences and potential barriers to housing access for program participants in order to assist the Housing Search Entities (HSEs) in planning housing searches. The form covers preferences for neighborhood type, public transportation, accessibility features, amenities, and other housing characteristics, as well as potential barriers to housing access such as criminal history, evictions, and poor credit.
The related state and local contract opportunity is for the MRC to create a pool of qualified HSEs to perform specific housing search tasks to assist in finding community-based housing for adults with disabilities and elders served by the aforementioned MRC programs. The contract opportunity is open to qualified entities in the state of Massachusetts, but does not provide any details on quantities, response dates, award dates, or contract terms.

Top Priorities When finding housing, it is not usually possible for all preferences to be met. The ideal setting is a match between available options and the most critical priorities. Below, identify the top three priorities. For example, 1) my own place, can be an efficiency or one bedroom; 2) need to live in either Malden or Revere area so I am near family; and 3) grocery store has to be within “walking” distance.
