About this file

This is a cost form attachment for the City of Milpitas' Request for Proposals (RFP) for Stormwater Inspection Services at Commercial and Industrial Facilities in California. The RFP seeks contractor services to support the City's Industrial and Commercial Site Controls Program as required under the Municipal Regional Stormwater Permit (MRP), Order No. R2-2022-0018, issued by the San Francisco Bay Regional Water Quality Control Board on May 11, 2022. The contract term extends up to five years. The cost form requests vendors to provide hourly rates across five years for the following tasks: Principal Engineer and Associate Engineer hours for meetings and correspondence with staff; Principal Engineer and Associate Engineer hours for program management, data management, materials preparation, notification letters, inspector packet scheduling, field issue coordination, and quality control/quality assurance inspection data management; Associate Engineer hours for inspections and re-inspections at commercial and industrial facilities; and Associate Engineer hours for assistance with follow-up to IDDE (Illicit Discharge Detection and Elimination) reports to the City.

Pricing submissions must include per-unit hourly rates for each service category across all five contract years, with evaluation based on the Total Base Bid amount. Unit prices may not increase by more than three percent over the prior year's hourly rate. Vendors must provide authorized signatures, printed names, and titles from an authorized signer. The cost form notes that evaluation points for cost will be determined using the Total Fee Estimate as the Total Base Bid for RFP evaluation purposes only. The City reserves the right to invoice only for actual services rendered and may not require all specified services. In the event of calculation errors, the unit price will govern over calculated totals.
