About this file

This is an Annual Report Template for the City of Milpitas' Industrial and Commercial Site Controls Program, which supports compliance with Provision C.4 of the Municipal Regional Stormwater Permit (MRP), Order No. R2-2022-0018. The template establishes reporting requirements for facility inspections, enforcement actions, staff training, and non-stormwater discharge documentation across industrial and commercial sites within the city's jurisdiction. Permittees are required to document the total number of inspections conducted, enforcement actions resolved within 10 working days or in a timely manner, and any violations requiring extended resolution periods. The template mandates reporting on enforcement action levels (Level 1 through Level 4), categorized by business type, and requires permittees to align enforcement responses with their established Enforcement Response Plan (ERP). Additionally, the form requires comprehensive documentation of staff training activities, including mandatory annual inspector training covering six core topics: urban runoff pollution prevention, inspection procedures, business inspection plans, enforcement response plans, illicit discharge detection and elimination (IDDE), and appropriate best management practices (BMPs) for industrial and commercial facilities.

The template serves as a standardized reporting mechanism for municipal stormwater programs under the regional permit framework administered by the San Francisco Bay Regional Water Quality Control Board. All data fields must be completed without exception, with detailed explanations required for any enforcement actions or potential and actual discharges not resolved within 10 days or resolved through extended but timely processes. The template accommodates multiple reporting methodologies, allowing permittees to report either as discrete enforcement actions per site or as total numbers of potential and actual discharges. Training documentation must specify attendance numbers and percentages for both industrial and commercial site inspectors and IDDE inspectors, with flexibility to include program-wide, region-wide, and permittee-specific training events. The template reflects the City of Milpitas' commitment to comprehensive stormwater quality protection and inspector accountability under the Municipal Regional Permit framework.
