About this file

This is an Annual Report Template for the City of Milpitas' Industrial and Commercial Site Controls Program, which operates under Provision C.4 of the City's Municipal Regional Stormwater Permit (MRP), Order No. R2-2022-0018 NPDES Permit No. CAS612008, issued by the San Francisco Bay Regional Water Quality Control Board on May 11, 2022. The template serves as the reporting mechanism for the City's stormwater inspection services at commercial and industrial facilities, which are procured through a separate Request for Proposals for a term of up to five years. The annual report form requires documentation of program highlights, facility inspection activities, enforcement actions taken, types of enforcement conducted, frequency of non-stormwater discharges by business category, and staff training summaries. Permittees must report the total number of inspections conducted, the number of enforcement actions or discrete discharges resolved within 10 working days or in a timely manner, and categorize enforcement actions by level (Level 1 through Level 4) as defined in their Enforcement Response Plan.

The report template mandates comprehensive tracking of both actual and potential non-stormwater discharges organized by business category, with no blank cells permitted in any data tables. Annual staff training documentation is required for all industrial and commercial site inspectors and IDDE inspectors, covering a minimum of six mandatory topics over the five-year permit term: urban runoff pollution prevention, inspection procedures, business inspection plans, enforcement response plans, illicit discharge detection and elimination, and appropriate best management practices for different facility types. The guidance allows for flexible training formats including program-wide, region-wide, or permittee-specific sessions, contractor training, tailgate safety meetings, document reviews, video training, and attendance at outside events such as conferences.
