About this file
This is Addendum Number 3 to a Request for Proposal issued by the Wisconsin Department of Transportation (WisDOT) for digitizing microforms and providing indexing services. The solicitation, Bid No. 510019, requires proposers to convert microforms in various formats—including microfilm reels, microfiche sheets, slides, and aperture cards—into digital images with proper saving, storage, naming, and indexing protocols. The addendum, posted on June 1, 2017, provides scanned examples of microfiche and aperture card layouts that were unavailable when Amendment 2 was released, specifying that DMV microfiche contains 208 images per sheet (16 wide by 13 tall) and noting that COM-generated sheets have 269 images while jacket fiche contain 54 images per sheet. Proposals were due by June 9, 2017 at 2:00 PM Central Time, and bidders were required to sign and return all amendments with their proposals. A sample is required as part of the solicitation response per section 1.3.3 of the RFP.
No specific pricing terms, contract duration, award dates, funding amounts, or other financial details are provided in this addendum. The document functions primarily as a clarification and information update to the original solicitation, ensuring that prospective bidders have access to representative examples and technical specifications necessary to prepare accurate proposals for the digitization project.
